About this file

This is a Pre-Job Hazard Analysis (JHA) form template used by NIST's Grounds Shop Maintenance for identifying and controlling safety risks prior to performing maintenance tasks. The form is designed to be completed before each task begins or when work conditions change, and requires documentation of identified hazards and corresponding control measures. The JHA includes a comprehensive checklist of potential hazards across multiple categories: environmental hazards (spill potential, weather conditions, chemical handling, ventilation, heat stress, lighting, housekeeping, and asbestos concerns); ergonomic hazards (working in tight areas, repetitive motions, heavy lifting); work-at-height hazards (barricades, floor holes, ladders, scaffolding, and fall arrest equipment); activity-specific hazards (operational lasers, welding, compressed gases, energized equipment, airborne particles, and slips/trips/falls); and personal limitations (distractions, working alone, noise levels, and physical limitations). The form also addresses permits required (confined space, excavation, hot work, roof access, scaffold work, radiation work) and specifies required Personal Protective Equipment (PPE) including safety glasses, hearing protection, respirators, hard hats, safety vests, chaps, safety shoes, and additional specialized equipment as needed. A task-hazard-control table must be completed documenting specific tasks, associated hazards, and implemented controls, with signatures required from both the employee/crew and supervisor/work group leader, as well as the OFPM Safety Office. This template serves as a preventive safety management tool applicable to the exterior lighting inspection and repair work described in the related NIST Boulder solicitation.
