About this file

This document is a Subcontractor Safety Injury and Illness Prevention Program Acknowledgement Form from the SLAC National Accelerator Laboratory. It outlines the requirements for subcontractors working at SLAC to comply with the laboratory's Injury and Illness Prevention Program (IIPP). Subcontractors must either adhere to the SLAC IIPP or submit their own IIPP for approval. Subcontractors with employees working on-site for over 30 days in a 12-month period must also provide comprehensive occupational medicine services. The form requires the subcontractor to provide contact information and certify compliance.

The related federal contract opportunity is for the procurement of a 3-chamber argon-purged glovebox system with specific technical requirements, including purification units, antechambers, vacuum pumps, sensors, and control systems. SLAC is seeking the Lowest Priced Technically Acceptable offer, with a response deadline of May 10, 2024. The resulting purchase order will be issued on a fixed-price basis, subject to SLAC's standard terms and conditions. Subcontractors must be registered in the System for Award Management (SAM.gov) and provide pricing substantiation.

As such, you must be aware of, and comply with, the requirements of the SLAC IIPP requirements. Alternatively, you have the option to develop and submit your own IIPP for SLAC review and approval. Furthermore, SLAC requires prime subcontractors to flow these requirements down to the sub-tier subcontractors that they hire. You can find further details about these requirements at the SLAC “Injury and Illness Prevention Program (IIPP)” website. This information is provided only as a guide – it is your responsibility to ensure you have read and understand the actual regulatory requirements.
