Turtle Mountain Elementary School – Bus Garage Replace Bus Garage Doors Statement of Work
BACKGROUND: The Turtle Mountain Bus Garage, located in Belcourt, North Dakota, is requesting remove and replace commercial garage doors. The contractor shall furnish all labor, materials and equipment required to remove and replace doors.
SCOPE:
The Contractor shall:
1. Remove damaged garage doors
2. Replace any hardware that may need replacing
3. Replace with Commercial Doors as follows:
a. 4 doors - 10’2X12’0 Energy Saver Flush Low headroom front
White – Insulated with R Value of 12 Windows in 3rd Section
2” Track
Single Trolley
b. 12 doors -11’2X11’9” Energy Saver Flush White – Insulated with R Value of 12
Windows in 3rd Section 2” Track
Single trolley
4. Remove and dispose of all debris and trash from project site to maintain a safe clean work area for staff safety.
5. Contractor is responsible for providing dumpsters to dispose debris.
6. Contractor will be responsible for dust protection and clean up.
7. Contract is responsible to replace the exterior perimeter weatherstripping.
8. Contract is responsible to set up the four overhead doors on the Northwest side of the building converted to have the door track run vertically up the wall to get them up higher and also change the operators to a center mount operator.
9. Provide a Schedule of Values and complete materials listing for ASD approval no less than 14 days prior to commencement of project.
10. Provide a Project Safety Plan and Project Safety POC for compliance with ETR/OJCA/OSHA safety regulations. Furnish and wear all applicable safety and health gear in the performance of this project.
11. Provide a written product warrantee for a One-year period and a workmanship warrantee for a One-year period.
12. Contractor will be responsible for TERO and EPA fees.
13. The Government will furnish all utilities necessary to complete this task.
WORK OUTSIDE REGULAR WORK HOURS
If the contractor desires to perform work under this contract outside regular work hours or on
Saturdays, Sundays or holidays, he shall apply, in writing, to the Contracting Officer no less than 10 calendar days prior to the planned start of such work. Regular work hours are, for the purpose of this clause, those hours the activity or Command where the work is to be performed normally works on a day-by-day basis. Application by the Contractor does not imply consent by the Government. Each application will be considered, and a decision made, on the potential benefit of permitting such work. Approval of a Contractor's application under this clause will be, unless otherwise agreed in writing, at no additional cost to the Government
CONTRACTOR SECURITY REQUIREMENTS
At the Pre-construction Conference [or prior to beginning work on the contract], the Contractor shall provide a list of all on-site personnel, including sub-contractors (including second and third-tier subcontractors) and suppliers, to the Contracting Officer. The contractor shall update this list when changes occur. Contractor personnel will be restricted to designated working areas. Any personnel violating this policy may lose access to the facility. Contractor personnel shall have photo identification at all times while working on BIA facilities. Contractors shall provide the
COR with 24 hours of advance notice of every delivery to the site (e.g., concrete, lumber, etc...)
and provide the company name, delivery person, and phone number of the firm(s) making deliveries. Otherwise access to the BIA facility may be denied. Employees may be required to submit to a background check and be United States citizens.
NOTE:
It is highly recommended that the contractor makes a site visit before submitting a bid proposal for this contract.
