About this file
This document is an Iran Contracting Act Certification form for RFP #3843, issued by the State of California, targeting bidders or proposers seeking to submit a contract proposal for goods or services valued at $1,000,000 or more. The certification requires companies to confirm they are not engaged in investment activities in Iran and are not financial institutions extending $20,000,000 or more in credit to entities providing goods or services in Iran's energy sector. Companies must complete either Option #1 (Certification) or Option #2 (Exemption) to be eligible to bid, with failure to submit this form potentially rendering the bid non-responsive.
The document outlines strict compliance requirements with penalties for false certifications, including potential civil penalties up to $250,000 or twice the contract amount, contract termination, and a three-year prohibition on bidding. Companies must certify under penalty of perjury that they meet the specified conditions, with the California Department of General Services maintaining a list of entities engaged in prohibited Iranian investment activities. The form provides space for companies to provide their name, Federal ID Number, authorized signature, and execution date, with two distinct paths for compliance: direct certification of non-involvement or demonstration of a case-by-case exemption.
