About this file

This is a Minimum Insurance Requirements document issued by Harris Health, a Texas-based healthcare organization, outlining mandatory insurance coverage standards for contractors engaged in the Room Upgrade and Gamma Camera Replacement project at Lyndon B. Johnson Hospital. The project carries an estimated budget of $320,225 and falls under the project category of less than $25 million. Contractors must maintain primary commercial insurance at their own expense throughout the contract term, with coverage types including Workers' Compensation (statutory limits per Texas law), Commercial General Liability, Automobile Liability, Umbrella/Excess Liability, and Professional/Errors & Omissions Liability where applicable. All insurance policies must be issued by companies licensed in Texas with an A.M. Best rating of at least A-VII. Harris Health must be named as an additional insured on General Liability, Automobile, and Umbrella Liability policies.

For projects under $25 million, Commercial General Liability must include a general aggregate limit of $2,000,000 and an each occurrence limit of $1,000,000. Automobile Liability coverage for bodily injury and property damage per accident is set at $1,000,000, while Workers' Compensation Employers' Liability requires $1,000,000 per accident and per disease limits. Umbrella Liability limits are $1,000,000 for each occurrence and general aggregate. Contractors must provide evidence of all insurance through unaltered certificates and waive all subrogation rights against Harris Health and its representatives. Products and completed operations insurance must be maintained for a minimum of five years after final project payment. Any insurance cancellations, material changes, or non-renewals require 30 days' written notice to Harris Health, with certified replacement policies provided within 30 days. Failure to maintain required coverage may result in contract termination at Harris Health's discretion.
