Military OneSource Program Policies and Procedures Attachment 10
SUBJECT: EXCEPTIONAL FAMILY MEMBER TRAINING RESOURCE COMPENDIUM
DATE: 08/16/23
REFERENCES: OSD Military OneSource Exceptional Family Member Program Policies and Procedures. (Attachment 04)
PURPOSE: To provide direction and guidance on Military OneSource Program and the Exceptional Family Member Program. The Contractor is responsible for ensuring Military OneSource staff and network providers adhere to the policies and procedures pertaining to the Exceptional Family Member Program. This Training Compendium will assist in referring service members and their families to the Exceptional Family Member Program.
INSTRUCTIONS FOR ENROLLMENT:
· Instructions for Completing DD Form 2792 Exceptional Family Member Medical Summary - This form is used to document a family member’s special medical needs and for enrollment in the Exceptional Family Member Program. This information assists military assignment personnel in matching the family member’s special medical needs against the availability of medical services at the projected duty station.
· Instructions for Completing DD Form 2792-1 Exceptional Family Member Special Education/Early Intervention Summary - This form is used to document the special education needs of a child with a disability, birth through age 21, and for enrollment in the Exceptional Family Member Program. This information will assist military assignment personnel in matching the child’s special education needs against the availability of educational services at the projected duty station.
Please refer families to their local EFMP Medical POCs and their EFMP Family Support for additional questions on EFMP enrollment.
EFMP ROLES:
Installation EFMP Family Support providers can support families from any branch of Service. Service-specific points of contact for EFMP Family Support are:
· Army – EFMP Manager at Army Community Service
· Marine Corps – EFMP Family Case Worker at Marine Corps Community Services
· Navy – EFMP Liaison at Fleet and Family Support Centers
· Air Force – EFMP Family Support Coordinator at Airman and Family
PROGRAM RESOURCES
TITLE: DoD Instruction 1315.19, "Exceptional Family Member Program", June 23, 2023 SUMMARY: This Instruction establishes policy, provides guidance, assigns responsibilities, and prescribes procedures for the implementation of the EFMP, including identifying family members with special needs that meet the DoD criteria for enrollment, coordinating assignments in the Continental United States , authorizing family member travel at government expense for active duty Service members who are assigned overseas and who have family members that meet the DoD criteria for identifying a family member with special needs, processing civilian employees who have family members with special needs for an overseas assignment, providing family support services and establishing a system of monitoring and assigning oversight responsibilities.
· Available online resources for Service Providers working with military families with special medical and/or educational needs can be found by logging to the service provider section of EFMP & Me, the digital self-service tool available on Military OneSource.
· Visit MilitaryINSTALLATIONS to locate installation EFMP staff.
RESPITE CARE RESOURCES:
· Information about the TRICARE Extended Care Health Option for Respite Care can be found on the TRICARE ECHO website.
· Information about the Military Services individual respite care programs can be obtained directly from Service Specific EFMP POCs (see Military OneSource site for Service specific web sites and the MilitaryINSTALLATIONS website for local installation EFMP Family Support Staff contact information.
EXCEPTIONS: There are no exceptions to this policy without Government Military OneSource Program Office written approval.
