About this file

This document is a Commercially Useful Function (CUF) Evaluation and Determination Worksheet (DFW 1038) issued by the State of California Department of Fish and Wildlife for the procurement NCR-HQ-RFQ-26-E0006 regarding live Sacramento Perch needed for fishing in the city. The worksheet is used to evaluate whether small business (SB) and/or disadvantaged veteran business enterprise (DVBE) suppliers meet CUF compliance requirements for either prime contractor or subcontractor roles. The evaluation assesses five core criteria: whether the supplier is responsible for executing a distinct element of work as described in the Bidder Declaration form (DFW 1082); whether the supplier performs, manages, or supervises the work involved; whether the supplier maintains control over pricing, quality, quantity, ordering, payment, and installation; whether the work performed is normal for the supplier's business services and functions; and whether the supplier's subcontracting does not exceed industry standards. Supporting documentation required for evaluation includes the DFW 1082 form, STD 843, SB/DVBE certifications, UNSPSC codes, and website information demonstrating the supplier's normal business activities.

A supplier is deemed CUF compliant only when all five evaluation criteria receive affirmative responses. If any criterion is answered negatively, the supplier is not CUF compliant and cannot be used per SCM Vol. F 3.A2.8 guidelines. The worksheet notes that SB and DVBE certification alone does not establish CUF compliance; rather, certification must be paired with demonstrated performance of actual work by the certified entity. The form contains checkbox fields for marking supplier type (prime or subcontractor), certification type (SB, MB, or DVBE), and yes/no responses for each compliance question, with space for evaluator signature, printed name, and program identification to complete the determination.
