Attachment III – Evaluation Criteria
Evaluation Criteria for Pure Water Service Contract
Evaluation Criteria: Acceptable/Unacceptable
Factor 1 – Past Performance (Acceptable/Unacceptable)
The Offeror shall provide a description of projects of at least one (maximum of 5) completed within the last five (5) years that involved tasks similar to those anticipated for this project.
The offeror shall include the following for each sample project:
a) Project name and location
b) Project scope and relevance to this SOW
c) Contract value (at award and completion)
d) Contract duration
e) Type and extent of subcontracting
f) Name of firm’s project manager and field superintendent for project
g) Names, addresses, and current telephone numbers for project owner and/or owner’s construction manager.
Factor 2 – Project Personnel, Emergencies (Acceptable/Unacceptable)
Provide information for the proposed key personnel (both primary and alternate) who will respond to emergencies involving any of the RO/DI system components. Please provide a resume, relevant experiences, certifications, and home address so that the required 2-hour response time can be confirmed. (The distance between the individual’s address and NIST’s address must be less than 2 hours per Google Maps search.) Key personnel shall not be modified, unless an equal or better is approved by the COR in writing, in accordance with CAR 1352.237-75 Key personnel.
Factor 3 – Project Personnel, Contract Management (Acceptable/Unacceptable)
Provide information for the proposed key personnel who will manage the contract (please indicate if individual proposed is the same as the individual proposed under Factor 2). Provide a resume, relevant experiences, and certifications that demonstrate how the contractor will meet or exceed the requirements of the SOW and control costs for over and above normal maintenance and repair. Key personnel clause CAR 1352.237-75 Key Personnel applies.
BASIS FOR TASK ORDER AWARD: This is a competitive best value source selection utilizing Lowest Price Technically Acceptable (LPTA) source selection procedures. Note:
1. By submission of an offer, the Offeror accepts all solicitation requirements, including terms and conditions, representations, certification, and technical requirements related to this contract.
2. Basis for LPTA is determined by evaluating firm-fixed price of routine maintenance requirements and proposed fully-burdened labor rates for over and above normal maintenance and repair. Contractor shall provide hourly rates for unscheduled maintenance and emergency services.
3. Offerors must be technically acceptable in ALL factors to be considered for award.
