About this file
This is a HUD Form 4736A, an Employer Certification Form issued by the U.S. Department of Housing and Urban Development in compliance with Section 3 of the Housing and Urban Development Act of 1968. The form is used to certify that workers meet the definition of Section 3 eligible workers for projects receiving HUD funding. The form requires documentation of employer and employee information, including the worker's name, address, phone number, and email, as well as the business name and contact details. A mandatory pre-bid site visit is required for the associated Boiler Steam Supply Line Riser Repairs project at John A.F. Hall Manor Homes, operated by the Harrisburg Housing Authority in Pennsylvania. The work encompasses inspection, repair, replacement, and restoration of steam supply line risers, associated valves, fittings, and related piping systems located beneath residential buildings and within occupied dwelling units. The Contractor must furnish all labor, materials, equipment, supervision, permits, and incidentals necessary to complete the work.
The project is funded through HUD's Capital Fund Program and is subject to Davis-Bacon and Related Acts (DBRA) requirements, which mandate prevailing wage compliance. To qualify as a Section 3 worker, an individual must either have annual income not exceeding $60,850 (based on annualized full-time wages at the time of hire or current employment), be employed by a Section 3 Business Concern, or reside within the service area or neighborhood of the project. The form requires certification under penalty of perjury, with warnings that false claims or statements are subject to criminal and civil penalties including up to five years of confinement and substantial fines. The current income limit threshold for Section 3 worker eligibility is $60,850, and the form is valid through March 31, 2028.
