About this file

This is a technical response attachment for a state and local contract opportunity issued by the Utah Department of Health and Human Services (DHHS) for audiology services at two correctional facilities: Utah State Correctional Facility (USCF) in Salt Lake City and Central Utah Correctional Facility (CUCF) in Gunnison. The contract requires offerors to provide all-inclusive audiology services to inmates, including on-site hearing evaluations, fitting and maintenance of hearing aids, and minor repairs. Offerors must confirm their ability to meet three mandatory minimum requirements: providing a certified audiologist to administer services, delivering on-site hearing evaluations and hearing aid services, and interpreting and providing patient testing results for review.

The technical response portion of the bid requires offerors to address four evaluation criteria in responses limited to one page each: demonstrating qualifications, skills, experience, training, and management procedures for administering audiology services; describing staff training and experience in correctional facility settings with documentation of licensure and qualifications from the past three years; describing capabilities for providing a certified audiologist to oversee services and ensure compliance with licensure requirements; and outlining a detailed plan for fulfilling the scope of work, including frequency of on-site services, response time capabilities for service requests, and timeline for interpreting and submitting patient results to DHHS. The document does not include information regarding pricing, contract term, award date, or other financial terms.
