About this file

This is an appendix containing a contractor inspection request and certification form for a Indefinite Delivery Indefinite Quantity (IDIQ) Performance Work Statement (PWS) contract related to hazard tree removal and fuels management services on the Rogue River Siskiyou National Forest in Oregon and California.

The form serves as a template for contractors to formally request inspection of completed work for payment purposes. Contractors must certify that specified acreage has been thinned and is ready for inspection, that inspection plots are physically marked on the ground with colored flagging, that inspection results have been recorded using the required Thinning Inspection Report, and that all work meets the Prosecution of Work clause requirements. The request requires contractors to submit the contract number, identify the specific item/sub-item/block number being inspected, provide estimated acres completed, include a map showing the location of the area ready for inspection, and provide remarks as needed. This inspection request and certification process appears to be a critical quality assurance mechanism within the contract to ensure hazardous tree removal and fuels reduction work meets specified standards before payment authorization.
