About this file
This is a Past Performance Questionnaire (PPQ) template used by the U.S. Department of the Interior, National Park Service to evaluate contractor past performance for solicitation opportunities.
The document establishes a two-part process where offerors complete Section A with project details including project title, location, description, company name, Unique Entity ID/CAGE Code, owner/project manager information, and contract dates and amounts. The offeror then submits the questionnaire to a project owner/evaluator who completes Section B, providing ratings and assessment information. The completed questionnaire must be returned to the offeror for inclusion in their proposal package and submitted to the contracting point of contact by the deadline specified in the solicitation. Offerors may submit between 3-5 past performance questionnaires, with all projects required to have been completed within five years of the evaluated contract completion date. Late submissions will not be accepted, and offerors should account for potential electronic transmission delays.
Section B requires evaluators to rate contractor performance across ten dimensions using a five-point scale (Outstanding, Above Average, Satisfactory, Marginal, or Unacceptable). Evaluation criteria include the company-client relationship, corporate management and integrity, quality control, schedule performance, any corrective actions issued, likelihood of future contract award, customer satisfaction with deliverables, and opportunity provided to the contractor to respond to negative comments. Evaluators must provide an overall rating and may include additional explanatory comments, particularly when ratings are Marginal or Unacceptable. The evaluator signs and dates the completed form.
