About this file
This is an attachment to a federal janitorial services contract for the Uinta-Wasatch-Cache National Forest, Logan Ranger District (Contract Number 1240LS26Q0018) that specifies credentialing, training, and background check requirements for contractor personnel.
The attachment establishes a USDA credential matrix defining IT access and physical access requirements based on access type and duration. Contractors must ensure all employees, including subcontractor personnel, complete five mandatory annual training courses: Understanding and Interrupting Unconscious Bias (90 minutes, completion within 90 days of assignment), Anti-Harassment Training (60 minutes, completion within 90 days of assignment), Information Security Awareness (60 minutes, completion within one year of last completion), USDA Records Management (60 minutes, completion within 60 days of assignment), and Section 508 compliance training (60 minutes, completion within 45 days of assignment). All training must be completed through AgLearn except USDA Records Management, which may be paper-based. The contract also requires background checks compliant with Presidential Directive HSPD-12 for unescorted federal facility access, with contractor-provided employee information submitted to the Contracting Officer Representative within ten calendar days of contract award. The Government will conduct up to three background investigations per 12-month period at no cost; additional investigations required due to excessive personnel turnover (more than three occurrences within a rolling 12-month period) will be deducted from the contractor's final invoice. No contract price adjustment is permitted for employee time spent obtaining credentials or submitting background check information.
