SECTION 5- INSTRUCTIONS

FAR 52.212-1 – Instructions to Offerors – Commercial Items. Addendum.

5.1 SUPLLEMENTAL INSTRUCTIONS- PREPARATION OF RFQ

(a) FAR Provision 52.212-3, Offeror Representations and Certifications- Commercial Items (Jan 2021) provision is a part of the Representations and Certifications that are submitted and maintained on the System for Award Management (SAM) website. Firms shall ensure that the Representations and Certifications are up to date in SAM. Potential awardees SHALL have an active SAM registration prior to award. The Government shall obtain a firm’s representations and certifications through this system to verify the firm’s Business Size Status and NAICS. Firms are not required to submit a copy of their representations and certifications but must submit contractor certifications for FAR 52.204-24, Representation Regarding Certain Telecommunications and Video Surveillance Services or Equipment and 52.204-26, Covered Telecommunications Equipment or Services- Representation.

(b) To assure timely and equitable evaluation of quotes, firms shall strictly comply with all instructions within this RFQ to ensure submission of a complete quote. Firms are required to meet all RFQ requirements, including terms and conditions and those identified as evaluation factors. Failure to furnish a complete quote, at the time of submission, may result in the quote being unacceptable to the Government and eliminated from consideration.

(c) This RFQ does not commit the Government to any costs incurred in the preparation and submission of a quote or for any other costs incurred by any firm submitting a quote in response to this RFQ.

(d) Quotes must be valid for a minimum of 90 days.

5.2 VOLUME FORMAT INSTRUCTIONS

5.2.1 Volumes

Volumes must be submitted separately and clearly marked. Volume I shall be your Non-Price response to Factor 1 and Volume II shall be your price quote in response to Factor 2. Each volume must contain a cover page with the following information: (1) Prime Contractor’s Name, DUNS, Contact Information (Name, Phone, Email, Address) (2) Volume Number & Title (3) RFQ Number.

5.2.2 Format and Page Count

There are no page limitations or font size/type requirements. Pages must be consecutively numbered.

5.3 INSTRUCTIONS FOR VOLUME I; FACTOR 1- REQUIREMENT CAPABILITIES AND

COMPLIANCE

The Volume I, Factor 1 shall consist of the following:

• A demonstration, through a brief written narrative, that the firm is capable of proving the materials, as listed on Attachment 1, Section 2.1.

• A written estimated shipping time and an estimated delivery date (within the delivery time specified in Attachment 1, Section 2.2) to the listed Delivery Locations in Attachment 1, Section 2.2.

• Submission of a Standard Commercial Warranty.

• Completed Provisions for FAR 52.204-24, Representation Regarding Certain

Telecommunications and Video Surveillance Services or Equipment and 52.204-26, Covered Telecommunications Equipment or Services-Representation. These provisions are included in Attachment 4.

o Factor 1 quotes may not contain pricing information.

o All subcontractors, suppliers, manufacturers must be disclosed.

5.4 INSTRUCTIONS FOR VOLUME II; FACTOR 2, PRICE

• The Quote shall include filled in pricing information on Attachment 2, Price Schedule.

• The quote shall include a firm fixed price for all items.

• The quote must include a not to exceed delivery/shipping/freight price. This price must be separately quoted for each Order, as specified on the Attachment 2 Price Schedule.

• The government is exempt from taxes.
