C.1 SCOPE OF CONTRACT

The Contractor shall provide all labor, materials, tools, testing, supervision, incidentals and permits necessary to properly complete the following:

a) Install temporary potable water tank.

b) Demolition of concrete spring box.

c) Replace spring box with new cast in place concrete spring box.

d) Paint spring box with NSF approved concrete paint on interior per specifications and paint per specifications on exterior.

e) Provide a qualified third-party coating inspector.

f) Sand blast and powder coat existing hatches and reinstall on spring box.

g) Install new shut off valves and all plumbing connections.

Double Springs- Double Springs is located in the Double Springs Campground approximately 28 miles Southeast of Flagstaff. To get to Double Springs head southeast on Lake Mary Road for 21 miles to Junction with Mormon Lake Road (County Road 90). Turn south (right) on Mormon Lake Road, continue for 5.1 miles to junction with Double Springs Campground rd. (Forest Service Road (FSR) 90F). Turn right onto FSR 90F continue for 0.3 miles to Spring Box.

M.1 QUOTE EVALUATION

Quotes will be evaluated using a best value approach. This allows the Government to accept other than the lowest priced quote. The Government may perform a comparative evaluation (comparing offers to each other) to select the contractor that provides the best value, considering the evaluation factors in this solicitation.

M.2 TECHNICAL EVALUATION FACTORS

Factor 1. Past Performance Past performance is a measure of the degree to which the offeror (including major subcontracts) has satisfied previous customers and complied with Federal, State and Local laws and regulations. The Government will contact references to assess the quality of the contractor’s work, timely completion, project management, and compliance with regulatory requirements.

The Government will consider the currency and relevance of the information, source of the information, context of the data and general trends in contractor performance. The Government shall consider this information, as well as information obtained from any other sources, when evaluating the offeror’s past performance.

Factor 2. Experience The Government will assess the specialized experience and skill of the firm with a focus on experience that is relevant to the specified project.

Factor 3. Technical Approach to Performing the Work When evaluating the Technical Approach to Performing the Work, the government will assess the reasonableness of the contractor’s proposed approach to performing the work including the use of biobased products in project implementation.

Double Springs Spring Box Demolition and Replacement Flagstaff Ranger District Coconino National Forest

STATEMENT OF WORK

Project:

This Statement of Work (SOW) outlines the tasks to properly demolish and replace the spring box and all associated plumbing at Double Springs on the Coconino National Forest. The spring box is within the Flagstaff Ranger District of the Coconino National Forest.

Location:

Double Springs- Double Springs is located in the Double Springs Campground approximately 28 miles Southeast of Flagstaff. To get to Double Springs head southeast on Lake Mary Road for 21 miles to Junction with Mormon Lake Road (County Road 90). Turn south (right) on Mormon Lake Road, continue for 5.1 miles to junction with Double Springs Campground rd. (Forest Service Road (FSR) 90F). Turn right onto FSR 90F continue for 0.3 miles to Spring Box.

34°56’34” N, 111°29’32” W

Background Information:

The spring box at Double Spring supplies water to Double Springs Campground, St Joe’s youth camp, Recreation residential houses and two private entities. The spring box has deteriorated.

Water is leaching out between the concrete and bed rock. The spring box will need to be replaced to ensure the safety of the users that rely upon this water source for potable water.

Sanitary Facilities:

There are vault toilets on site that will be made available for use for the duration of this project.

Contractor will be responsible for keeping the toilets clean and stocked with supplies.

Period of Performance:

The decommissioning and replacement of the springs box be completed within 120 days of the issued notice to proceed.

Contract Time:

The contractor is permitted to perform work on weekends.

Utilities:

There is no electricity available at the sites.

There is no water available at these sites. Contractor is responsible for providing their own source of water and electricity.

Inspections:

Inspections will be performed by the government by the COR or designated inspector.

Tasks The Contractor shall provide all labor, materials, tools, testing, supervision, incidentals and permits necessary to properly complete the following:

c) Replace spring box with new cast in place concrete spring box.

d) Paint spring box with NSF approved concrete paint on interior per specifications and paint per specifications on exterior.

e) Provide a qualified third-party coating inspector.

All work is to be performed in accordance with applicable Federal, State, and local regulations including requirements of IPC, OSHA, EPA, ADEQ, ADWR and Coconino County.

The contractor shall adequately protect the work sites, surrounding lands, employees and general public during the entire length of the project. The contractor shall replace any fencing that is required to be removed to complete this project. The contractor shall be responsible for all damages or injury caused due to their actions or negligence.

Tasks are Pay Items outlined in the Bid Schedule.

All tasks shall be completed per specifications

Task 1: Mobilization

All activities associated with mobilization and demobilization to perform the tasks associated with the specifications. The site is in a remote location and access is down a short section of dirt road. Activities include transportation of labor, material, and equipment. Weather normally starts impacting the road conditions in mid-November, which can prevent access to the spring box. Road conditions generally improve after winter in late March – early April to allow access to the spring box.

Task 2: Spring Box Demo

The existing Spring Box shall be demolished. The spring box shall be dewatered. All concrete, rebar and excess plumbing shall be disposed of at an approved land fill. All materials removed and designated for disposal shall be removed from National Forest Lands and disposed of in accordance with local, state and federal regulations regarding disposal of construction waste- No materials shall be left on Forest Service lands. The existing hatches shall be removed for reuse.

Task 3: Temporary Tank

The work consists of providing all labor, materials, equipment, and plumbing connections and all other incidentals necessary for a complete temporary potable water tank until the spring box replacement is completed and accepted. The temporary potable water tank shall be installed to provide water for any users that are currently relying on the spring for water. If the contractor is unable to use the spring water to fill the tank, water will need to be hauled from an approved potable water source. The potable temporary tank will need to be functional prior to any demolition activities.

The temporary tank should be placed as close as possible to the existing spring box so the tank can continue to be a gravity system. Work will also include removing the temporary system after completion of the spring box and placing the spring box back into service.

All work performed shall meet the requirements of the Uniform Plumbing Code, Latest Edition. All plumbing materials shall be NSF approved.

Task 4: Cast in Place Concrete Spring Box

A new concrete spring box shall be installed per specification and plans. All specifications and plans shall be followed. The contractor shall work with the COR to determine penetration placement for the overflow pipes, distribution lines and drain lines required for the operation of the spring box.

Task 5: Sampling and Testing of Concrete

The concrete shall be tested in accordance with the specifications.

Task 6: Plumbing connections

The contractor shall provide plumbing connections and valves for the users that are currently utilizing the spring for water. All valves shall be stop and waste valves to prevent the valves from freezing in the winter.

Task 7: Spring Box painting (interior and exterior) The inside of the spring box shall be painted with an NSF/ANSI 61 approved sealant per specification. The concrete will require a 28-day period to cure prior to paint and epoxy coating for both interior and exterior of spring box. The exterior of the tank shall be painted with paint per specifications.

Task 8: Coating Inspection Services The contractor shall provide an independent NACE Certified Coating Third Party Inspector to verify and record all coating system applications. The following list is the inspector’s responsibilities:

1. Maintain permanent written records of Hold Point inspection.

2. Verify and record that the coatings and other materials are as specified (i.e., manufacturer, product name & product batch dates).

3. Verify and record condition of coatings and material along with their storage procedures.

4. Verify and record environmental conditions while on site.

5. Surface Preparation Inspection – Verify and record that the specified surface preparation meets or exceeds specified cleanliness standard and surface profile. Check for characteristics or defects that would adversely affect the performance or appearance of coating systems.

6. Verify and record coating mixing and thinning procedures, i.e. batch dates, amount mixed, material temperatures, induction time etc, if on site.

7. Verify and record application procedures, if on site.

8. Coating Inspection – Verify and record the DFT readings of each coat. Check the applied coating film for characteristics or defects that would adversely affect performance or appearance of coating systems, i.e. runs, sags, skippers, inconsistent / poor coverage. Communicate any possible problems that may need to be addressed and corrected.

9. Dry film thickness readings of ferrous and non-ferrous substrates shall be done in accordance with SSPC-PA 2 (Level 3).

10. NACE Inspector to observe all holiday testing being conducted by the Contractor.

11. All inspection equipment to be used must be certified and still within certification during the inspection process.

12. Provide written inspection reports and photo logs for each inspection trip.

13. Generate a punch list, conduct follow up inspection and provide recommendations to achieve full compliance and acceptance.

14. Complete a final inspection.

15. Provide Final Report.

Task 9: Hatches- Sand Blast and Paint

The existing hatches shall be removed, sandblasted and painted. The paint shall be NSF/ANSI 61 approved paint. Once the spring box is installed the hatches shall be reinstalled with new gaskets that provide an airtight seal to prevent insects and rodents from entering the spring box.
