About this file

This document is a Substance Abuse Testing Certification form from the Federal Transit Administration (FTA), aligned with 49 CFR Part 655 regulations for transit operations. The certification requires contractors and subcontractors performing safety-sensitive functions to establish and implement a substance abuse program that complies with FTA's regulations for preventing alcohol misuse and prohibited drug use. Contractors must agree to produce documentation of compliance, permit inspections by authorized representatives from the County, U.S. Department of Transportation, and the Florida Department of Transportation, and submit annual certifications and Management Information System (MIS) reports by specific deadlines.

The certification covers safety-sensitive functions including operating revenue and non-revenue service vehicles, controlling vehicle dispatch, maintaining revenue service vehicles, and carrying firearms for security purposes. Contractors are required to certify compliance annually before March 15 and submit MIS reports by February 15, and must also ensure that all subcontractors at all tiers certify compliance with the same regulatory requirements. The document is sourced from the FY2021 Annual List of Certifications and Assurances for FTA Grants and Cooperative Agreements, indicating it is part of the annual compliance process for transit-related federal funding.
