About this file
This is a bid package form template from the Oregon Parks and Recreation Department for the Champoeg State Heritage Park Reservoir Replacement Project (ITB S-63400-00011750). The document contains multiple required forms including the Bidder Information and Certifications Form (A-1), Pricing Form (A-2), Bid Security Form (A-3), and First-Tier Subcontractor Disclosure Form (A-4). The forms require detailed company information, responsibility inquiries, performance history, and certification requirements including Construction Contractors Board (CCB) registration, public works bonding, and pay equity certification. Bidders must demonstrate experience with three similar projects completed within the last three years.
The pricing form breaks down the project into eight bid items including permits and mobilization, erosion control, demolition, site preparation, reservoir foundation, water reservoir, water piping and valves, and electrical work. A 10% bid security is required with submission. The forms require disclosure of any subcontractors certified under ORS 200.055 as disadvantaged, minority-owned, woman-owned, emerging small business, or veteran-owned businesses. The template includes specific instructions for agencies to create protected Word documents that allow bidders to fill out the forms electronically.
