About this file
This document is a Master Agreement Respite Services Evaluation Tool Worksheet for a Request for Applications (RFA) issued by the Massachusetts Department of Mental Health (DMH). DMH is seeking to establish a pool of qualified providers from which DMH Areas can purchase Site-Based Respite and/or Mobile Respite Services on an as-needed basis. Applicants must submit separate applications for on-site and mobile respite services, with both electronic and hard copy submissions required prior to March 31, 2027. DMH will evaluate applications based on criteria across six categories, including the bidder's background, staffing, service model, scenarios, site locations, and quality management. Applicants must receive an overall "good" rating to be deemed qualified for a DMH site office or location.
The document outlines detailed application requirements and evaluation criteria, but does not provide specifics on pricing terms, set-asides, incumbent providers, funding sources, or other financial details. The RFA is focused on establishing a qualified provider pool for DMH's future purchase of respite services on an as-needed basis.

APPLICATION REVIEW
For the purpose of evaluating the Application, the Application has been clustered into six (6) different Categories as indicated below. There are two steps to evaluating the Application. The first step is to assign each Application category a rating using the following criteria:
Excellent (E) – The Application is comprehensive, feasible and appropriate. It satisfies all RFA requirements and exceeds many of the requirements. No weaknesses are identified. It demonstrates an excellent understanding of the services to be provided and the issues/barriers that a Contractor may need to address when providing services. It exceeds the needs of DMH.
Very Good (VG) – The Application is comprehensive, feasible and appropriate. It satisfies all RFA requirements and exceeds some of the requirements. Identified weaknesses, if any, are minor. It demonstrates a good understanding of the services to be provided and the issues/barriers that a Contractor may need to address when providing services. It exceeds the needs of DMH.
Good (G) – The Application is comprehensive, feasible and appropriate. It satisfies all the RFA requirements. It has weaknesses and/or does not exceed any specifications. Identified weaknesses, if any, are minor. It demonstrates a reasonable understanding of the services to be provided and the issues/barriers that a Contractor may need to address when providing services. It meets the needs of DMH.
Fair (F) – The Application is not fully comprehensive, feasible or appropriate. It satisfies most, but not all of the RFA requirements. Weaknesses are identified; they may or may not be substantial. It demonstrates a reasonable understanding of the services to be provided and the issues/barriers that a Contractor may need to address when providing services. It does not meet the needs of DMH.
Poor (P) – The Application is not fully comprehensive, feasible or appropriate. It does not satisfy most of the RFA requirements. Weaknesses are identified and they are substantial. It does not demonstrate a reasonable understanding of the services to be provided and the issues/barriers that a Contractor may need to address when providing services. It is not responsive to the needs of DMH.
The second step is to give the Application an overall rating based on the ratings given to each Category and the level of importance (weight) assigned to each Category as indicated below. If an Application has been submitted for more than one location (Site-Based) or DMH Site Office (Mobile Respite), then the Applicant will receive an overall rating for each location/Site. The overall rating will be calculated by using the same scores for Categories I, III and VI for all and using the location/site specific scores for Categories II and IV and V. Applicants were instructed to provide different responses to Categories II and IV; one each for each location/site. To be deemed qualified for a DMH Site Office or location an Applicant must receive an overall rating of “good” for its Application for these locations and sites.
