About this file

This document is a Vendor's Participant List form for a solicitation funded by federal assistance through the U.S. Department of Transportation, specifically involving Broward County and the Fort Lauderdale-Hollywood International Airport (FLL). The form is designed to establish and maintain a database of firms participating or seeking to participate in federally assisted contracts, with a focus on Disadvantaged Business Enterprise (DBE) and Airport Concession Disadvantage Business Enterprise (ACDBE) Program requirements.

The form requires detailed information from prime vendors and their subcontractors, including federal tax ID numbers, firm names, contact details, annual gross receipts, and demographic information about the majority owner. Vendors must specify their type of work, contract amount or percentage, and applicable NAICS codes. The document emphasizes compliance with federal regulations, noting that vendors must submit the completed form with their submittal or within three business days of the county's request, with potential non-responsiveness consequences for failure to comply.
