About this file

This is a Past Performance Questionnaire (PPQ) template used by the Department of Interior to evaluate contractor past performance during source selection for federal contract opportunities.

The form collects structured evaluations from government officials or project stakeholders who have direct experience with a contractor's performance on previous contracts. Evaluators provide firm information including project title, location, contract number, contract dates, and initial and final amounts. The evaluator must identify their name, organization, address, email, and role in relation to the project. The questionnaire uses a five-point rating scale (Outstanding, Above Average, Satisfactory, Marginal, and Unacceptable) to assess four primary performance areas: relationship with the client's contract team, overall corporate management and integrity, quality control, and ability to meet the performance schedule. Additional yes/no questions address whether cure notices, show cause letters, reprimands, payment suspensions, or terminations were issued; whether the evaluator would award another contract to the contractor; customer satisfaction with final product and services; and whether the contractor was given opportunity to respond to negative comments. The form concludes with an overall rating and space for supporting comments, followed by evaluator signature and date fields. The document includes a notice to contractors restricting use of this information for promotional purposes or endorsements, citing FAR 31.205-1 and DIAR 1452.203-70.
