About this file
The document is a Quality Assurance Surveillance Plan (QASP) Matrix for a Mental Health Professionals contract (Solicitation 1202SC25Q2900) issued by the USDA Forest Service. The QASP outlines six key performance objectives for clinicians providing Critical Incident Stress Management (CISM) services: 1) Availability and preparation, 2) Ongoing assessment and coordination, 3) Adherence to International Critical Incident Stress Foundation (ICISF) guidelines, 4) Confidentiality compliance, 5) Deferment of governmental inquiries, and 6) After action review/final close-out. Each objective includes specific performance standards, surveillance methods, and acceptable quality levels (AQLs), with most requiring 95-100% compliance. The CIPS Group Leader and Lead Instructor are responsible for monitoring performance and ensuring standards are met, with surveillance conducted on an ongoing/per-incident basis.
