About this file
This is a Vendor Acknowledgement Form issued by the City of Lubbock, Texas, in connection with a contract opportunity for body work and accident damage repair services for Citibus vehicles. The form establishes the procedural and compliance requirements that vendors must satisfy to be considered for the contract award. Prospective contractors must acknowledge receipt of and compliance with multiple certification and disclosure requirements, including insurance requirements, suspension and debarment certification, and various Texas Government Code provisions. The form requires vendors to confirm they will furnish valid insurance certificates within ten business days of award notification and certify that neither their organization nor its principals are suspended or debarred by a Federal agency. Vendors must also complete vendor risk assessment questions regarding cybersecurity issues, business continuity plans, and the capture of personally identifiable information, with supporting documentation required if answering affirmatively to any question.
The form requires completion and execution by an individual authorized to bind the company, with failure to complete the vendor acknowledgement or provide requested information potentially resulting in proposal disqualification on grounds of non-responsiveness. The form mandates disclosure of company identification information including Tax ID, SAM.gov Unique Entity ID, and CAGE Code, along with designated contact information for both a contract signatory and a contact for clarifications regarding the procurement. Vendors must also certify compliance with Texas Government Code requirements prohibiting boycotts of Israel and energy companies, as well as discriminatory practices against firearm entities or trade associations for contracts valued at $100,000 or more with companies having ten or more full-time employees. The form additionally requires certification that the company does not appear on the Texas Comptroller's website listing companies that conduct business with Iran, Sudan, or Foreign Terrorist Organizations.
