About this file
This document is a Safety and Environmental Record Certification sheet for a VA contract opportunity (Solicitation 36C26125R0038) to improve security at the Mountain View VA Center. The certification requires contractors to meet specific safety and environmental compliance standards, including having no more than three serious, or one repeat, or one willful OSHA or EPA violations in the past three years, and maintaining an Experience Modification Rate (EMR) of 1.0 or less.
Contractors must submit two key pieces of documentation: 1) A self-certification letter confirming their safety violation status, and 2) A signed letter from their insurance carrier verifying their current EMR. If a contractor's EMR exceeds 1.0, they must provide a written explanation from their insurance carrier detailing the reasons for the higher rate and an anticipated timeline for reducing it. The document includes a table for tracking violations and EMR for the years 2022-2025, with a signature line for company representation.
