About this file

This document is a Request for Information (RFI) related to a federal contract opportunity for the Annual Inspection of USDA NCAH Fall Protection Systems and Equipment. The RFI includes responses to four questions from a potential offeror regarding the inspection and testing requirements for the fall protection systems and equipment. The responses indicate that the last load testing was conducted in July 2020, and that no additional load testing is required under the contract. The RFI also references the related federal contract opportunity, which is a firm-fixed-price contract for the annual inspection of fall prevention systems and equipment at a facility in Ames, Iowa. The contract is set aside for 100% small business and will be awarded using a Lowest Price Technically Acceptable (LPTA) evaluation process with a technical factor for the inspection services meeting the specified requirements and a past performance factor.

Request Response

Could you kindly provide the most recent inspection or certification report related to this project? This information should not be necessary for the purpose of submitting a proposal related to this requisition.

According to ANSI standards, anchor points and Horizontal Lifeline (HLL) systems designed for fall arrest should undergo load testing every five years. Could you inform us of the last time these systems were load tested? If this information is unavailable, please confirm if a load testing needs to be performed during the base year of the contract.

The last load/pull and 5-year certification testing was conducted in June/July 2020. This contract does not include load test in the base year or subsequent option years.

Could you provide the total length of all the HLL systems involved? I have attached the last load testing report dated July 1, 2020, which includes Infinity Access roof top drawings of all Roof top anchors and HLL systems drawings. The lengths of each area and quantities can be determined from said report/drawing.

To ensure our reports are reviewed and stamped by our engineering department, we require the rooftop structural plans showing how the anchors and HLL systems are attached to the building.

As stated in question 3. This information is included in the July 2020 report.
