General Information
On the dates referenced above, Diversified Fall Protection (DFP) completed the onsite load testing of the existing anchorages for use as part of a suspended access system located at the address referenced above. Inspection and testing were completed as directed by Tom Ferguson, qualified fall protection engineer, as confirmation of capacity and suitability for suspended access use. All testing was conducted in accordance with OSHA and ANSI recommendations.
This report describes the testing procedures, setup, equipment, data collection, analysis of the collected test data, as well as recommendations for repairs or modifications as necessary. Refer to Infinity Access LLC drawings dated 6/22/06 attached as Appendix C, for plan view locations and details for anchorages included in the testing. Only anchorages as identified in this report and attached drawings are included in the certification.
Description of Equipment Inspected
Rigid Post Roof Anchor (46)
Testing Prescriptions
Applied Load Pull Test Procedure: The load generating device (chain hoist or hydraulic cylinder) was attached to each anchorage point along with an in-line, calibrated dynamometer to measure the forces generated. During testing, inspectors gradually increased the load to reach the designated proof load, which was maintained for three (3) minutes, inelastic deformation (if applicable) was identified and recorded, as well as any other observations or issues with anchorage connection. Refer to Appendix A.
Test Limitations: The test results and data analysis expressed herein is not a warranty that the components tested are necessarily defect-free. DFP takes no responsibility for and makes no claims or opinions with respect to any latent or concealed defects or other defects which may not be visible, but which may exist prior to or as of the date of the test.
Equipment Inspection: Each anchorage component was additionally visually inspected for exposed, visible, and accessible components for any obvious signs of wear, loose or damaged mountings, scarring, cracking, rust, damage, bending, distortion, or corrosion. Anchorage equipment labels, tags, signage or user instructions made available were also inspected.

Summary and Recommendations
Only the specified anchorages as identified in this report and associated drawings are certified for suspended access use, and are rated for a working load of 1,250 lbs (5,000 lbs fracture/detachment load, 4-1 Safety Factor) and may be used to support (1) equipment tieback OR (1) safety tieback.
We suggest that you continue to periodically inspect the roof anchors and inform DFP if any unsafe conditions arise between the required annual inspections. Authorized users must fully understand the operating procedures of the fall protection system and are responsible for performing daily inspections prior to each system use to confirm proper function prior to use. Anchor recertifications should take place at periods not to exceed 10 years from the date of inspection. All documentation of annual inspections, recertifications, and the anchorage system log-book should be kept in a safe storage area and should be made available upon request by persons using the system. DFP is committed to addressing any further questions or providing you with any future safety solutions. Please contact us with any questions or concerns.
