Notes to Quoters:
1. This solicitation is for preventative maintenance services for autoclave and washer services under a base year and four option year firm fixed price contract. Price must include all costs including but not limited to supplies, equipment, labor, and supervision to accomplish the services at the site.
2. This solicitation is issued as a small business set aside. The NAICS code for this acquisition is 811210, with a size standard of $34M.
3. Evaluations will be conducted in accordance with 52.212-2 Evaluation—Commercial Products and Commercial Services. Quoter’s are responsible for ensuring completeness of quotations. The Government may evaluate solely on the information provided in the quotation. Quotations that include additional terms may be excluded from consideration. Alternate Pricing Schedules will not be considered.
4. Interested vendors must submit:
1) Quoter name, contact information, DUNS/SAM UEI number and verification of SAM.gov registration,
2) Signed copy of the SF1449 Form and firm fixed price line-item pricing (include below in pricing schedule),
3) Proof of technical acceptability by providing:
a. Past performance references. Quoters shall include point of contact information
(name, address, telephone, email, description of item delivered) for 3 references that have received similar services.
b. Narrative statement of capability. Outline experience and qualifications in preventative maintenance services for autoclaves and washers contracts of this type and size (to include identified personnel and qualifications), and a narrative explaining how the contractor plans to ensure performance, monitor quality assurance, and supervise work.
5. All quoters must be registered in SAM. Quoters must be registered in SAM when they submit their quote. SAM.gov | Home Please make sure your registration is current.
Implementation Of Executive Order 14173, 14168, and 14148 Notice: System updates may lag policy updates. The System for Award Management (SAM) may continue to require entities to complete representations based on provisions that are not included in agency solicitations.

General Information
1.0 Scope of Work
Provide on-site preventative maintenance on all autoclaves and washers.
2.0 Background
Provide preventative maintenance on all autoclaves and washers including all necessary lubrication, adjustments, verifications of performance, cleaning of valves and piping to ensure the equipment is functioning in the manner prescribed by the equipment manufacturer.
Contractor Requirements
3.0 Technical Requirements / Tasks
The Contractor is responsible for inspecting the equipment for damage and replace broken or worn-out parts as needed.
They are also responsible for making all the calibrations needed, and making sure the equipment is functioning as it should. And should be available for customer service call and emails about unexpected issues during business hours of the media prep department from 8:00am – 4:30pm Monday – Friday.
4.0 Government Furnished
The autoclaves and washers are owned by the USDA/APHIS/VS at Ames, IA. This work will be performed in the Media Prep and Glassware department laboratory. At 1920 Dayton Avenue in Ames, IA 50010 between the business hours of 8:00am to 4:30pm, Monday – Friday. Work will NOT be performed after the business hours stated above or on any federal holidays.
5.0 Deliverables / Schedule
Key Deliverables: On-site repair and services, including parts and labor.
Description: Biannual on-site preventative maintenance of autoclaves and washers by a technician including parts and services.
Objective Due: When requested biannually.

Travel to Ames, IA, accommodations, and any other travel arrangements shall be the responsibility of the awardee.
7.0 Contractor’s Key Personnel
Technician must be trained in repairing and services autoclaves and washers. They must also be able to access and acquire any proprietary parts or software needed for repair of the equipment.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor’s directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.
(e)
(1) Notwithstanding the requirements of the clauses in paragraphs (a), (b), (c), and (d) of this clause, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (e)(1), in a subcontract for commercial products or commercial services. 52.217-8 Option to Extend Services. (Nov 1999)
The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days of contract expiration (base or option year).

52.217-9 Option to Extend the Term of the Contract.
As prescribed in 17.208(g), insert a clause substantially the same as the following:
Option to Extend the Term of the Contract (Mar 2000)
(a)The Government may extend the term of this contract by written notice to the Contractor at any time before the contract expires. Exercise of any option shall be evidenced on a Standard Form (SF) 30.
(b)If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c)The total duration of this contract, including the exercise of any options under this clause, shall not exceed five years and six months.
52.237-2 Protection of Government Buildings, Equipment, and Vegetation. (Apr 1984)
The Contractor shall use reasonable care to avoid damaging existing buildings, equipment, and vegetation on the Government installation. If the Contractor’s failure to use reasonable care causes damage to any of this property, the Contractor shall replace or repair the damage at no expense to the Government as the Contracting Officer directs. If the Contractor fails or refuses to make such repair or replacement, the Contractor shall be liable for the cost, which may be deducted from the contract price.
