RE: FINAL Hazardous Material Evaluation/Abatement Recommendation Report Lake I\,lead Lodge Discontinuation & Adaptive Reuse Study Lake Mead National Recreation Area PIVOS Number 150265 Contract Number C2000091400 Task Ordet· Number T2011101100 MACTC Project# 4666100006.07

Dear Ms. Dey:

MACTEC Engineering and Consulting, Inc. (MACTEC) was requested to perform an asbestos survey and limited lead-based paint survey at Lake Mead Lodge \Vhich consists of four buildings located in the Lake Mead National Recreation Area east of Boulder City, Nevada. MACTEC perfrwmed the inspection and sampling on April 6 and 7, 2010. MACTEC also per formed a walk-through of the building to identify potential hazardous material issues within the building.

The work was performed in accordance Contract No. 1443C20000091400, Task Order No;

T20l 1101100.

The Lake Mead Lodge Complex consisted of the following buildings:

Building Description Estimated Size Bllilding A Annex building - Lodge 2,500 square feet

Rooms 1-8 BuildingB Lodge Rooms 1-19 4,000 square feet Building C Main Office and Suites 6,000 square feet

Building D Lodge Rooms 20-37 4,000 square feet

Asbestos Survey

Prior to surveying and sampling the buildings, Ms. Roxanne Dey and an anned ranger perfom1ed a walk-though of the buildings to insure no vagrants occupied the buildings. Ms. Dey indicated the bui I dings had been abandoned for approximately one year. She was not aware of past abatement activities. Ms. Dey also informed the inspector that rat nesting and droppings had been observed and that precautions for hanta virus were necessary within the building.

Consequently prior to entrance into the buildings, the inspector donned a respirator tyvek suite, MACTEC Engineering and Consulting1, Inc.

Ms. Roxanne Dey Lake Mead National Recreation Area July 21,2010 disposable rubber gloves, and disposable rubber boots. The inspector also reviewed the job hazard analysis (HIA) for the site prepared by MACTEC.

On April 6 and 7, 2010, Torn Colwell , a certified asbestos inspector performed inspection and sampling of identified homogeneous sampling areas at the subject site. The goal of the survey and sampling was to identify suspect asbestos containing materials (ACM) and sample those materials to identify the presence of ACM within the building. The inspector conducted a reconnaissance of the building's visible and readily accessible areas to identify potential ACM.

The inspector did not disassemble equipment or demolish structural and or finished materials to collect samples.

Homogeneous sampling areas contain suspect materials that appear by texture, color, and wear to be uniform and appear to have been applied or installed during the same general time period.

Once homogeneous sampling areas were identified, MACTEC collected representative samples of identified homogeneous suspect materials in general accordance with Asbestos Hazard Emergency Response Act (AHERA) requirements.

Once collected, the samples were shipped to Reservoir Environmental Inc. (Reservoir) for laboratory analysis. Reservoir is a National Voluntary Laboratory Accreditation Program (NVLAP) certified laboratory. Reservoir utilized the EPA recommended method of polarized light microscopy (PLM) with dispersion staining to analyze the samples and report the type and percent concentrations of asbestos. PLM utilizes the unique optical crystallographic properties of asbestos for identification. A minimum of 10% quality control samples were collected by utilizing duplicates of selected samp1es to confirm laboratory accuracy. Duplicate samples are denoted with a "D' at the end of the sample designation and were included in the laboratory analysis. Duplicate samples were found to be consistent with the original samples.

Table 1 depicts the identified and sampled homogeneous areas, location, and description of the material , condition and reported laboratory analytical results for the material. Homogeneous Area Definition Sheets detailing the identified homogeneous areas are included in Attaclm1ent A.

Laboratory analytical results for the collected samples are presented in Attachment B. Photos of various identified and sampled materials are presented in Attachment C. Certification documentation for the asbestos inspector is included in Attachment D.

Results of the asbestos survey and sampling indicated the presence of ACM in the following materials:

• 9-inch by 9-inch floor tiles - Buildings A and C

• l-foot by 1-foot floor tile - Buildings A, H, C, and D

• Domestic water white corrugated paper pipe insulation and mud fittings - Buildings B and D.

• Bathroom floor linoleum - Buildings A, B, C, and D.

• Spray-on acoustical ceiling spray - Building C, conference room.

The remaining tested materials did not indicate the presence of asbestos in the collected samples.

It should be noted that a single sample is adequate to define a homogeneous material as asbestos containing. However to define a homogenous material as asbestos free requires asbestos less than 1 percent for the entire representative sampling set.

July 21, 2010

Asbestos Abatement Estimated Costs Typical abatement costs are provided to give a general idea of potential abatement costs based upon quantity of various types of materials. For a more accurate cost, an actual cost estimate should be obtained from a licensed abatement contractor.

Material Descri2tion Price Range/sguare feet(ft:2) Average Cost (ft2) Vinyl Floor Tile $1.00-5 .75 $3.40

Vinyl Floor Tile $2.00- 7.00 $4.50 With mastic

Thennal Systen.1 insulation $12.00 - 30.00 $22.50 On p]pes with fittings

Glovebagging $13 .00- 30.00 $21.50/linear foot

Spray-on material $12.00- 30.00 $22.50

(flat surface) Exterior Surfacing Material $3.00 - 12.00 $5.00

Drywall $6.00 - 12.00 $9.00

Other costs may be involved such as mobilization costs, pem1itting, containment, placement of scaffolding as needed, and air monitoring costs.

Lead-Based Paint Sampling

During the April 6 and 7, 2010 inspection, MACTEC identified pre-dominant paints on the interior and exterior of the four buildings making up Lake Mead Lodge and collected chip samples for analysis for lead-based paint (LBP). LBP is defined as paint or other surface coatings that contain lead equal to or in excess of 1 .0 milligrams per square centimeter or more than 0.5% by weight. The goal of the limited paint sampling was to provide data regarding the scope of LBP within the facility. MACTEC identified and sampled a total of sixteen ( 16) pre dominant paints and collected paint chip samples from those locations . Paint chip samples were delivered to Reservoir and analyzed for lead by atomic absorption spectrometry (AAS) (an EPA preferred method) . Reservoir is a member of the National Lead Laboratory Accreditation Program (NLLAP).

Table 2 depicts the identified pre-dominant paints, location, and description of the material, condition , and laboratory analytical results for the material. The laboratory analytical report is included in Attachment B. Photographs of select paints sampled are included in Attachment C.

One of the 16 tested paints was defined as LBP. The paint defined as LBP was collected from a blue tnm and post paint located on the exterior of Building D (Bldg D# LPA). That sample indicated a lead concentration of 1.91 %. The remaining 15 paint sample locations collected were found to be less than 0.5% lead and not considered to be LBP.

July 21, 2010

If a more thorough evaluation of the LBP in the building is required, an evaluation of the building can be performed using an x-ray fluorescence (XRF) unit to identify LBP in the facility. An XRF survey is an exhaustive search of all surfaces within all rooms of the buildings. The XRF provides on-site results of lead content.

Hazardous Materials Survey

During the April 6 and 7 2010 site visit, MACTEC perfom1ed a walk-through of the building and associated property to identify potentia l hazardous materials which might be a concern to the National Park Service. Photographs of noteworthy conditions are included in Attachment C.

MACTEC reviewed a Phase I Environmental Site Assessment (ESA) report provided by the National Park Service. The Phase I ESA was developed by Kleinfelder and dated August 27, 2008 (Project #96034.1 ). The Phase 1 ESA Teport included a cover letter, photographs, and a radius report. The report did not include text or conclusions indicating the identification of recognized environmental conditions (RECs). The photos and environmental radius reporl were reviewed. The photos indicated storage of chemica1s in the main building (Building C). At the time of the April 6 and 7, 20 IO inspection, those chemicals had been removed from the facflity.

A review of the radius report indicated no regulated findings within the ASTM l 527-05 prescribed search distances.

During the April 6 and 7, 2010 walk-through, the following conditions were observed and deemed note, vorthy:

• Pad-mounted transformers were observed behind each bui1ding. Transfonners can contain PCBs. No labeling denoting PCB content was identified on the b·ansformers and based on age and appearance, they are assumed to contain PCBs. The following transformer designations were observed:

Buildi11g Location Transformer Deshmatious Building A - Annex Behind Building AA-106

Building B-Rooms 1-19 Behind Building AA-107 & AA-257 Building C - Main Office Behind Building AA-194 & AA-200

Building D - Rooms 20-37 Behind Building AA-48 & AA-216

• Friable asbestos was noted in the maintenance rooms of Buildings Band D. The friable asbestos was in poor condition in these moms. If contractors are to perform work in these or other rooms in Buildings Band D, they should b e notified of the presence of friable asbestos and not to disturb these materials. Additionally, prior to any proposed demo! ition or renovation, abatement of ACM must be performed.

• Fluorescent lighting is typicaHy associated with lamp ballasts. Lamp ballasts can contain PCBs. Additionally, used light tubes can contain mercury. Used ballasts and 1ight tubes should be disposed of by using approved recycling or disposal vendors. Lamp ballasts and light tubes were observed in Building C. These materials will require specialized handling and disposal.

July 21 , 2010

• 1-fanta virus is a disease associated with animal waste . Evidence of animal intrusion into the buildings was noted .specifically in Building C. Precautions should be taken to reduce exposure to these animal wastes . These precautions should include respiratory protection· eye shielding; and disposable outer clothing including tyvek, gloves, and booties.

• Evidence of drug cooking was observed in Room 7 of Building B . Vapors ·from drug making activities may impact permeable surfaces within the room.

Conclusions

On April 6 and 7 2010 MACTEC perfonned an asbestos survey, lead paint chip sampling, and hazardous materials survey of the Lake Mead Lodge complex located in the Lake Mead National Recreation Area. ACM was identified in flooring materials, pipe insulation, and sprayed-on acoustical ceiling material. Prior to implementation of renovation or demolition activities, abaten1ent of identified ACM will be required. LBP was identified in one of the collected samples on Blue exterior paint on Building D. The remaining collected paint chip samples djd not indicate the presence of LBP. Hazardous mateda]s were identified including PCB containing pad-mounted transformers and light ballasts, hanta virus concerns and evidence of drug making paraphernalia. Specialized handling of identified hazardous materials would be required in the event of a renovation or demolition of the complex.

Thank you for allowing MACTEC to assist you with this project. If you have any questions please contact Tom Colwell at (303) 293-6053.
