About this file

This is an Indoor Air Quality Investigation Report prepared by Summit Environmental Services, Inc. for the U.S. Fish and Wildlife Service at their facility located at 10847 Green River Road #2 in Spotsville, Kentucky, dated June 10, 2025. The investigation was conducted in response to occupant complaints regarding possible indoor mold issues stemming from previous water intrusion events. Six non-viable air samples were collected from various interior locations (second floor southwest and northeast bedrooms, living room, and first floor near stairs) plus one exterior comparison sample, along with three surface tape samples from suspect materials including a hot water heater room ceiling, exposed blue Styrofoam wall insulation, and restroom shower fiberglass insulation. Laboratory analysis by Environmental Hazards Services (EHS) detected elevated levels of Penicillium/Aspergillus group spores in all five interior air samples compared to the exterior baseline, with total spore counts ranging from 1,400 to 18,000 spores/m³. Specific mold spores identified in surface samples included Chaetomium, Trichoderma, Penicillium conidiophores, and Stachybotrys species.

Based on the analytical results and visual observations indicating elevated moisture levels (>15%) in first-floor baseplate areas and shower zones, the report recommends that a Professional Mold Remediation Company (PMRC) properly clean all surfaces and contents on both floors using HEPA vacuums and EPA-approved antimicrobial solutions while employing negative pressure enclosure protocols and respiratory protection for workers. Additional recommendations include disposing of loose drywall on the first floor before cleaning, utilizing dehumidifiers to dry all baseplates in the shower area and along the west wall, and properly cleaning the HVAC system. The report emphasizes that mold spore conditions are contingent upon building conditions and climate, with results valid only for the date collected. All laboratory work was conducted using direct light microscopy methods under AIHA LAP accreditation, with sample analysis and chain-of-custody documentation provided in appendices.
