About this file

This document is an Insurance Requirements form for a Ship-to-Shore Crane 5kV Power Cable Replacement project at Port Everglades, issued by Broward County's Crane Section. The document outlines comprehensive insurance coverage requirements for a contractor performing power cable replacement work, specifying detailed liability insurance parameters across multiple categories including General Liability, Auto Liability, Worker's Compensation, and Employer's Liability.

The insurance requirements mandate minimum liability limits, with General Liability set at $1,000,000 per occurrence and in aggregate, and Auto Liability coverage of $500,000. The document stipulates that Broward County must be listed as the Certificate Holder and endorsed as an additional insured, with 30 days' written notice required for cancellation (10 days for non-payment). The contractor's insurance must provide primary coverage, cannot require contribution from the County, and any self-insured retention must be declared and approved. The document includes specific provisions for maritime-related work, noting requirements for U.S. Longshoremen & Harbor Workers' Act and Jones Act coverage for activities near navigable waters.
