This amendment No. 5 provides answers to questions and includes supplemental annotated drawing. Note this amendment # 5 does not extend the closing date and time but Amendment # 4 did - please refer to Amendment 4 .
Except as provided herein, all terms and conditions of the document referenced in Item 9A or 10A, as heretofore changed, remains unchanged and in full force and effect.

Response to questions
Hopi Site Improvements (Sidewalk Replacement)
General questions/comments –
Q 1 Do we have to provide new civil drawings?
a. No. Legible markups of construction drawings will be accepted. Swing ties or sub-foot latitude and longitude location of re-located utilities installed storm drain manholes, invert elevation of installed storm drains will be required markups.
Q 2 Will there need to be a Tribal Inspector present while we dig?
a. No. Facility personnel or IHS project manager will be on site.
Q 3 What is the timeline to start all 3 stages?
a. Upon receiving Notice to Proceed, contractor, IHS project manager, and facility manager will work to establish a focused time line. Contractor should pl an to schedule simultaneous work (as an example, employee entrance sidewalk demolition and construction coincident with installation of storm drain system).
Q 4 How much time will be allowed for testing and results?
a. Contractor should plan to deliver soil samples to laboratory one week prior to density testing requirement in order to have results available at time of field testing
Q 5 Where it states “provide pedestrian and vehicular traffic control”, will cones blocking off area suffice?
a. Where vehicle traffic must be controlled, refer to the MUTCD. Pedestrian traffic may be controlled using cones.
b. Contractor should also provide highly visible signage to be located at clinic exits alerting patients and employees that exits are temporarily inaccessible.
Generator questions/comments – No new drawings will be issued and all measurements for bid quantities are the responsibility of the bidder. I have provided some estimation below.
Q 6 Could you supply me with elevation for the platform and stairs? I need col umn base elevation, top of steel for bar grate and stair rise and run.
a. Actual measurements are the responsibility of the contractor. These measurements are estimations only and do not relieve the contractor from the responsibility of measuring for bid or take off quantities.
b. The generator is mounted on a concrete slab.
i. Column Base elevation – Using top of slab as a reference point (0), natural ground is approximately -12 inches to -14 inches
ii. Top of steel bar grate – Per sheet 9 of 9, top of grate to match top of generator base below oil drains, field verify height. Top of slab to bottom of generator base, below oil drains is approximately +14 inches to +16 inches
iii. Stair rise and run – Per sheet 9 of 9, max rise is 7 inches. Run will depend on elevation of ground at first step.

Sidewalk questions/comments – No AutoCad or surveyed drawings are available for sidewalk estimation. Quantities used for bid and construction take off should be gathered by interested bidders via a site visit. The attached exhibit provides approximate lengths for sidewalk sections.
Q 7 In reference to Attachment 5a and 5b in Amendment 2, can we please receive more information in regards to dimensions of the sidewalks?
a. Responsibility for accurate bid and take off measurements lies with the contractor.
Sidewalk measurements are rough approximations. Accurate bid and material quantities can only come from field verification and measurement.
b. See attached exhibit for rough estimates of partial sidewalk dimensions. These exhibits represent the drawings for the sidewalk construction portion of this project.
Q 8 A partial floor plan I required of the Health Care Facility and “Emergency Garage” with room/entry identification in order to properly estimate placement of sidewalks and coordinate phasing. (i.e. Dialysis Entrance, Employee Entrance, Elect. Service panel location).
Will this be provided as an amendment before bid is due?
a. See attached sidewalk exhibit for location of Dialysis Entrance and Employee
Entrance.
Storm drainage, emergency equipment pad, paved driveway questions/comments –
Paved driving path will be constructed per the geotechnical report, designed for truck access (80-kN EAL of 73,000 utilizing AC and ABC
Q 9 Will there be a plan sheet indicating the location for the equipment pad?
a. The equipment pad, also referred to in the statement of work as the trailer pad, is located as indicated by note 17 on sheet 3 of 9.
Q 10 Is the proposed fuel area raised, and is it made of concrete?
a. The fuel area referred to on sheet 3 of 9, note 16 is not raised and it will be paved per the geotechnical report, designed for truck access, utilizing asphalt concrete
(per note 8, same sheet) and aggregate base course.
Q 11 The “Equipment Pad” detail no.”8”, page 9 of 9 indicates a 12” thick slab poured against the CMU building wall and footing; and refers to civil drawings for size and location of equipment pad. There are no indications of this pad on the civil drawings and no note of a size for an “Equipment Pad”. There is also no mention of a 12” thick slab in “Part 1-The
Schedule-Section C”. Please confirm a size and location of the equipment pad if required.
a. The equipment pad is to be constructed as designed per sheet 9 of 9 (i.e. 12” thickness). The pad will be located as indicated by note 17 on sheet 3 of 9. Trailer pad shall be 21’ x 30’ as stated in the schedule of work.

Q 12 Does the statement in the miscellaneous section of the contract stating “Contractor will be responsible for all takeoff quantities for bidding purposes. All quantities on plan set are estimates only and are not to be used for bidding purposes” appl y to the footage and elevations on the plan as well?
a. Ultimately, the contractor is responsible for quantities. The lengths and elevations provided were designed utilizing survey equipment and methods, however per keynotes in plan sets (e.g. 9, 10, 11, and 13 on sheet 4 of 9), contractor is responsible to verify elevations and quantities
