The schedule details 17 well decommissioning sites across the Mystic River and Custer Ranger Districts, with GPS coordinates provided for each location. Work items include mobilization to each site, well plugging using specified materials (bentonite grout, cement grout, or native materials), steel casing removal, concrete slab removal, pump removal, and salvage of bat gates and cattleguards. Quantities are specified for each line item, measured in lump sum (LS) for mobilization and salvage work, linear feet (LF) for casing removal, cubic yards (CY) for plugging materials, and square yards (SY) for concrete removal. The scope encompasses wells ranging from 4 feet to 80 feet in depth with casing diameters from 3 inches to 9 feet.

Vendors must provide unit pricing for each line item, with all prices rounded to the nearest whole dollar without cents. The pricing template requires submission of a 20% bid bond (not to exceed $3 million) and 100% performance and payment bonds using Standard Forms 24, 25, and 25A.
