About this file

This is a Questions and Answers document issued as Amendment 001 to Solicitation Number 12445226Q0036 for mold remediation and reconstruction services at the USDA Forest Service, Lake City Airtanker Base Administrative Operations building in Lake City, Florida. The document clarifies contractor requirements and project scope across 15 vendor inquiries. Key clarifications include: the contract combines mold remediation and construction into a single award; all tile flooring will be removed to prevent change orders for additional mold discovered during demolition; HEPA vacuuming and wiping of all walls, floors, furniture, and content throughout the building is required; carpet removal is limited to specific areas detailed in the Performance Work Statement Section 2.3; all building contents requiring removal must be mold/mildew free before storage in another on-site building, with the Contracting Officer Representative (COR) approving disposal items; ductwork should be estimated as metal, flexible, and potentially ductboard; ductwork cleaning is considered cross-contamination cleaning required throughout the building; cabinetry and millwork specifications are detailed in PWS Section 2.5 with final COR approval required; ceiling water damage around vents and potential mold in undiscovered areas are not currently included but may be addressed via modification once building access is verified; attic access is available through the mechanical room; water damage around the water heater may be considered post-access; the parking lot is available as a lay down area; and no local or city permits are required. The original leak causing mold contamination has been repaired, and all requirements are further detailed in the referenced USDA Performance Work Statement and attachments.

Lake City ATB Contractor Questions & Answers

1. Is this going to be one contract for mold remediation and construction?

· Yes, this will only be one contract combining mold remediation and construction.

2. Is the tile flooring all going to be pulled up so we don’t have to go back for a MOD if more mold is found during the demolition?

· Yes, please refer to the USDA Performance Work Statement; Section 2.3 Mold Remediation, Selective Demolition, and Cleaning; bullet A5 for areas of removal and approximate quantities.

3. To what extent is the wipe down of the building to get rid of the mold? (it was noted that there were lots of small items left on desks, picture frames, etc. as seen in the pictures online)

· Per attachment 6, the contractor shall HEPA vac and wipe down all walls, floors, content/furniture, etc. throughout the building.

4. Is the carpet to be removed or cleaned?

· The carpet is to be removed from the areas listed in the USDA Performance Work Statement; Section 2.3 Mold Remediation, Selective Demolition, and Cleaning; bullet A5 which lists areas of removal and approximate quantities.

5. Where do we want items stored and will there be a list of what to keep and what to dispose of?

· All items will be stored in one of the other buildings on site as identified by the COR. The COR will approve of all items that are to be disposed of. Please see the USDA Performance Work Statement; Section 2.3 Mold Remediation, Selective Demolition, and Cleaning; bullet A2.

· Any items that are currently in the building that will require removal during the project must be mold/mildew free prior to storing in another building.

6. What is the duct work made of inside the building?

· For estimating purposes, estimate duct cleaning to be metal, flexible, and potentially ductboard.

7. Was the original leak fixed?

· Yes, the original cause of the leak has been repaired, however please review the USDA Performance Work Statement; Section 2.3 Mold Remediation, Selective Demolition, and Cleaning; bullet A3 and A4 for more details.

8. Is the duct work to be considered cross contamination cleaning it will be needed throughout the building?

· Yes, please reference attachment 5 and 6.

9. Are there any specific measurements on the cabinets, door and drawer layouts? What is the surface material?

· Please see USDA Performance Work Statement; Section 2.5 Reconstruction; bullet D. Cabinetry and Millwork, also note that the COR will give final approval of the layout.

10. There are signs of ceiling water damage around the vents. Will that be part of the repair work?

· Due to current building access limitations this cannot be verified at this time and it is not included in the contract. Once the building is safe to enter, we will determine if a modification is necessary.

11. There are concerns with the temperatures and humidity levels inside the building. Since the building has been sitting for a while with partial demolition there is a possibility of mold in other areas not listed for demolition.

· Due to current building access limitations we cannot determine if other areas are affected. If other areas are discovered to be impacted, we will determine if a modification is required.

12. What kind of access to the attic does the building have.

· There is attic access in the mechanical room.

13. There was a discussion of some water damage around the water heater caused when the water heater was installed.

· This is not currently in the contract but may be considered after we are able to access the building.

14. Where will the contractor be permitted to have a lay down area for construction?

· The parking lot in front of the building will be made available. It currently has chemical retardant stored there but that will be removed before construction.

15. Are local or city permits required for this project?

· No permitting is required.
