DIVISION 1 – GENERAL REQUIREMENTS
SECTION 011000 SUMMARY OF WORK
PART 1 - GENERAL
1.1 SUMMARY
A. This project is located at the Bend Guard Station, approximately 12 miles south of Highway 2 and Lower Thompson Lake, along Bend Road. Construction shall be performed between May 1st and September 30th.
B. Project Summary Description: The project consists of, but is not limited to, replacing the existing galvanized iron pipe water distribution system with new HDPE water distribution line from the well to the Cook House; installing a frost free sanitary hydrant near the horse corral; replacing the existing yard hydrant with a “post-flushing” hydrant connected to the new main; replacing the existing well controls and pressure tanks, and all the plumbing work associated. The Contractor shall provide prices for all items shown on the Schedule. The following descriptions are provided as an outline of major work items and shall not be constructed as an inclusive or complete listing of all items of Work under this Contract.
This project includes but is not limited to the following major items of work as shown on the plans and specifications.
1. Mobilization: Moving equipment and other necessary work items to and from the project site.
2. Abandon the existing water lines running from the existing well to the Cook House, and from the cook house to the existing yard hydrant. Also includes removing and replacing the two existing yard hydrants as noted on the Plans.
3. Install new water 2” HDPE distribution system from existing well to the Cook House.
4. Install new 1” HDPE water service and frost free sanitary hydrant near horse corral.
5. Install 2” HDPE water service and corresponding “post-flushing” hydrant approximately halfway between the well and Cook House, as shown on the Plans.
6. Connect the new 2” HDPE main to the existing well.
7. Remove and replace existing pressure tanks and well controls in the basement of the Cook House, and connect the new 2” HDPE piping to the existing building plumbing.
8. Install 2-inch electrical conduit and control wires.
9. Includes clearing & grubbing, erosion control, and grading and seeding of all disturbed areas.
C. Government or Owner refers to the USDA Forest Service. Contracting Officer (CO) refers to the person assigned by the USDA forest Service to administer the contract. Soon after Contract award, the Contracting Officer will designate a Contracting Officers Representative (COR).
1.1 GENERAL SITE CONDITIONS AND WEATHER
A. The elevation of the site is approximately 3,300 feet above mean sea level. The construction site experiences moderate snowfall and moderate to cold temperatures, with snow on the ground typically from December through April.
1.2 USE OF PREMISES
A. General: Contractor shall have limited use of premises for construction operations, including use of Project Site, during construction period. Contractor's use of premises is limited by the Government's right to perform work at the site.
B. Contractor shall at all times conduct their operations to ensure the least inconvenience to the public and employees.
C. Confine storage of materials to areas as approved by the CO.
D. Contractor shall provide adequate signing and barricades and take necessary safety measures to protect the public during all construction operations. Contractor shall minimize disturbance of all undisturbed areas.
E. Preservation of Natural Features: Confine all operations to work limits of the project. Prevent damage to natural surroundings. Restore damaged areas, repairing or replacing damaged trees and plants, at no additional expense to the Government.
F. Provide temporary barriers to protect existing trees and plants and root zones.
1. Do not remove, injure, or destroy trees or other plants without prior approval.
Consult with CO and remove agreed-on roots and branches that interfere with construction.
2. Do not fasten ropes, cables, or guys to existing trees.
3. Carefully supervise excavating, grading, filling, and other construction operations near trees to prevent damage.
G. Existing Utilities: Notify CO of proposed locations and times for excavation.
1. Contractor shall be responsible for locating and preventing damage to known utilities. If damage occurs, repair utility at no additional expense to the Government.
2. Interruption of Existing Utility Service: Notify CO, 48 hours in advance of any interruption of existing utilities.
1.3 MEALS AND LODGING
A. Meals and lodging are commercially available along Highway 2 approximately 20 miles north of the construction site. Meals will not be available at Forest Service facilities. Camping at nearby campgrounds may be allowed if approved.
1.4 CAMPING
A. Permission may be granted to camp at a nearby location or within this campground.
1.5 FIELD VERIFICATION
A. Field verify all new and existing dimensions affecting the work of this contract before ordering products.
1.6 REQUIREMENTS OF THE OFFERER’S PROPOSAL
A. The Government Drawings and Specifications are the basis for this Contract.
B. The work shall conform to the Drawings as closely as possible. Those products listed in the Specifications are preferred products, chosen for compatibility, longevity, durability, and quality. However, equal or better alternatives submitted by the Offeror will be considered.
1.7 PERMITTING
A. The Contractor shall be responsible for obtaining all permits and associated fees required to complete the project.

SECTION 010250 CONSTRUCTION STAKING
1.1 SUMMARY
A. This work shall consist of the construction staking of the water line in accordance with the Drawings and Specifications. The work includes the furnishing of all labor, equipment, instruments, materials, transportation, and other incidentals necessary to complete the construction staking in accordance with these Specifications and acceptable engineering practice.
B. Construction staking shall be accomplished under the direction of a Civil Engineer, Engineering Technician, or Land Surveyor closely associated and familiar with construction staking; periodic visits to the project site are required.
1.2 MEASUREMENT AND PAYMENT
A. No separate payment will be made for the work included under this section; rather payment shall be considered to be included in the items of work listed in the Schedule of Items.
PART 2 - PRODUCTS
1.1 GOVERNMENT-FURNISHED STAKING
A. The government will provide the contractor with a survey control sheet which shows horizontal and vertical control for site to be staked.
1.2 STAKES
A. Identification stakes and hubs shall be of sufficient length and width to provide and solid set in the ground and to provide space for marking above ground when applicable.
Other dimensions and materials may be used, such as steel reinforcing bars, wire flagging and markers, and metal pins, if approved in writing by the Contracting Officer.
1.3 SURVEY NOTE PAPER AND BOOKS
A. Paper for survey notes shall be moisture-resistant paper. Notes shall be contained in books with covers that will protect the contents and retain the pages in numerical sequence during field use. Field notebooks or note paper shall be furnished by the Contractor.

PART 2 - EXECUTION
2.1 GENERAL
A. The Contractor shall be required to provide all necessary staking and control for all phases of construction, with the exception of the staking provided in section 2.1 above. The contractor shall use the data and information shown on the project drawings to establish the location of all facilities to be constructed under this contract. The Contractor may choose any method of establishing horizontal and vertical controls provided the following minimum precision and accuracy requirements of .01 feet are met.
2.2 ESTABLISHING AND REFERENCING IMPROVENT LOCATIONS
A. Waterlines
1. Water lines shall be staked at the locations shown on the drawings with the specified minimum burial depth. Minor adjustments to the alignment may be made only with approval from the CO.
2.3 DISCREPANCIES
A. The contractor shall compare the staked horizontal and vertical alignment with the design data. Differences between previously recorded and observed elevations of benchmarks shall be referred to the Contracting Officer. Staking of these areas shall be deferred until the Contacting Officer resolves these differences.
2.4 SURVEY NOTES
A. All survey data shall be neatly recorded in survey books. All survey notes shall become the property of the Forest Service. Errors shall be deleted by lining out. Date, crew names and positions, instrumentation, and weather shall be recorded in the notes at the beginning of each day’s work.

SECTION 011400 WORK RESTRICTIONS
1.1 USE OF PREMISES
A. Use of Site: Limit use of premises to work in areas indicated. Do not disturb portions of site beyond areas in which the Work is indicated. Assume full responsibility for the protection and safekeeping of materials and products stored on the site.
1. Limits: Confine constructions operations to area designated by the Contracting Officer.
2. Government Occupancy: Allow for Government occupancy of site for use during periods of wildfire protection activities and other activities as requested by the Contracting Officer.
3. Entrances: Keep entrances serving the site clear and available to Government employees, and emergency vehicles at all times. Do not use these areas for parking or storage of materials.
Schedule deliveries to minimize use of entrances.
Schedule deliveries to minimize space and time requirements for storage of materials and equipment on-site.
1.2 OCCUPANCY REQUIREMENTS
A. Full Government Occupancy: Cooperate with Government during construction operations to minimize conflicts and facilitate Government usage. Perform the Work so as not to interfere with Government operations.
1.3 SCHEDULING
A. The Contractor shall coordinate scheduling and timing of required administrative procedures to avoid conflicts and insure orderly progress of the work. Proposed schedule shall be provided to the Contracting Officer 14 days prior to initiation of on-site work.
B. Schedule and conduct operations to minimize down time of water supply to facilities.
Submit schedule to the Contracting Officer, outlining proposed construction sequence.
C. Schedule and conduct operations to minimize erosion of soils.
D. The Contractor’s schedule shall consider seasonal weather patterns for the project area.
Any costs incurred by the Contractor due to weather delays shall be borne by the Contractor at no additional cost to the Government.
1.4 PROTECTION OF PUBLIC AND PROPERTY
A. Provide and maintain temporary barricades and warning signs to control access to protect the public and construction personnel from construction hazards.

B. Comply with Federal, State, and local laws, rules, and regulations concerning construction safety and health standards. Do not expose the public to or require any workers to work under conditions that are unsanitary, hazardous, or dangerous to health and safety.
C. Repair damage to existing public and private improvements to original condition at no additional cost to the Government. Control dust and noise to protect adjacent property from damage.
1.5 SAFETY
A. The Contractor shall familiarize him/herself with current OSHA rules and observe these regulations at all times during the term of this Contract.
1.6 RECYCLED PRODUCTS
A. The Contractor is encouraged to use construction products and materials manufactured from recovered materials or recycled where it is economical and practical to do so.
1.7 WASTE DISPOSAL SITE
A. No solid waste disposal service will be available. Contractor will need to coordinate waste disposal services.
1. All excavated material not used in the construction of embankments or backfilling of trenches, or other excess material resulting from the excavation and embankment operation shall be hauled to the designated disposal area.
2. All demolition materials, garbage, and other refuse generated shall be removed from the project site and legally disposed of off Government property in an approved landfill or legal disposal area. It shall be the Contractor's responsibility to locate a legal disposal area, to secure any necessary permits and to pay all costs involved in the disposal of materials.
3. The disposal site shall be left suitable for seeding.
B. Remove all waste materials from the site on a regular basis. Do not dispose of any materials onsite by burial or burning.
C. Dispose of all hazardous waste, including treated wood, in accordance with applicable state and federal regulations.
1.8 INTENT TO BEGIN WORK
A. After Notice to Proceed is issued and the Post-Award Conference, the Contractor shall notify the Contracting Officer of his/her intent to begin initial construction at least five days in advance.
1.9 PROGRESS MEETINGS
A. Meetings shall be scheduled as directed by the Contracting Officer to review progress and facilitate efficient, orderly prosecution of the Work.

B. Meetings shall be held at the Contractor's project site field office. The Contracting Officer’s Representative and Contractor's Project Superintendent shall be in attendance.
Others shall attend as necessary for issues to be discussed.

SECTION 011700 ACCIDENT PREVENTION
1.1 DESCRIPTION
A. The work of this section consists of establishing an effective accident prevention program and providing a safe environment for all personnel and visitors.
1.2 SUBMITTALS
A. Accident Prevention Program: Before on-site work begins, submit for approval an accident prevention program. The Contracting Officer will review the proposed program for compliance with OSHA and project requirements. If the program requires any revisions or corrections, the Contractor shall resubmit the program within 10 days.
No progress payments will be processed until the program is approved. The program shall include:
1. Name of responsible supervisor to carry out the program.
2. Weekly and monthly safety meetings.
3. First aid procedures.
4. Outline of each phase of the work, the hazards associated with each major phase, and the methods proposed to ensure property protection and safety of the public, government personnel, and the Contractor's employees. Identify the work included under each phase by reference to specification section or division numbers.
5. Training, both initial and continuing.
6. Planning for possible emergency situations, such as floods, fires, cave-ins, slides, explosions, power outages, and windstorms. Such planning shall take into consideration the nature of construction, site conditions, and degree of exposure of persons and property.
B. Certificates: Provide certificates from a mechanic that all mechanical equipment has been inspected and meets OSHA requirements.
C. Submit a copy of test reports, as required by OSHA, for personnel working with hazardous materials.
D. Submit a brief report of safety meetings and of inspections.
E. Upon request, submit proof of employees' qualifications to perform assigned duties in a safe manner.
1.3 QUALITY ASSURANCE
A. In case of conflicts between Federal, state, and local safety and health requirements, the most stringent shall apply. Equipment or tools not meeting OSHA requirements will not be allowed on the project sites. Failure to comply with the requirements of this section and related sections may result in suspension of work.

B. Qualifications of Employees:
1. Ensure that employees are physically qualified to perform their assigned duties in a safe manner.
2. To as great an extent as possible, do not allow employees to work whose ability or alertness is impaired because of drugs, fatigue, illness, intoxication, or other conditions that may expose themselves or others to injury.
3. Operators of vehicles, mobile equipment, hoisting equipment, and hazardous plant equipment shall be able to understand signs, signals, and operating instructions, and be capable of operating such equipment. Provide operating instructions for all equipment. Newly hired operators shall be individually tested by an experienced operator or supervisor to determine if they are capable of safely operating equipment.
1.4 ACCIDENT REPORTING
A. Reportable Accidents: A reportable accident is defined as death, occupational disease, traumatic injury to employees or the public, property damage by accident in excess of $100, and fires. Within 7 days of a reportable accident, fill out and forward to the Contracting Officer a DI-134 form, which may be obtained from Contracting Officer.
B. All Other Accidents: The Contractor shall report all other accidents to the Contracting Officer as soon as possible and assist the Contracting Officer and other officials as required in the investigation of the accident.
PART 2 - PRODUCTS
2.1 FIRST AID FACILITIES.
A. Provide adequate facilities for the number of employees and the type of construction at the site.
2.2 PERSONNEL PROTECTIVE EQUIPMENT
A. Meet requirements of NIOSH and MSHA, where applicable, as well as ANSI.
2.3 BARRIERS
A. See Division 1, Section 015100 “Temporary Facilities and Controls” for barriers.
PART 3 - EXECUTION
3.1 EMERGENCY INSTRUCTIONS
A. Post telephone numbers and reporting instructions for ambulance, physician, hospital, fire department, and police in conspicuous locations at the work site.

3.2 ESCAPE ROUTES
A. Provide and maintain adequate escape routes at all times in accordance with the Life Safety Code (NFPA 101-91). No corridor, aisle, stairway, door, or exit shall be obstructed or used in a manner that interferes with escape routes.
3.3 PROTECTIVE EQUIPMENT
A. Inspect personal protective equipment daily and maintain in a serviceable condition.
Clean, sanitize, and repair, as appropriate, personal items before issuing them to another individual.
B. Inspect and maintain other protective equipment and devices before use and on a periodic basis to ensure safe operation.
3.4 SAFETY MEETINGS
A. As a minimum, conduct weekly 15-minute "toolbox" safety meetings. These meetings shall be conducted by a foreman and attended by all construction personnel at the worksite.
B. Conduct monthly safety meetings for all levels of supervision. Notify the Contracting Officer so that he may attend. These meetings shall be used to review the effectiveness of the Contractor's safety effort, to resolve current health and safety problems, to provide a forum for planning safe construction activities, and for updating the accident prevention program. The Contracting Officer will enter the results of the meetings into his daily log.
3.5 HARD HATS AND PROTECTIVE EQUIPMENT AREAS
A. A hard hat area will be designated by the Contracting Officer. The hard hat area shall be posted by the Contractor in a manner satisfactory to the Contracting Officer.
B. It is the Contractor's responsibility to require all those working on or visiting the site to wear hard hats and other necessary protective equipment at all times. As a minimum, provide six hard hats for use by visitors. Change liners before reissuing hats.
3.6 TRAINING
A. First Aid: Provide adequate training to ensure prompt and efficient first aid.
B. Hazardous Material: Train and instruct each employee exposed to hazardous material in safe and approved methods of handling and storage. Hazardous materials are defined as explosive, flammable, poisonous, corrosive, oxidizing, irritating, or otherwise harmful substances that could cause death or injury.

SECTION 012900 PAYMENT PROCEDURES
1.1 SUMMARY
A. Measurement and payment for contract work will be made only for and under those pay items included in the Schedule of Items. All other work, labor, materials, equipment, and incidentals necessary to successfully complete the project will be considered as included in the payment for items shown. This section defines the method of measurements and basis of payment for work items listed in the Schedule of Items.
1.2 DETERMINATION OF QUANTITIES
A. The following measurements and calculations shall be used to determine contract quantities for payment.
1. For individual construction items, longitudinal and lateral measurements for area computations shall be made horizontally or corrected to horizontal measurement unless otherwise specified. Measurements for geotextiles, netting and erosion control blankets shall be along slope lines.
2. For excavation or embankment volumes, the average end area method shall be used to compute volumes. However, if in the judgment of the Contracting Officer, the average end area method is impractical, measurement shall be made by volume in hauling vehicles or by other three-dimensional methods.
3. For Structures, they shall be measured according to neat lines shown on the drawings or as altered by the Contracting Officer, in writing, to fit field conditions.
4. For items that are measured by the linear foot, such as pipe culverts, fencing, guardrail, piping, utilities, and underdrains, measurements shall be made parallel to the base or foundation upon which the structures are placed.
5. For aggregates weighed for payment, the tonnage shall not be adjusted for moisture content, unless otherwise provided for.
6. For standard manufactured items (such as fence, wire, plates, rolled shapes, pipe conduits) identified by gauge, weight, section dimensions, and so forth, such identifications shall be considered the nominal weights or dimensions. Unless controlled by tolerances in cited specifications, manufacturer's tolerances shall be accepted.
1.3 UNITS OF MEASUREMENT
A. Payment shall be by units defined and determined according to U.S. Standard measure and by the following:
1. Cubic Yard (CY): A measurement computed by one of the following methods:
Excavation, Embankment, or Borrow -- The measurement computed by the average end area method from measurements made longitudinally along a centerline or reference line.

Material in Place or Stockpile -- The measurement computed using the dimensions of the in-place material.
Material in the Delivery Vehicle -- The measurement computed using measurements of material in the hauling vehicles at the point of delivery. Vehicles shall be loaded to at least their water level capacity. Leveling of the loads may be required when vehicles arrive at the delivery point.
2. Each (EA): One complete unit, which may consist of one or more parts.
3. Linear Foot (LF): Linear feet measured horizontally.
4. Lump Sum (LS): One complete unit, which may consist of one or more parts.
1.4 METHOD OF MEASUREMENT
A. One of the following methods of measurement for determining final payment is designated on the Schedule of Items for each pay item:
1. Actual Quantities (AQ) - These quantities are determined from actual measurements of completed work.
2. Lump Sum Quantities (LSQ) - These quantities denote one complete unit of work as required by or described in the contract, including necessary materials, equipment, and labor to complete the job. They shall not be measured.

SECTION 012910 MOBILIZATION
1.1 SUMMARY
A. This item is intended to compensate the Contractor for operations including, but not limited to, those necessary for the movement of personnel, equipment, supplies, and incidentals to the project site; for payment of premiums for bonds and insurance for the project; and for any other work and operations which must be performed or costs that must be incurred incident to the initiation of meaningful work at the site and for which payment is not otherwise provided for under the contract.
1.2 MEASUREMENT AND PAYMENT
A. The measurement shall be lump sum for mobilization.
B. Payment:
1. Bond premiums will be reimbursed after receipt of the evidence of payment.
2. 50% of the lump sum, not to exceed 5% of the original contract amount, will be paid following completion of 5% of the original contract amount not including mobilization and bond premiums.
3. Payment of the remaining portion of the lump sum, up to 10% of the original contract amount, will be paid following completion of 10% of the original contract amount not including mobilization and bond premiums.
4. Any portion of the lump sum in excess of 10% of the original contract amount will be paid after final acceptance.
5. Progress payments for mobilization and preparatory work shall be subject to retainage.
PART 3 - EXECUTION (NOT USED)

SECTION 013300 SUBMITTALS
1.1 SUMMARY
A. This Section includes administrative and procedural requirements for submitting Shop Drawings, Product Data, Samples, and other miscellaneous submittals.
B. See other specification section within this package for additional requirements on submittal.
1.2 SUBMITTAL PROCEDURES
A. Coordination: Coordinate preparation and processing of submittals with performance of construction activities.
1. Coordinate transmittal of different types of submittals for related parts of the Work so processing will not be delayed because of need to review submittals concurrently for coordination.
The Contracting Officer reserves the right to withhold action on a submittal requiring coordination with other submittals until related submittals are received.
B. Processing Time: Allow enough time for submittal review, including time for re-submittals, as follows. Time for review shall commence on Contracting Officer's receipt of submittal.
1. Initial Review: Allow 14 days for initial review of each submittal. Allow additional time if processing must be delayed to permit coordination with subsequent submittals. Contracting Officer will advise Contractor when a submittal being processed must be delayed for coordination.
2. If intermediate submittal is necessary, process it in same manner as initial submittal.
3. Allow 14 days for processing each re-submittal.
4. No extension of the Contract Time will be authorized because of failure to transmit submittals enough in advance of the Work to permit processing.
C. Identification: Place a permanent label or title block on each submittal for identification.
1. Indicate name of firm or entity that prepared each submittal on label or title block.
2. Provide a space on label or beside title block to record Contractor's review and approval markings and action taken by Contracting Officer.
3. Include the following information on label for processing and recording action taken:

D. Deviations: Highlight, encircle, or otherwise identify deviations from the Contract Documents on submittals.
E. Additional Copies: Unless additional copies are required for final submittal, and unless Contracting Officer observes noncompliance with provisions of the Contract Documents, initial submittal may serve as final submittal.
F. Use for Construction: Use only final submittals with mark indicating action taken by Contracting Officer in connection with construction.
1.3 MEASUREMENT & PAYMENT
A. No separate measurement and/or payment will be made for this section. Payment shall be included with work shown in the schedule of items.
PART 2 - PRODUCTS
2.1 ACTION SUBMITTALS – (Submittals requiring Contracting Officer approval)
A. General: Prepare and submit Action Submittals required by individual Specification Sections.
1. Number of Copies: Submit three copies of each submittal, unless otherwise indicated. Contracting Officer will return two copies. Mark up and retain one returned copy as a Project Record Document.
B. Product Data: Collect information into a single submittal for each element of construction and type of product or equipment.
1. If information must be specially prepared for submittal because standard printed data are not suitable for use, submit as Shop Drawings, not as Product Data.
2. Mark each copy of each submittal to show which products and options are applicable.
3. Include the following information, as applicable:
Manufacturer's written recommendations.
Manufacturer's product specifications.
Manufacturer's installation instructions.
Manufacturer's catalog cuts.
Wiring diagrams showing factory-installed wiring.
Compliance with recognized trade association standards.
Compliance with recognized testing agency standards.

C. Shop Drawings: Prepare Project-specific information, drawn accurately to scale. Do not base Shop Drawings on reproductions of the Contract Documents or standard printed data.
1. Preparation: Include the following information, as applicable:
Dimensions.
Identification of products.
Fabrication and installation drawings.
Roughing-in and setting diagrams.
Wiring diagrams showing field-installed wiring, including power, signal, and control wiring.
Notation of dimensions established by field measurement.
2. Wiring Diagrams: Differentiate between manufacturer-installed and field-installed wiring.
D. Contractor's Construction Schedule: The contractor shall submit a Construction Schedule, for approval by Contracting Officer, in accordance with the contract provisions within 5 days of commencement of work.
E. Samples: Prepare physical units of materials or products, including the following:
1. Samples for Initial Selection: Submit manufacturer's color charts consisting of units or sections of units showing the full range of colors, textures, and patterns available.
2.2 INFORMATIONAL SUBMITTALS - (Submittals NOT requiring Contracting Officer approval)
A. General: Prepare and submit Informational Submittals required by other Specification Sections.
1. Number of Copies: Submit three copies of each submittal, unless otherwise indicated. Contracting Officer will not return copies.
2. Certificates and Certifications: Provide a notarized statement that includes signature of entity responsible for preparing certification. Certificates and certifications shall be signed by an officer or other individual authorized to sign documents on behalf of that entity.
3. Test and Inspection Reports: Comply with requirements in Division 1 Section "Quality Requirements."
B. Material Test Reports: Prepare reports written by a qualified testing agency, on testing agency's standard form, indicating and interpreting test results of material for compliance with requirements.
C. Field Test Reports: Prepare reports written by a qualified testing agency, on testing agency's standard form, indicating and interpreting results of field tests performed either during installation of product or after product is installed in its final location, for compliance with requirements.

D. Maintenance Data: Prepare written and graphic instructions and procedures for operation and normal maintenance of products and equipment. Comply with requirements in Division 1 Section "Closeout Procedures."
E. Manufacturer's Instructions: Prepare written or published information that documents manufacturer's recommendations, guidelines, and procedures for installing or operating a product or equipment. Include name of product and name, address, and telephone number of manufacturer.
PART 3 - EXECUTION
3.1 GENERAL
A. Review each submittal and check for compliance with the Contract Documents. Note corrections and field dimensions. Mark with approval stamp before submitting to Contracting Officer.
B. Approval Stamp: Stamp each submittal with a uniform, approval stamp. Include Project name and location, submittal number, Specification Section title and number, name of reviewer, date of Contractor's approval, and statement certifying that submittal has been reviewed, checked, and approved for compliance with the Contract Documents.
C. Contracting Officer will not review submittals that do not bear Contractor's approval stamp and will return them without action.
D. Submittals not required by the Contract Documents will not be reviewed and may be discarded.
E. Substitutions – Whenever materials, products, and equipment are listed by name or brand in the specifications and/or on the drawings, it is used as a measure of quality, utility, or standard. If the Contractor prefers to use any other brand or manufacturer of same quality, appearance and utility to that specified, he shall request substitution as provided below, not less than 30 days before the planned installation of the item. The Contracting Officer will approve or disapprove the request for substitution.
F. Requests for substitutions will only be considered if contractor submits the following:
1. Complete technical data including drawings, complete performance specifications, test data, samples and performance tests of the article proposed for substitution.
Submit additional information if required by Contracting Officer. All items in the above information shall be circled, tagged, or marked in some way to indicate all deviations or differences which the proposed item differs from the originally specified item.
2. Similar data as above for item originally specified. All items shall be marked to identify where/how the proposed substitution will differ.
3. A statement by the Contractor that the proposed substitution is in full compliance with the contract documents, applicable codes, and laws.

4. The Contractor shall be responsible for any effect upon related work in the project for any substitution and shall pay any additional costs generated by any substitutions.
G. Submittals shall be made as required by and called for in the drawings and specifications.
3.2 SUBMITTAL SCHEDULE
A. Submittals shall be made as required by and called for in the drawings and specifications.
3.3 REVIEW OF SUBMITTALS
A. The Contracting Officer will review submittals with reasonable promptness, indicating in writing the appropriate approval category ("Approved," "Disapproved," "Approved Subject to Corrections Shown") upon them. One (1) copy of the plan shall be returned to the Contractor and when corrections are required, the Contractor shall make such corrections. The plan shall then be resubmitted by the Contractor for approval until an unqualified approval is obtained. After unqualified approval is obtained, the Contractor shall furnish the Contracting Officer with three (3) copies of the approved submittals.
B. Review by the Contracting Officer does not relieve the Contractor from responsibility for errors which may exist in the submitted data.
C. Revisions:
1. Make revisions required by Contracting Officer.
2. If the Contractor considers any required revision to be a change, the Contractor shall notify the Contracting Officer as provided for in the General Conditions.

SECTION 014000 QUALITY REQUIREMENTS
1.1 SUMMARY
A. This Section includes administrative and procedural requirements for quality assurance and quality control.
B. Testing and inspecting services are required to verify compliance with requirements specified or indicated. These services do not relieve Contractor of responsibility for compliance with the Contract Document requirements.
1. Specified tests, inspections, and related actions do not limit Contractor's quality-control procedures that facilitate compliance with the Contract Document requirements.
2. Requirements for Contractor to provide quality-control services required by Contracting Officer, or authorities having jurisdiction are not limited by provisions of this Section.
C. See Divisions 2 through 32 Sections for specific test and inspection requirements.
1.2 DEFINITIONS
A. Quality-Assurance Services: Activities, actions, and procedures performed before and during execution of the Work to guard against defects and deficiencies and ensure that proposed construction complies with requirements.
B. Quality-Control Services: Tests, inspections, procedures, and related actions during and after execution of the Work to evaluate that completed construction complies with requirements. Services do not include contract enforcement activities performed by Contracting Officer.
C. Testing Agency: An entity engaged to perform specific tests, inspections, or both.
Testing laboratory shall mean the same as testing agency.
1.3 SUBMITTALS

D. Qualification Data: For testing agencies specified in "Quality Assurance" Article to demonstrate their capabilities and experience. Include proof of qualifications in the form of a recent report on the inspection of the testing agency by a recognized authority.

1. Date of issue.
2. Project title and number.
3. Name, address, and telephone number of testing agency.
4. Dates and locations of samples and tests or inspections.
5. Names of individuals making tests and inspections.
6. Description of the Work and test and inspection method.
7. Identification of product and Specification Section.
8. Complete test or inspection data.
9. Test and inspection results and an interpretation of test results.
10. Ambient conditions at time of sample taking and testing and inspecting.
11. Comments or professional opinion on whether tested or inspected Work complies with the Contract Document requirements.
12. Name and signature of laboratory inspector.
13. Recommendations on retesting and re-inspecting.
1.4 QUALITY ASSURANCE
A. Fabricator Qualifications: A firm experienced in producing products similar to those indicated for this Project and with a record of successful in-service performance, as well as sufficient production capacity to produce required units.
B. Factory-Authorized Service Representative Qualifications: An authorized representative of manufacturer who is trained and approved by manufacturer to inspect installation of manufacturer's products that are similar in material, design, and extent to those indicated for this Project.
C. Installer Qualifications: A firm or individual experienced in installing, erecting, or assembling work similar in material, design, and extent to that indicated for this Project, whose work has resulted in construction with a record of successful in-service performance.
D. Professional Engineer Qualifications: A professional engineer who is legally qualified to practice in jurisdiction where Project is located and who is experienced in providing engineering services of the kind indicated. Engineering services are defined as those performed for installations of the system, assembly, or products that are similar to those indicated for this Project in material, design, and extent.
E. Testing Agency Qualifications: An agency with the experience and capability to conduct testing and inspecting indicated, as documented by ASTM E 548, and that specializes in types of tests and inspections to be performed.
1.5 QUALITY CONTROL
A. Contractor Responsibilities: Unless otherwise indicated, provide quality-control services specified and required by authorities having jurisdiction.

1. Where services are indicated as Contractor's responsibility, engage a qualified testing agency to perform these quality-control services.
Contractor shall not employ the same entity engaged by the Government, unless agreed to in writing by Contracting Officer.
2. Notify testing agencies at least 48 hours in advance of time when Work that requires testing or inspecting will be performed.
3. Where quality-control services are indicated as Contractor's responsibility, submit a certified written report, in duplicate, of each quality-control service.
4. Testing and inspecting requested by Contractor and not required by the Contract Documents are Contractor's responsibility.
5. Submit additional copies of each written report directly to authorities having jurisdiction, when they so direct.
B. Special Tests and Inspections: Contractor will engage a testing agency to conduct special tests and inspections required by authorities having jurisdiction as the responsibility of Contractor.
1. Testing agency will notify Contracting Officer and Contractor promptly of irregularities and deficiencies observed in the Work during performance of its services.
2. Testing agency will submit a certified written report of each test, inspection, and similar quality-control service to Contracting Officer with copy to Contractor and to authorities having jurisdiction.
3. Testing agency will submit a final report of special tests and inspections at Substantial Completion, which includes a list of unresolved deficiencies.
4. Testing agency will interpret tests and inspections and state in each report whether tested and inspected work complies with or deviates from the Contract Documents.
5. Testing agency will retest and re-inspect corrected work.
C. Manufacturer's Field Services: Where indicated, engage a factory-authorized service representative to inspect field-assembled components and equipment installation, including service connections. Report results in writing.
D. Re-testing/Re-inspecting: Regardless of whether original tests or inspections were Contractor's responsibility, provide quality-control services, including retesting and re-inspecting, for construction that revised or replaced Work that failed to comply with requirements established by the Contract Documents.
E. Testing Agency Responsibilities: Cooperate with Contracting Officer and Contractor in performance of duties. Provide qualified personnel to perform required tests and inspections.
1. Notify Contracting Officer and Contractor promptly of irregularities or deficiencies observed in the Work during performance of its services.
2. Interpret tests and inspections and state in each report whether tested and inspected work complies with or deviates from requirements.
3. Submit a certified written report, in duplicate, of each test, inspection, and similar quality-control service through Contractor.

4. Do not release, revoke, alter, or increase requirements of the Contract Documents or approve or accept any portion of the Work.
5. Do not perform any duties of Contractor.
F. Associated Services: Cooperate with agencies performing required tests, inspections, and similar quality-control services, and provide reasonable auxiliary services as requested. Notify agency sufficiently in advance of operations to permit assignment of personnel. Provide the following:
1. Access to the Work.
2. Incidental labor and facilities necessary to facilitate tests and inspections.
3. Adequate quantities of representative samples of materials that require testing and inspecting. Assist agency in obtaining samples.
4. Facilities for storage and field-curing of test samples.
5. Preliminary design mix proposed for use for material mixes that require control by testing agency.
6. Security and protection for samples and for testing and inspecting equipment at
Project site.
G. Coordination: Coordinate sequence of activities to accommodate required quality-assurance and quality-control services with a minimum of delay and to avoid necessity of removing and replacing construction to accommodate testing and inspecting.
1. Schedule times for tests, inspections, obtaining samples, and similar activities.
1.6 MEASUREMENT AND PAYMENT
A. No separate payment will be made for the work included under this section; rather payment shall be considered to be included in the items of work listed in the Schedule of Items.
PART 2 - PRODUCTS (NOT USED)
PART 3 - EXECUTION
3.1 REPAIR AND PROTECTION
A. General: On completion of testing, inspecting, sample taking, and similar services, repair damaged construction and restore substrates and finishes.
1. Provide materials and comply with installation requirements specified in other Sections of these Specifications. Restore patched areas and extend restoration into adjoining areas in a manner that eliminates evidence of patching.
2. Comply with the Contract Document requirements for Division 1 Section "Cutting and Patching."
B. Protect construction exposed by or for quality-control service activities.

C. Repair and protection are Contractor's responsibility, regardless of the assignment of responsibility for quality-control services.

SECTION 014030 CONTRACTOR QUALITY CONTROL
1.1 SUMMARY
A. The work of this section consists of preparing and executing a quality control program.
1.2 SUBMITTALS
A. As specified in Division 1 Section “Submittal Procedure”.
B. Quality Control Plan:
1. At the time of the preconstruction conference, submit for approval a written Contractor Quality Control (CQC) plan.
2. If the plan requires any revisions or corrections, the Contractor shall resubmit the plan within 10 days.
3. The Government reserves the right to require changes in the plan during the contract period as necessary to obtain the quality specified.
4. No change in the approved plan may be made without written concurrence by the Contracting Officer.
5. The plan shall include:
A list of personnel responsible for quality control and assigned duties. Include each person's qualifications.
A copy of a letter of direction to the Contractor's Quality Control Supervisor outlining assigned duties.
Names, qualifications, and descriptions of laboratories to perform sampling and testing, and samples of proposed report forms.
Methods of performing, documenting, and enforcing quality control of all work.
Methods of monitoring and controlling environmental pollution and contamination as required by regulations and laws.
C. Contractor's Quality Control Daily Reports: Submit showing all inspections and tests on the first workday following the date covered by the report.
D. Test Reports:
1. Submit Daily Test Information Sheets with Quality Control Daily Reports.
2. Submit failing test results and proposed remedial actions within four hours of noted deficiency.
3. Submit three copies of complete test results not later than three calendar days after the test was performed.
E. Off-Site Inspection Reports: Submit prior to shipment.

F. If the CQC plan and Quality Control Daily Reports are not submitted as specified, the Contracting Officer may retain all payments until such time a plan is approved and implemented.
1.3 QUALITY ASSURANCE
A. General:
1. The quality of all work shall be the responsibility of the Contractor. Testing shall be the responsibility of an independent testing laboratory.
2. Inspect and test all work often enough to ensure that the quality of materials, workmanship, construction, finish, and functional performance is in compliance with applicable specifications and drawings.
3. Quality Control Daily Reports shall be completed by the Quality Control Supervisor.
4. Test reports shall be completed by person performing the test.
5. The Contracting Officer may designate locations of tests.
Design and complexity of the project.
Location of the project.
Characteristics of area construction labor market.
Duration of project.
Cost and type of contract.
Amount and type of off-site fabrication.
B. Quality Control Staff:
1. The Contractor’s Quality Control Supervisor may also perform the duties of Project Superintendent.
2. The Contractor’s Quality Control Supervisor shall be assigned no other duties.
3. The Contractor’s designated Quality Control Supervisor shall be on the project site whenever contract work is in progress.
4. The Contractor’s job supervisory staff may be used to assist the Quality Control
Supervisor, supplemented as necessary by additional certified testing technicians.
C. Testing Laboratory and Equipment:
1. Employ certified independent laboratories to perform sampling and testing. The testing laboratory organization shall be certified for the type of testing work to be done.
2. All measuring devices, laboratory equipment, and instruments shall be calibrated at established intervals against certified standards in accordance with NBS requirements. Upon request, measuring and testing devices shall be made available for use by the Government for verification tests.

PART 3 - EXECUTION
3.1 OFF-SITE CONTROL
A. Items that are fabricated or assembled off-site shall be inspected for quality control at the place of fabrication.
3.2 ON-SITE CONTROL
A. Notification:
1. Notify the Contracting Officer at least 48 hours in advance of the preparatory phase meeting.
2. Notify the Contracting Officer at least 24 hours in advance of the initial and follow-up phases.
B. Preparatory Phase: Perform before beginning each feature of work.
1. Review control submittal requirements with personnel directly responsible for the quality control work. As a minimum, the Contractor's Quality Control Supervisor and the foreman responsible for the feature of work shall be in attendance.
2. Review all applicable specifications sections and drawings related to the feature of work.
3. Ensure that copies of all referenced standards related to sampling, testing, and execution for the feature of work are…
