1 DESCRIPTION OF SERVICES

The contractor shall provide all personnel, supervision, equipment, tools, materials, and services required for pest control. This includes animal carcass removal, and mechanical rodent control cleaning methods such as sweeping and vacuuming spider cobwebs in shelters, test cells, production/storage facilities, tunnels, and administrative areas. The areas that the Contractor will service are areas that are not part of the Government Civil Engineering (CE) Office’s Charter to service/maintain. All work will be accomplished for 309 Missile Maintenance Group (MMXG), 309 Commodities Maintenance Group (CMXG) and 309 Maintenance Support Group (MXSG) buildings, shelters and tunnels listed in Appendix D located at Hill Air Force Base (HAFB), Utah

(UT) and at the Utah Test and Training Range (UTTR) located approximately 100 miles West of

HAFB.

2 BASIC SERVICES

The contractor shall remove dead rodents, birds, animal carcasses and related biological waste or debris using method outlined in Appendix F, and clean the production/storage facilities, shelters, test cells, and tunnels listed in Appendix D. The contractor shall set traps and lay poison to exterminate live rodents and spiders. The contractor shall spray areas identified in Appendix D for insects, arachnid, and other arthropod pests such as Black Widow, Hobo, Brown Recluse, Wolf, and Yellow Sack Spiders, wasps and cockroaches as well as vacuum dead insects that are left behind bi-weekly using a High Efficiency Portable Air (HEPA) Vacuum. The contractor is liable for the health and welfare of their personnel. The contractor shall ensure work is in accordance with (IAW) Center for Disease Control, HAFB Department of Public Health and Department of

Entomology guidelines. The contractor shall perform to the standards in this contract and have a minimum of five (5) years’ experience.

2.1 MECHANICAL CLEANING TASKS

2.1.1 Each time the facilities are cleaned, the contractor shall perform mechanical cleaning by removing dirt and debris from inside the facilities to include floors, maintenance pits, lifting grates and cleaning the drains mechanically. The

Government Representative (GR) is the shop supervisor for the building being mechanically cleaned. If the GR determines that the drains need to be flushed, the contractor shall flush the drains with water using the Government’s water source/faucets. The contractor shall mechanically clean foundations, shelves, ledges, and around and under missile rails. The contractor shall mechanically clean and disinfect inside facilities, equipment, impeller housing, under elevated flooring, and underground tunnel (below ground level) maintenance pits to meet the safety needs of the OO-ALC as identified in this PWS. The contractor shall remove rodent droppings, carcasses, nests, litter, dust, soil, and any other biological debris related to rodents and winch housings. The contractor shall mechanically clean the brushes above the doors by hand and drag nests and branches from underneath and behind them to remove anything that power washing does not clean out and mechanically clean/disinfect area and vacuum, when needed as well as clean and/or power wash animal or bird feces from breezeways and building exteriors.

2.1.2 The contractor shall provide a cleaning schedule to the Contracting Officer

Representative (COR) and Program Manager (PM) within ten (10) business days following contract award, and within three (3) business days following any changes, identifying the estimated time projected to clean buildings listed in

Appendix D. (CDRL A003)

2.2 Rodent Control

2.2.1 The contractor shall implement rodent control measures by placing multi-traps equipped with a minimum of one (1) one-ounce bar of poison per trap in each defined area. The contractor is responsible for ensuring that all traps are properly installed and maintained, and that poison levels are consistently monitored and replenished as necessary to effectively control rodent activity. The contractor shall not use poison in administrative areas, instead only place traps. The contractor shall implement comprehensive measures to manage and control larger rodents (e.g., rats) within the buildings at UTTR that require them. Setting up and maintaining appropriate traps and baits specifically designed for larger rodents and providing ongoing monitoring and maintenance to ensure effectiveness. The contractor must have the necessary equipment and expertise to handle larger rodents. All interventions must be documented and reported regularly to ensure compliance with military safety and health standards. As a rule, the number of traps and poison for each facility shall be as follows:

2.2.1.1 There shall be at least one (1) multi-trap placed with the appropriate bait on the trap for every 2,000 square feet of facility.

2.2.1.2 Every facility with less than 2,000 square feet total shall have a minimum of two (2) multi-traps unless the authorized GR deems a variation is necessary.

2.2.1.3 For every multi-trap placed there must be at least one (1) one-ounce bar of poison placed in each non-administrative facility.

2.2.2 The contractor shall place the traps and poison in the areas most appropriate for effective rodent/pest control in coordination with the GR.

2.2.3 The contractor shall replace the poison, reset traps, remove carcasses, and perform mechanical cleaning every three (3) months (once a quarter).

2.2.4 The contractor shall comply with Air Force Instruction (AFI) 32-1053, Pest

Management Program and provide monthly data on pesticide use IAW AFI 32-

1053 (CDRL A001).

2.2.5 The contractor shall provide a monthly status report identifying the number of carcasses found in each building during the cleaning process. The report shall be submitted to the (COR) (PM) by the 5th calendar day of each month (CDRL

A001).

2.2.6 The contractor shall identify, by building number and specific location, areas needing repair (such as door sweeps, holes, cracks, drainage, etc.) to deter or prevent rodents or spiders monthly from entering these facilities. The report shall include date performed, application site, applicator name/certification number, name and amount of pesticide used and be submitted to the OO-ALC (COR), and the 75 Civil Engineering Section (CES) Entomology Program Manager (EPM), monthly (CDRL A001).

2.2.7 The contractor shall provide the Bioenvironmental Office with the Material

Safety Data Sheet (MSDS) or Safety Data Sheet (SDS) information prior to initial usage of product (CDRL A002).

2.2.7.1 The contractor shall ensure all applications of pesticide are made by personnel certified by the State of Utah in the appropriate pest control categories.

2.2.7.2 The contractor shall ensure all pesticides are registered by the

Environmental Protection Agency (EPA) and used according to label instructions. Currently approved pesticides are listed in

Appendix E. This list is not all inclusive and is subject to change.

The contractor shall use only pesticides listed in the Approved

Standardized Department of Defense (DoD) Chemical Listing.

The contractor shall get approval from the OO-ALC

Environmental, Safety and Occupational Health (ESOH) Office and the Pest Management Office prior to use of products that are not listed in Appendix E. The contractor shall contact the

Bioenvironmental and 75 CES Entomology Program Managers prior to using any pesticides not listed in Appendix E.

2.2.7.3 The contractor shall furnish copies of pesticide labels and SDSs to the ESOH, HazCell Project Manager, Hazardous Material

Compliance Office within 30 calendar days of contract award, or prior to initial usage of product if changes occur during the PoP

(CDRL A002).

2.2.8 The contractor shall not use cleaning measures that will adversely affect the weapon systems in the facilities and their related explosives. The contractor shall follow safety measure listed in Appendix F.

2.2.9 The contractor shall take precautions during cleaning procedures to prevent hardware, tools, liquids, or personnel from bumping, hitting, scraping, or even touching any part of the test equipment (e.g., test stands) and weapon systems

(e.g., motors, boosters, nozzles, etc.). It is imperative that special care is taken to ensure that no grounding wires be disconnected, and no part of the weapon system is sprayed with any chemicals.

2.2.10 The contractor shall clean all Real Property Installed Equipment (RPIE) such as winch housing controls, mounting rails, etc. During cleaning, the contractor shall not open electrical panel boxes, air handling units (AHUs), compressors, air conditioners (A/C), and heating units within any facility.

2.2.11 The contractor shall not use prohibited devices IAW Safety briefing given by the

GR, prior to being escorted around the facilities. The contractor shall not have radio frequency devices (e.g., cell phone, radios, key fobs, tablets, computers, iPads, etc.), cameras, or picture-taking devices in their possession at the buildings or areas being serviced. These buildings are listed in Appendix D.

2.2.12 The contractor shall carry two (2), 10-lb Bicarbonate (BC) type fire extinguishers in their vehicle(s) in case a spark from their vehicle or a spark from other equipment should occur. Contractor employees shall be trained in the use of these fire extinguishers.

2.2.13 The contractor shall remove bird excrement, feathers, nesting material, and any other bird debris from buildings on a bi-annual basis. The debris is found on the outside of and above the large sliding cargo doors (over 10 feet) and in the trenches beneath these doors. The contractor shall power wash the outside of

Buildings 1320, 1321, 1403, 1404, 1405, 2329, and 2330 on a bi-annual basis.

The electrical power must be turned off prior to pressure cleaning. The contractor shall not spray directly on electrical equipment or door seals. A manual drive shaft and manual disconnect lever (for manual operation of these doors) are located at the top of each of the doors. The contractor shall ensure the drive shaft disconnect lever and areas around them are clean of bird debris. The contractor shall notify the GR (Shop Supervisor) and COR when these buildings have been power washed.

2.2.14 The contractor shall comply with procedures established by Federal, State, and local Government Offices that detail how to minimize risk and control Hantavirus and other potential animal borne disease identification and cleanup. For further information on the Hantavirus disease and other potential animal borne disease, to include cleanup and disposal procedures, contact the HAFB Public Health

Flight at (801) 586-9768.

2.2.15 Over and Above (O&A) Work: When required, the contractor shall respond to emergency pest control, contingency operations, or other special circumstances not covered under the fixed price, scheduled arrangement. This work shall be performed utilizing the O&A contract line-item number (CLIN).

2.2.15.1 The contractor shall respond to O&A situations within 24 hours after notification by the COR or the Contracting Officer (CO).

The CO or COR will notify the Contractor via email that the emergency pest control is required under the O&A CLIN.

2.2.15.2 The Contractor shall not perform O&A work unless authorized by the CO or COR. The contractor shall not consider direction or requests for O&A from any other government personnel, including building managers, shop supervisors, etc. as valid authorization.

2.2.16 The contractor shall conduct bi-weekly visits to the Utah Test and Training

Range (UTTR) location to thoroughly inspect for any traps triggered by rodents.

During this bi-weekly visit, contractor shall replace any activated traps and take all necessary actions to effectively manage and mitigate rodent activity. This includes proper removal and disposal of all spent traps and animal carcasses and any other necessary pest mitigation activities.

2.2.17 The contractor shall be responsible for conducting bi-weekly visits to the UTTR location to thoroughly inspect for any traps triggered by rodents. The contractor must promptly replace any activated traps and take all necessary actions to effectively manage and mitigate rodent activity. This responsibility includes ensuring that inspections are comprehensive, timely, and that all issues are addressed to maintain operational standards.

2.3 SPECIFIC TASKS

2.3.1 The contractor shall ensure all buildings/areas identified in Appendix D are clean with respect to all pests and pest control measures.

2.3.2 The contractor shall follow all Occupational Safety and Health Act (OSHA) standards when cleaning inside/outside the buildings for spiders, webs, and other insects. The contractor shall clean above cargo doors inside/outside the buildings, which may be higher than 10 feet and lay new traps and poison.

2.3.3 If the contractor identifies the need to move an item or equipment that could bump, hit, scrape, or touch any part of the weapon system (e.g., motors, boosters, nozzles, etc.) or cause damage to property, the contractor shall get approval or assistance as necessary from the GR (Government Escort or Shop Supervisor) prior to moving the object.

2.3.4 The contractor shall clean between the missile storage racks in all buildings, making sure to reach five feet from the ends of the missile motors that are stored.

The contractor also needs to clean all walkways between the missile storage racks.

2.3.5 During the pre-inspection, or while cleaning any building, if the contractor finds evidence of potentially damaged, suspect, or asbestos-containing materials, they shall be identified to the COR in the monthly report (CDRL A001) and the contractor shall not clean these areas.

2.3.6 The contractor shall be responsible for disposal of all waste associated with pest control to include but not limited to feces, carcasses, spent traps and poison and other refuse in accordance with local, State, and Government regulations.

2.3.7 The contractor shall maintain the temperature and humidity in each building as they clean per the following directions:

2.3.7.1 Doors shall be kept open for no longer than three (3) hours and then closed for four (4) hours before they can be re-opened. No more than one (1) cargo door shall be open at one time. This will allow the temperature to be maintained within the desired limits.

2.3.8 The contractor shall use a HEPA Vacuum and follow proper usages guidelines.

2.3.9 Considering the storage bunker can have an explosive dust atmosphere, the vacuum cleaner must be intrinsically safe with regards to Electro-Static

Discharge (ESD). This includes but is not limited to; being equipped with a grounded (three-wire) power cord, and proper bonding of all internal, metal components.

2.3.10 Considering the ESD and the missiles are in a storage condition, the vacuum cleaner must not have an electrical field intensity greater than 140 dBuV/m at any frequency. The contractor shall have the technical specifications for the device on hand for government validation of these specifications when in use.

2.4 SPECIFIC TASKS FOR BUILDING TYPES

2.4.1 The contractor shall clean the outside of buildings identified in paragraph 2.2.13 above for bird excrement, feathers, and nesting material. The contractor shall use high pressure cleaning to remove debris. The contractor shall not spray directly on electrical equipment and door seals. The electrical power must be turned off prior to any high-pressure cleaning. The contractor shall contact the GR

(Government Escort or Shop Supervisor) to have the power turned off.

2.4.1.1 In buildings where applicable, the contractor shall be required to remove the winch clutch covers to clean and reinstall after cleaning and mechanically clean inside the heating ducts. The contractor shall not spray directly on the clutch, cable, and cable spool. The contractor shall mechanically clean the rail structure and under the missile/motors in applicable buildings and clear spiders from facility.

2.4.2 The contractor shall clean under the raised floor in buildings 30025, 60005, 60020 at the UTTR. The contractor shall not step or walk on any area under the raised floor. The contractor shall use caution when spraying and vacuuming these areas. The tunnels are non-entry permit required, confined spaces.

2.5 PROHIBITED PEST CONTROL MEASURES

2.5.1 Electrically Operated Devices

2.5.1.1 Electromagnetic exclusion or control devices, ultrasonic repellent or control devices, and outdoor electrocuting devices are not approved for use on (DoD) installations. Insect electrocuting devices are permitted to use indoors to control flying insects when selected, purchased, located, and used in accordance with

AFPMB TG 29.

2.5.2 Paints and Coatings Containing Pesticides

2.5.2.1 Pesticide-containing paints are not approved for use on DoD property, including:

2.5.2.2 Interior and exterior pesticide-containing paints applied to structural surfaces, such as walls, ceilings, and siding.

2.5.2.3 Pesticides formulated and labeled as paint additives.

2.5.2.4 Fungicide-containing paints that inhibit mildew may be used if application directions include no special restrictions due to the fungicide.

2.5.2.5 Approved marine anti-fouling compounds or coatings may be applied to protect surfaces of watercraft.

2.5.3 Preventive or Scheduled Pesticides Treatments

2.5.3.1 Regularly scheduled, periodic pesticide applications are not approved for DoD property, except in situations where the PM plan clearly documents that no other technology or approach protects personnel or high-value property. DoD installations will not use preventive pesticide treatments, including automated misting devices, unless the appropriate Preventative Maintenance

Coordinator (PMC) has given approval based on current surveillance information or records documenting past disease vector or pest problems requiring this approach.

2.6 MANAGEMENT CONTROL PLAN (MCP)

2.6.1 General Plan Provisions

2.6.1.1 The MCP shall describe the contractor’s approach to

accomplishing the requirements of this PWS.

2.6.2 Roles and Responsibilities

2.6.2.1 The MCP shall identify an individual within the company to serve as the primary point of contact between the government and contractor.

2.6.3 Service Schedule

2.6.3.1 The MCP shall include a detailed service schedule based on the scope of work outlined in the PWS. The schedule is subject to change after award based on government needs.

3 SERVICE SUMMARY

The services summary table for the basic contract is listed below. Specific performance objectives and thresholds unique to the Task Order requirement will be identified in its associated PWS.

Circumstances or external influences beyond the control of the Contractor which impact the ability of the Contractor to meet performance standards will not affect Contractor performance measurements. 4 CONTRACT DATA REQUIREMENTS LIST

4.1 Contract data/information shall be delivered, as assigned by the PM, electronically IAW the corresponding requirement for each Data Item listed in Table 4-1 (below). The contractor shall have no-more-than one error per quarter for all CDRL submittals. The contractor shall submit CDRLs according to the due date indicated and have no-more-than one CDRL submitted late and after the identified due date per quarter. Grammatical errors do not count as errors. Substantive mistakes shall be corrected within two business days and corrective action taken to prevent recurrence of inaccurate and/or late CDRLs.

5 GENERAL INFORMATION

5.1 GOVERNMENT FURNISHED PROPERTY

5.1.1 There will be no Government furnished property issued under this contract.

5.1.2 The Government will provide telephone service limited to matters related to the performance of this contract (Official Business/Local Calls Only).

5.2 GOVERNMENT FURNISHED SERVICES

5.2.1 Base Civil Engineering

5.2.1.1 The Government will provide fire prevention and protection;

inspection and maintenance of Government furnished fire extinguishers and systems, pest control, and ground maintenance.

At HAFB the Fire Department telephone extension is 911 for emergencies and (801) 777-3021 for routine calls. At UTTR and

Oasis, the Fire Department telephone extension is 911 for emergencies (using a land line only) and for emergencies and routine calls using a cell phone (801) 777-1555 (dialing 9-1-1 on a cell phone at UTTR/Oasis will direct the caller to Wendover or

Toole).

5.2.2 Employee Training

5.2.2.1 The Government (OO-ALC) will provide training for explosive safety, emergency preparedness, and mishap preservation and security awareness only if unavailable elsewhere.

6 GENERAL INFORMATION

6.1 INSTALLATION-SPECIFIC SAFETY AND HEALTH STANDARDS

6.1.1 Volunteer Protection Program (VPP)

6.1.1.1 Hill AFB, along with the entire Air Force, has adopted the VPP as recognized by (OSHA). Consequently, VPP impacts all

“applicable contractors” operating on Air Force Installations. It is the contractor’s responsibility to ensure its employees and managers have a comprehensive understanding of VPP as well as full compliance with OSHA requirements. Detailed information on VPP is available on the OSHA website. Contractors, whether regularly involved in routine site operations or engaged in temporary projects such as construction or repair, must follow the safety and health rules of the installation or VPP site.

6.1.1.2 VPP requirements apply to contractors whose employees work more than 1,000 hours per quarter. The contractor shall flow down these requirements to all subcontractors who qualify as applicable contractors under this contract.

6.1.1.3 Contractors are required to provide from their OSHA 300 information the Total Case Incident Rate/Days Away Restricted or Transferred (TCIR/DART) rates and submit them to the CO as part of the installation’s annual OSHA VPP self-evaluation report. (CDRL A004)

6.1.2 Safety Program

6.1.2.1 The contractor shall implement a safety program that ensures protection of Government personnel and property. (See

Appendix C). The program shall be submitted within 15 calendar days of contract award. Program shall be in accordance with Appendix C and be revised as the contractor's situation changes. (CDRL A005)

6.1.2.2 The contractor shall comply with Appendix C of this PWS and obtain acceptance of the plan by COR before the start of the performance period.

6.1.2.3 In the event of an accident/mishap, the contractor shall take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence. A written or email copy of the mishap/incident notification shall be sent within three calendar days to the Government Representative, who will forward it to

OO-ALC/SEG safety office. (CDRL A006)

6.1.2.4 The contractor must adhere to all applicable health and safety regulations, including the proper handling and disposal of pest control chemicals.

6.1.2.5 Compliance with local, state, and federal regulations is mandatory, including those related to pesticide use and environmental protection.

6.1.2.6 The contractor must also maintain up-to-date safety data sheets

(SDS) for all chemicals used.

6.1.3 Environmental Controls

6.1.3.1 The contractor shall ensure their employees are made aware that the performance of these services will occur in industrial areas.

The industrial complex at Hill Air Force Base has the potential to expose workers to hazardous materials, which may include, but are not limited to, hexavalent chrome, cadmium, beryllium, lead, etc. While the contractor will not perform duties in a marked and signed regulated area (where exposures are expected to exceed the permissible exposure limits set by 29 CFR1910 Subpart Z, there may be contact hazards with these materials during cleanup of dust, metal shavings, etc. The contractor shall ensure proper protective measures and training are taken to protect contracted employees from hazards, including controlling airborne dust.

Specifically, the contractor is required to use a moist floor sweep to minimize dust during sweeping activities in Buildings 40060, 60075, and 60077.

6.1.3.2 Compliance with Laws and Regulations. The contractor shall be knowledgeable of and comply with all applicable Interstate, Federal, State, and Local laws, regulations, and requirements regarding environmental protection. In the event environmental laws/regulations change during the term of this contract, the contractor is required to comply as such laws come into effect. If there is an increase or decrease in cost because of the change, the contractor shall inform the CO pursuant to notice requirements and negotiate a modification to the contract.

6.1.3.3 Notification of Environmental Spills. If the contractor spills or releases any substance contained in 40 CFR 302, including hazardous materials, into the environment, the contractor or its agent shall immediately call 911. If the call is made from an on base phone it will go directly to Hill AFB dispatch. If using a cell phone, it will go off base, ask the 911 dispatcher to be transferred to Hill AFB. The liability for the spill or release of such substance’s rests solely with the contractor and its agent.

6.1.3.4 Material Storage and Use. The contractor shall follow manufacturer’s guidelines and professional recommendations for control of humidity, temperature, cleanliness, and materials handling. This includes hazardous materials.

6.1.3.5 Targeted Chemicals. The contractor is not required to purchase or use any Class I Ozone Depleting Compounds (ODCs). The contractor shall not use any Class I ODCs or EPA 17 chemicals without notifying the CO in advance of the anticipated usage

(quantity, rate, and time period) of each such substance.

6.2 SECURITY REQUIREMENTS

The contractor shall comply with all Security, Force Protection, and Antiterrorism requirements enforced at each installation. The contractor shall comply with local unit in and out-processing procedures for all contractor personnel to ensure proper security-related briefings and debriefings are accomplished.

6.2.1 BASE ACCESS

6.2.1.1 All security requirements will be IAW the Standard Form (SD)

312, Classified Information Non-Disclosure Agreement. DD 254 is not required. The contractor is required to be escorted into each facility in the MAMs area and at UTTR.

6.2.1.2 The contractor shall complete a “Request for Identification

Credential (Hill AFB Form 496)” and Optional Form 36, for each employee requiring access to Hill AFB. The requests shall be submitted to the COR or GR for signature then to OO-

ALC/OMD Security for processing. The Hill AFB Form 496 can be used to obtain a Base and Government computer, Defense

Biometric Identification System (DBIDs) Access ID, and/or

1199 Flight-Line Badge.

6.2.1.3 The contractor is required to comply with all security regulations and directives as identified herein and other security requirements as are shown elsewhere in this contract. The contractor shall comply with Standard Form (SD) 312, Classified

Information Non-Disclosure Agreement.

6.2.1.4 The contractor shall obtain a Building ID and access card from the appropriate Building Security Manager for all their employees.

6.2.2 CONTROLLED AREA ACCESS

6.2.2.1 The contractor shall gain access to controlled areas only by coordinating and scheduling an escort through the COR or other government representative. The contractor shall comply with the instructions of the assigned escort at all times to ensure safety and security within these areas.

6.2.3 PHYSICAL SECURITY

6.2.3.1 The contractor shall be responsible for safeguarding all

Government property they have access to. At the close of each work period, Government facilities, property, and materials shall be secured.

6.2.3.2 Lock Combinations (NOTE: Optional; depending on local requirements).

6.2.3.3 The contractor shall establish and implement methods of ensuring any lock combinations are not revealed to unauthorized persons. The contractor shall ensure lock combinations are changed when personnel having access to the combinations no longer have a need to know such combinations. These procedures shall be included in the contractor’s Quality Control

Plan. (CDRL A007)

6.2.4 Security Incidents

6.2.4.1 Should a security incident occur, the contractor shall take reasonable and prudent action to establish control of the scene, prevent further violation, and to preserve evidence until released by proper authority; Immediately report to the site location

Group Security Manager all available facts related to each instance, take precautions as the Security Manager may reasonably require for security purposes, cooperate fully and assist Government personnel as the Government investigates the violation.

6.2.4.2 Participation shall last until the investigation is complete.

6.2.4.3 The contractor shall create an Incident/Mishap report when a contract employee loses a CAC or Restricted/Controlled Area

Badge Line badge. The Incident/Mishap report must be submitted no later than 1 business day. (CDRL A006)

6.2.5 Communications Security

6.2.5.1 All communications with DoD organizations are subject to

Communications Security (COMSEC) review.

6.2.5.2 The DoD authorizes the military departments to conduct

COMSEC monitoring and recording of telephone calls originating from, or terminating at, DoD organizations.

6.2.5.3 The Government advises the Contractor that at any time they place a call to, or receive a call from, a DoD organization, they are subject to COMSEC procedures.

6.2.5.4 The Contractor assumes the responsibility for ensuring wide and frequent dissemination of the above information to all employees dealing with official DoD information.

6.2.6 Operations Security (OPSEC)

6.2.6.1 Contractors who have access to Ogden Air Logistics Complex

(OO- ALC) facilities or critical information shall have OPSEC training IAW AFI 10-701, Operations Security (OPSEC), (Complete reference in Appendix E). Initial training will be provided by Group SM and lasts approximately one hour, with recurring annual training lasting approximately one-half hour.

Contractors shall immediately comply with Installation and

Group OPSEC programs.

6.2.6.2 Public release of any information is not authorized and, if requested, will comply with Public Affairs and all other OPSEC measures to ensure the best balance of lawful disclosure for the

American public and denial of advantage for our adversaries.

6.2.6.3 No photography, or videotaping, is allowed on the installation without approval from the owning organizations Group

Director/Commander on an approved photography letter from

Public Affairs. This includes the use of personal cell phones with picture and video capability.

6.2.6.4 Critical Items/Information shall not be discussed outside normal contract conditions. A Critical Items/ Information List (CIL) can be provided and should not be considered all inclusive.

6.2.7 Controlled Unclassified Information (CUI) generated by the contract, and its employees, shall ensure protective measures are applied. ALL DoD unclassified information MUST BE REVIEWED AND APPROVED FOR RELEASE before it’s provided to the public in accordance with DoDD 5230.09, Reference J.

6.2.7.1 Disclosure of information is prohibited for release of unclassified information to the public without approval of the contracting activity.

6.2.7.2 Contractors will comply with CUI requirements detailed in

DoDM 5200.01-V4, DoD Information Security Program:

Controlled Unclassified Information (CUI).

6.3 EMPLOYEE TRAINING

6.3.1 The contractor shall be required to contact the COR within seven (7) business days after contract award to obtain government provided trainings that will need to be completed by each contractor that will be involved with this contract.

6.3.2 The contractor shall complete all mandatory training and certifications and ensure they are provided to the COR.

6.3.2.1 The contractor shall provide their own Hazardous

Communication training and Forklift training to their employees if they will be using a Forklift, JLG high lift or any type of aerial lift.

6.3.2.2 The contractor is responsible for renting their own forklift, JLG or aerial lift equipment if they are needed.

6.3.2.3 The contractor shall provide fire extinguisher training to their employees and be documented through the COR.

6.3.3 Contractor personnel must receive Explosive Safety Awareness Training if their duties require entry into the Explosive Clear Zone. This training will be provided by the OO-ALC Weapons Safety Manager for all area located at HAFB. For

UTTR and Oasis, contractor personnel must receive Explosive Safety Awareness

Training that will be provided by the Additional Duty Weapons Safety Manager

(ADWSM), 75 CEG/CEUE, (801) 775-6209. All other safety issues will be directed to the CO. All training will be completed during the orientation period of this contract. This training must be accomplished before an Air Force (AF) Entry

Control Card (AF Form 1199) will be issued.

6.3.4 Additional training may be required if contractor requires access to special areas or equipment.

6.4 CONFLICTS OF INTEREST

6.4.1 The contractor shall not assign to contract or task order performance any person, including any employee of the U.S. Government, if employing that person would create a conflict of interest under any law, regulation, or policy of the United States

Government.

6.4.2 Contractor shall not assign to contract or task order performance, any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval according to DoD Directive 5500.7-R and any other applicable regulation or policy of the Department of the Air Force or its subordinate commands, as applicable, including but not limited to Air Force

Materiel Command.

6.5 QUALITY CONTROL PLAN

6.5.1 The contractor shall establish a complete Quality Control Program (QCP) via a

Quality Control Plan to ensure the requirements of this contract are provided as specified. The contractor's quality management program shall comply with the minimum quality management systems elements specified in the American National

Standards Institute (ANSI), American Standards Organization (ASO), and

International Standard Organization (ISO) family of standards. The report shall be submitted to the COR and PM during delivery of the contract proposal. (CDRL

A007)

6.5.1.1 Performance Standards and Monitoring: The quality

management plan shall identify the service summary items and the contractor’s approach to meeting these performance goals.

6.5.2 Quality Assurance

6.5.2.1 The Contracting Officer Representative (COR) is the authorized

Government representative(s) who will perform assessments of the Contractor’s performance. After contract award, the identity of the COR(s), with a letter defining duties and authority, will be promptly furnished to the Contractor.

6.5.2.2 The COR(s) or alternate(s) will inform the task manager in person when discrepancies occur and will request corrective action. The COR(s) or alternate(s) will make a notation of the discrepancy on the assessment checklist with the date and time the discrepancy was noted and will request the task manager (or authorized representative) to initial the entry on the checklist.

6.5.2.3 Any matter concerning a change to the scope, prices, terms, or conditions of this contract shall be referred to the Contract

Officer (CO).

6.5.2.4 The services to be performed by the contractor during the period of this contract are always subject to review by the CO or authorized representative(s).

6.5.2.5 The Government will periodically evaluate the contractor’s performance by appointing a representative(s) to monitor performance to ensure services are received. The Government representative will evaluate the contractor’s performance through inspections and receipt of complaints from base personnel.

6.5.2.6 The Government may inspect each task as completed or increase the number of quality control inspections if deemed appropriate because of repeated failures discovered during quality control inspections or because of repeated customer complaints.

6.5.2.7 The government may periodically schedule program

management reviews. The contractors authorized representative shall participate in these reviews within five (5) business days of notice from the government.

6.6 HOURS OF OPERATION

6.6.1 The contractor shall perform services during the standard operating hours of 0800 to 1600, Monday through Friday. Any deviation from this schedule must be approved in advance by the government. A deviation is considered early or late arrival by more than one (1) hour from the originally scheduled time.

Additionally, the contractor must notify the government of any anticipated absences or adjustments to the work schedule in a timely manner. Special or emergency situations (e.g., weather-related situations, base closures, reduced activity around Christmas/New Year’s Day) may necessitate the contractor operating on a reduced basis. The CO or COR will notify the contractor of any reduced hours, otherwise the contractor shall maintain the agreed upon schedule.

6.6.2 The contractor shall perform the services required under this contract on the following days and hours (except on Federal Holidays):

• MMXG Buildings at HAFB and UTTR:

• Monday through Thursday 0700 – 1500

• MMXG UTTR Buildings (40060, 60060, 60075 and 60077):

• Monday through Wednesday 0600 – 1630

• Thursday 0600-1300

• MMXG Buildings at UTTR Oasis:

• Monday 1200 – 1700

• Tuesday and Wednesday 0700 – 1700

• Thursday 0700 – 1200

• CMXG Buildings at HAFB:

6.6.3 Business hours are subject to change due to Government worksite availability, i.e., Government furloughs. The contractor shall work, with prior approval of the

CO, extended hours to ensure timely completion of work at no additional cost to the Government. In the event of a lock-down for the Strategic Arms Reduction

Treaty (START) with Russians inspectors, the cleaning must cease until the inspections are finished. This is usually only a 24-hour delay in which the contractor would have to stand down.

6.6.4 Unless otherwise required by the Government, the Contractor shall not have access to Government facilities to work on the following federal holidays: New Year’s

Day, Martin Luther King’s Birthday, President’s Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Columbus Day, Veterans’ Day, Thanksgiving Day, and Christmas Day. If the holiday falls on Sunday, it is observed on Monday. If the holiday falls on Saturday, it is observed on Friday.

6.6.5 Down Days/Weather Days. The Government will notify the contractor when official notice has been given for Energy/Weather Down days. The contractor is not expected to work on any Energy/Weather Down days.

6.6.5.1 Rescheduling. Down Days/Weather days may be rescheduled if conditions/emergency operations require.

6.7 CONTINUATION OF ESSENTIAL DOD CONTRACTOR SERVICES DURING

CRISIS

6.7.1 IAW AFI 63-124, paragraph 2.7.1, and DODI 3020.37, this requirement is not considered mission essential.

6.8 CONSERVATION OF UTILITIES

6.8.1 The contractor shall ensure that contractor employees follow applicable Air Force base utilities conservation practices to reduce waste as much as practicable. The contractor shall be aware of OO-ALC Energy Policy. A copy shall be provided upon request. Contractors shall contribute to the effectiveness of the OO-ALC Energy

Management System (EnMS), including the achievement of its objectives and energy targets. Through this contribution, the benefits of improved energy efficiency are realized. It is expected that the contractor shall ensure their activities are in line with improving OO-ALC energy performance. Contractors found negligent of these requirements can have a negative impact.

6.8.2 The contractor shall be responsible for operating under conditions that prevent the waste of utilities, which include the following:

6.8.2.1 Lights shall be used only in areas where and when work is being performed.

6.8.2.2 Mechanical equipment controls for heating, ventilation, and air conditioning systems shall not be adjusted by the contractor or by contractor employees unless authorized.

6.8.2.3 Water faucets or valves shall be turned off after the required use.

6.8.2.4 Government telephones, computers, fax machines, copy

machines, and other equipment shall be used only for official

Government business.

6.9 FOREIGN OBJECT DAMAGE (FOD) AND TOOL CONTROL

6.9.1 The contractor shall follow the applicable guidance in chapters/paragraphs/sections of AFI 21-101 and HQ AFMC, Hill AFB, and 309 MXW supplements to the AFI

21-101 concerning base tool (chapter 10 section B), and FOD control policies

(chapter 14, and sub paragraphs designated as 41), daily. Cloth rags are considered tools and will follow the same regulations and procedures as tools.

6.9.2 If tools, and/or equipment are lost, the contractor will report the loss to the GR/COR by either phone call or email no later than two hours after discovery. If tools are lost or not accounted for, then the contractor shall implement the lost tool procedures of

AFMC supplement to AFI 21-101, chapter 10, section B.

6.9.3 Contractors performing functions in industrial areas on the center shall have a method for controlling and accounting for all tools used. All contractors working on the center shall inventory their tools at the beginning and end of each shift and at the end of each task to check for any lost or missing tools.

6.10 INHERENTLY GOVERNMENTAL FUNCTIONS/PERSONAL SERVICES

6.10.1 None of the functions to be performed under this requirement are inherently

Governmental. Additionally, no part of this PWS or the Basic contract shall be construed to require the Contractor to provide personal services to the Government.

The Contractor shall comply with this PWS in its entirety.

6.10.2 The Government will neither supervise contractor employees nor control the method by which the Contractor performs the required tasks. Under no circumstances shall the Government assign tasks to, or prepare work schedules for, individual contractor employees. It shall be the responsibility of the contractor to manage its employees and to guard against any actions that are of the nature of personal services or give the perception of personal services. If the contractor believes or has reason to believe any actions constitute or are perceived to constitute personal services, it shall be the contractor’s responsibility to notify the GR, COR, and CO immediately.

These services shall not be used to perform work of a policy, decision-making, or management services, i.e., inherently Governmental functions. All decisions relative to programs supported by the contractor shall be the sole responsibility of the

Government.
