About this file
This document is a Summary of Benefits and Coverage (SBC) for the Aetna SelectSM - ACO HDHP health insurance plan offered by Schertz-Cibolo-Universal City Independent School District in conjunction with AWH Baptist Health System for the 2026 coverage period (January 1 – December 31, 2026). The plan is an Exclusive Provider Organization (EPO) with two tier provider options. Key coverage parameters include Tier 1 individual and family deductibles of $5,000 and $10,000 respectively, with corresponding out-of-pocket limits of $6,550 and $13,100. Tier 2 carries higher deductibles of $7,000 and $14,000 with out-of-pocket limits of $8,500 and $17,000. The plan covers preventive care services without cost-sharing prior to deductible satisfaction and does not require specialist referrals. In-network primary care visits, specialist consultations, diagnostic testing, and imaging services are subject to 30% coinsurance after the deductible is met, while Tier 2 providers require 40% coinsurance. Out-of-network services are generally not covered except for emergency room care and emergency medical transportation, which are covered at 30% coinsurance regardless of network status.
The plan excludes prescription drug coverage, dental care, routine eye care, glasses, acupuncture, bariatric surgery, cosmetic surgery, and long-term care services. Limited coverage is available for chiropractic care (20 visits annually), hearing aids (one per ear every three years), and rehabilitation and skilled nursing services (60 visits and 60 days annually, respectively). Hospital facility and physician services are subject to 30% coinsurance for Tier 1 and 40% for Tier 2 providers. Mental health and substance abuse services follow the same cost-sharing structure. Maternity office visits are covered at no cost as preventive services, while childbirth and delivery facility and professional services incur 30% coinsurance with Tier 1 providers. Premium amounts are to be provided separately. The plan meets Minimum Essential Coverage and Minimum Value Standards requirements. Aetna administers the plan and can be contacted at 1-888-982-3862 for additional information, grievances, or appeals.

Summary of Benefits and Coverage: What this Plan Covers & What You Pay for Covered Services Schertz-Cibolo-Universal City Independent School District : AWH Baptist Health System Open Access® Aetna SelectSM - ACO HDHP
Coverage Period: 01/01/2026-12/31/2026
Coverage for: Individual + Family | Plan Type: EPO
The Summary of Benefits and Coverage (SBC) document will help you choose a health plan. The SBC shows you how you and the plan would share the cost for covered health care services. NOTE: Information about the cost of this plan (called the premium) will be provided separately. This is only a summary. For more information about your coverage, or to get a copy of the complete terms of coverage, www.HealthReformPlanSBC.com or by calling 1- 888-982-3862. For general definitions of common terms, such as allowed amount, balance billing, coinsurance, copayment, deductible, provider, or other underlined terms, see the Glossary. You can view the Glossary at https://www.healthcare.gov/sbc-glossary/ or call 1-888-982-3862 to request a copy.
Important Questions Answers Why This Matters:
What is the overall deductible?
Tier 1: Individual $5,000 / Family $10,000. Tier 2: Individual $7,000 / Family $14,000.
Generally, you must pay all of the costs from providers up to the deductible amount before this plan begins to pay. If you have other family members on the plan, each family member must meet their own individual deductible until the total amount of deductible expenses paid by all family members meets the overall family deductible.
Are there services covered before you meet your deductible?
Yes. In-network preventive care is covered before you meet your deductible.
This plan covers some items and services even if you haven't yet met the deductible amount. But a copayment or coinsurance may apply. For example, this plan covers certain preventive services without cost sharing and before you meet your deductible.
See a list of covered preventive services at https://www.healthcare.gov/coverage/preventive-care-benefits/
Are there other deductibles for specific services? No. You don’t have to meet deductibles for specific services.
What is the out-of-pocket limit for this plan?
Tier 1: Individual $6,550 / Family $13,100. Tier 2: Individual $8,500 / Family $17,000.
The out–of–pocket limit is the most you could pay in a year for covered services. If you have other family members in this plan, they have to meet their own out–of–pocket limits until the overall family out–of–pocket limit has been met.
What is not included in the out-of-pocket limit?
Premiums, balance-billing charges & health care this plan doesn't cover. Even though you pay these expenses, they don’t count toward the out–of–pocket limit.
Will you pay less if you use a network provider?
Yes. See www.aetna.com/docfind or call 1-888- 982-3862 for a list of Tier 1 providers.
You pay the least if you use a provider in Tier 1 Provider. You pay more if you use a provider in Tier 2 Provider. You will pay the most if you use an out-of-network provider, and you might receive a bill from a provider for the difference between the provider’s charge and what your plan pays (balance billing). Be aware, your network provider might use an out-of-network provider for some services (such as lab work). Check with your provider before you get services.
Do you need a referral to see a specialist? No. You can see the specialist you choose without a referral.
