Solicitation No. 12024B19R0001 US Forest Service Large Fire Support Exclusive Use Helicopters National Office

TABLE OF CONTENTS

OFFEROR’S CHECKLIST

Failure to provide the following information may render your Offer unacceptable. Please use this guide to assist you in completing your package.

This checklist is an aide to the offeror to help to prepare response. Offeror has the responsibility to ensure all required documents are included in package.

Complete the SF-1449, Solicitation for Commercial Items with Blocks 17 (with phone and DUNS number) and Blocks 30a- 30c completed?

Complete Section A-1, Schedule of Items with all pricing information completed for the

Items in which the Offeror would like to be selected. Shall include both Excel and Pdf versions and shall include all prices for Daily Availability and Fixed Flight Rate from Exhibit 12 for the make and model offered for all four years with a total estimated value for each year and for the all four years.

Complete Section A-1 Make, Model, and N Number of Aircraft Offered

Complete Section A-1 with the HOGE Jettisonable or HOGE Non Jettisonable as identified by the Item.

Complete Section A-1 with Bucket or Tank Weight with the make and model of the

Bucket or Tank Offered.

Complete Section A-1 with the Helicopter Equipped Weight

Complete Section A-1 with the Relief Crew Transportation Cost (both one way and round trip)

Complete Section A-5 with the pilot crew requirement (one, two, or three pilot crew).

Complete Section A-7 with the crews work schedule

Complete Section A-9 to identify if you can operate in the identified countries and/or

Alaska

Complete Section A-12 to identify if you authorize the aircraft to be flown by a Government Pilot

Provide Points of Contact for the Company to include name, phone number, and email

Provide the Mandatory Documentation as follows:

Copy of your 133 Operating Certificate and Ops Specs

Copy of your 135 Operating Certificate and Ops Specs, if the offeror possesses a

135 Certificate 135 Operating Certificate not required.

Copy of your 137 Operating Certificate and 137 Operations Specifications

Submit a current aircraft Equipment List that documents installed equipment on the aircraft when weighed – Use Form A, Complete print and sign Form B Exhibit 21.

Copy of current weighing documentation including (must have been weighed within the last 12 months):

Scales listed by Make and Model and Serial Number

Calibration Date

Weighed by: Name, signature, and certificate type and number of individual certifying Weighing of the aircraft.

Copy of documentation (name, weight, arm, and moment of each item) of any equipment added or removed since the aircraft was weighed. Must include any equipment that will be added to meet the contract specifications. Form C Exhibit 21

Manufacturer’s technical data for weight and capacity of the tank or bucket being used as applicable. Please indicate which specific model for each aircraft being offered by circling or indicating with an arrow or similar. Do not highlight as it doesn’t show on photocopies.

Provide copies of all Hover Performance Charts (current and FAA approved) that are used to calculate payload. All charts required to be used to accurately calculate performance must be provided.

Summary table listing all modifications to the aircraft by STC number Field

Approval and include installation dates of the modifications. Submit STC’s and 337’s for additional Items required in Section A-10

Provide an accurately completed Interagency Load Calculation Form for each aircraft (Exhibit 13). Note: Make sure fuel weight and fuel duration equal)

Copy of the aircraft registration and Certificate of Airworthiness for all offered aircraft

Provide the names of Chief Pilot, Director of Maintenance (Maintenance Manager), Director of Operations (Flight Operations Manager) with supporting documentation that they meet the requirements of B-11(b)

Excerpts from your Safety Management System (SMS) Components that document how you meet the elements of Exhibit 31 to include:

Pages from your SMS

Company Operations Specifications

Company Emergency Response Plan

Examples or additional documentation that confirms compliance with Exhibit 31 and SMS

Total number of all company flight hours for both fixed and rotor wing over the past 5 years

Total number of aircraft for each of the past 5 years Not needed

Total number of accidents over the past 5 years

Form E-9 or similar information that documents the Companies organizational experience that shows time in business, number of flight hours supporting the same or similar work, number of aircraft operated

Information on past performance that is not in CPARS. This information shall be submitted by the reference using Exhibit 15 (or similar information) to Michael.mcfarlane@usda.gov

Copy of Representations or Certifications if the information is not current on sam.gov

Acknowledgement of any material Amendments to the solicitation. This is accomplished by signing and included the initial page of the Amendment (SF30)

Identify fire shelter weight as 8 pounds in equipped weight. Reference B-19 (f).
