AMENDMENT 4

Amendment to Design Package and Haul Route

1) These amended general notes are listed on the revised design general notes spreadsheet highlighted in yellow and a map has been provided for the haul route:

• Specified source of crushed aggregate surfacing material is from a government supplied at Guadalajote Material Source Site (location map included in solicitation package).

• CONTRACTOR IS RESPONSIBLE FOR MAINTENANCE OF ALL HAUL ROADS

USED BEFORE, DURING, AND AFTER OPERATIONS. NO SEPARATE PAYMENT

WILL BE MADE FOR THIS REQUIRED MAINTENANCE.

• ALL CONSTRUCTION VEHICLES SHALL BE CLEAN AND FREE OF WEEDS

AND OTHER HARMFUL MATERIALS.

• Compaction for the road surfacing and rolling dip aggregate will be Compaction Method C Section 204.11: (c) Placement Method 3. Adjust the moisture content of the material to a moisture content suitable for compaction. Fill the interstices around rock with earth or other fine material as practical. Operate hauling and spreading equipment uniformly over the full width of each layer until there is no visible evidence of further consolidation.

Make at least three complete passes.

• The Specified haul route to and from the project site will be as follows: Haul Route with full trucks: From the Guadalajote Flat material source site, head southwest toward South Harshaw Road (FSR 49) for approximately 870 FT turn right onto South Harshaw Road (FSR 49) and proceed for approximately 5.9 miles. Turn right onto North Harshaw Road (FSR 58), stay right after approximately 0.5 miles and proceed for approximately 2.2 miles. Stay left to continue on FSR 58 and proceed for approximately 2.1 miles to project site. Return Route with empty trucks: From project site proceed 1.0 mile on FSR 58 west to FSR 214 take a left on FSR 214 and proceed 4.55 miles to FSR 49 Harshaw Road, take a right and proceed North for 0.75 miles to the Guadalojote material site turn off, proceed right for 870 feet to the material site.

2) The additive option for the culvert installations has been deleted from the solicitation since the FS road crew will be installing these. In place a new additive option has been added for the North Bypass resurfacing.

3) Change the time to complete the project from 30 days to 60 days.

See Attachments 11 (Haul Route Map), 12 (Revised Design Spreadsheet), and 13 (Revised SOI to be completed and submitted with proposal).

New proposal due date is October 18, 2024 @ 4:00 p.m. local Arizona time.
