STATEMENT OF WORK
UNDERGROUND STORAGE TANK COMPLIANCE INSPECTIONS/TESTING
MALMSTROM AFB, MT
1.0 PURPOSE:
The purpose of this project is to have a private third-party inspector conduct underground storage tank (UST) compliance inspections/testing (triennial and annual) and walkthrough checklists at Malmstrom Air Force Base (MAFB) facilities in the 10th, 12th and 490th Missile Squadrons (MS) and on base in accordance with ARM 17.56 Underground Storage Tanks Petroleum and Chemical Substances and MCA Title 75, Chapter 11 Underground Storage Tanks. The contractor shall provide all plant, labor, equipment, supervision, services, and materials to execute and satisfy the requirements of this Contract.
2.0 SCOPE:
UST compliance inspectors licensed by the Montana Department of Environmental Quality (Montana DEQ) shall conduct UST compliance inspections/testing at the facilities listed below. All annual and triennial testing must be done on Montana specific RP1200 testing forms. Compliance Inspectors shall be certified for testing on the Veeder-Root ATG systems.
Inspectors shall submit copies of their current Montana DEQ Compliance Inspectors license and Veeder-Root Certifications to the Contracting Officer. The Contracting Officer will not issue a notice to proceed until he has received a copy of each inspector’s license.
The main base has 4 permitted UST systems. The details of these 4 permitted sites are listed in the following table.

Each Missile Launch Facility (LF) has one 4,000-gallon double walled fiberglass reinforced plastic (DFRP) UST containing diesel fuel for an emergency generator. The USTs at the LFs are monitored by Veeder Root 350+ TLS automatic tank gauging (ATG) as the leak detection. Fuel lines are DFRP or double walled flex (DWF). Leak detection for the USTs is by continuous interstitial liquid sensor monitoring.
Each Missile Alert Facility (MAF) has one 12,000-gallon lined steel UST except MAFs H-01 and D-01 which have a 12,000-gallon double walled steel tank. The USTs at the MAFs are monitored by Veeder Root 350+ or 450+ TLS ATG as the leak detection. Fuel lines from the USTs to the generator rooms are plastic in plastic secondary containment ducting. They are graded to drain to the tank sump and are continuously monitored for leaks using a liquid sensor in the tank sump. The steel USTs are cathodically protected by an impressed current system.
10th MS: 50 LFs: A-02 to A-11, B-02 to B-11, C-02 to C-11, D-02 to D-11, and E-02 to E-11; and 5 MAFs: A-01, B-01, C-01, D-01 and E-01.
12th MS: 50 LFs: F-02 to F-11, G-02 to G-11, H-02 to H-11, I-02 to I-11, and J- 02 to J- 11; and 5 MAFs: F-01, G-01, H-01, I-01 and J-01.
490th MS: 50 LFs: K-02 to K-11, L-02 to L-11, M-02 to M-11, N-02 to N-11, and O-02 to O-11; and 5 MAFs: K-01, L-01, M-01, N-01 and O-01.
3.0 PERFORMANCE CONTRACT LINE ITEMS:
3.1 Triennial Compliance Inspections/Testing
Work shall consist of all work necessary to conduct a UST triennial compliance inspection at main base, MAFs and LFs in accordance with Montana DEQ regulations including but not limited to:
Compliance inspection Sump test Overfill test (Alarm) Overfill test (Automatic shut off valve) Spill Bucket leak test Cathodic protection test of all steel UST’s (Typically MAF’s) Paperwork provided by Malmstrom (Review for compliance).
Work shall include submitting required reports to the Montana DEQ and copies to MAFB. All triennial testing shall be done on Montana specific RP1200 testing forms.
Work MAFB will provide cathodic protection system testing and cathodic protection system inspection documentation to the Contractor prior to start of each inspection. The Contractor shall complete and sign the applicable Montana specific RP1200 testing forms and send to the Malmstrom Tank Program Manager (341 CES/CEIEC) for signature as operator. The contractor shall submit the completed forms to the Montana DEQ within 10 days of completion of the on-site inspection.
3.2 Annual Functionality Testing
Work shall consist of all work necessary to conduct UST Annual Functionality Testing at main base, 15 MAFs and 150 LFs including:
ATG (Tank Probe functionality and ATG programming) (TLS-350+ or 450+ Veeder-Root) Sump (Manway) Sensor functionality test Interstitial Sensor functionality
Work shall include submitting required reports to the Montana DEQ and copies to MAFB. All annual testing shall be done on Montana specific RP1200 testing forms. The contractor shall submit the completed forms to the Montana DEQ within 10 days of completion of the on-site inspection.
3.3 Annual and Monthly Walkthrough
Contractor shall complete Walkthrough Checklists (annual and monthly) and provide to the Malmstrom Tank Program Manager (341 CES/CEIEC) within 10 days of completion of the on-site inspection (Attachment A: Malmstrom AFB Annual UST.pdf, Attachment B: Malmstrom AFB Monthly UST.pdf)
3.4 Security/Government Delays
It shall provide for payment for delays due to other government activities at a site or on base. Work shall be measured in one- quarter hour increments and payment made based on the contract unit hourly price for “Security/Government Delays.” See 6.9 Government Delays.
4.0 WORK SCHEDULE/WORK TIMES:
4.1 Escort Limitations: Work at the Missile Facilities may be restricted due to the requirement for escorts and the possibility that escorts may not be available at times. One escort will be provided. An additional escort may be provided depending on availability.
4.2 Work Hours: Access to missile facilities will be made available from 7:00 A.M.
(initiate process to gain site entry) to 5:00 P.M. (off site). See also 6.0 Site Security.
Working hours, including site security process time for the contractor, will normally be between the hours of 7:30 A.M. and 4:30 P.M., excluding Saturdays, Sundays and Federal Holidays.
4.3 Proposed Schedule: All inspections shall be completed within 90 days of the next inspection due date for the sites (See Attachment C).
The Contractor shall submit within seven (7) days after Notice to Proceed a proposed schedule including the sequence of work at missile sites and dates of work at each site.
Work shall be accomplished one (1) site at a time because only one Government inspector/escort will be available.
4.4 Schedule Responsibility: Scheduling shall be the responsibility of the Contractor.
Contractor shall submit a proposed weekly schedule of sites to the 341 CES/CEIEC (x7128) by 9:00 A.M. the Tuesday of each week to coordinate the schedule for the upcoming week. Failure to do so may result in the Contractor being denied access to any site. Delays of this type are at no expense to the Government.
4.5 Contractor Schedule Changes: The Contractor shall submit any proposed Missile site schedule change request to 341 CES/CEIEC (x7128) as early as possible to avoid conflicts with other activities at the site.
4.6 Government Schedule Changes: The contracting officer may direct a change in the work sequence if other missile construction or maintenance activities interfere with the original schedule. If the site cannot be worked as scheduled, it will be rescheduled at no cost to the Government. The contractor shall provide means for the contracting officer to contact him after regular working hours for notification of emergency changes to schedule. Delays due to weather, to include inability of Government Inspector to travel to the site because of Roadmaster-declared hazardous road conditions, shall result in no cost to the Government.
5.0 GENERAL REQUIREMENTS (MISSILE COMPLEX)
5.1 Utilities
No utilities will be available to the Contractor. The Contractor shall provide any necessary portable power, water, etc. All waste shall be disposed of at a properly licensed disposal facility, unless otherwise approved.
5.2 Special Work Conditions
Entry to support structures at Missile LFs to inspect leak detection systems requires the capability to climb down ladders to depths of approximately 10 feet at the LFs.
Contractor will be responsible for shoveling snow as necessary to conduct the inspections.
6.0 MISSILE FIELD SITE SECURITY:
6.1 General
6.1.1 Ensure all contract personnel protect all government property, equipment and follow established fire marshal and security directives while in Air Force restricted areas.
Anti-Terrorism (AT) Awareness Training: In accordance with DoDI 2000.1v1 AFI 10-245, Anti-terrorism (AT) Program Implementation, ensure Level 1 – AT Awareness Training is completed by all contractors within 30 days of requiring access, and annually thereafter. The training will be provided by the Government.
The objective of the training is to provide personnel with requisite knowledge necessary to remain vigilant and enable employment of AT tactics, techniques, and procedures.
Force Protection: Contractor personnel are required to abide by all real-world Force Protection Condition (FPCON) measures, random anti-terrorism measures (RAMs), shelter-in-place orders, evacuations and lock-down notices as directed.
Examples include mandatory badge and ID checks, facility single points-of-entry and searches of hand carried items.
6.1.2 Each Government Escort is only allowed to escort six personnel at one time.
If the Contractor plans to have more than six personnel on-site at one time the Contractor needs to notify the Missile Engineering Scheduler (731-7191) not later than 9:00 A.M. on Tuesday of the week prior to the scheduled workweek.
6.1.3 Government personnel will brief control methods and actions taken to enforce control in and around secure areas. Contract personnel may be removed from secure areas if classified material or information is present. Follow up briefings will be given to all personnel throughout the contract period.
6.1.4 All Contractor personnel shall be restricted to the area within the site and to those USAF controlled areas outside the site fence as designated by the Government inspector/escort. All activities of the Contractor personnel, while on site, shall be conducted in strict compliance with all site security. Notify the escort official prior to departing a site.
6.1.5 Contractor personnel can be subject to personal search upon entrance to the MAF or LF.
6.1.6 Vehicles may be searched upon entering and leaving the MAF and LF.
Contractor vehicles and equipment will be searched by Air Force personnel.
Exterior bins and compartments shall be accessible for inspection. Primary emphasis of the searches will be placed on locating explosive devices, unusual objects, intruders, and other objects of suspicious nature.
6.1.7 Upon entering and exiting a restricted area, all individuals, hand-carried items and vehicles will be subject to inspection for readily detectable contraband items IAW the 341 MW Integrated Defense Plan, 31-1. It is the responsibility of the vehicle and/or equipment operator to present items and vehicles to be searched to include ensuring exterior bins and compartments are accessible.
6.1.8 Only authorized personnel are allowed to take photographs on site.
6.1.9 The Contractor shall protect all government property and comply with all Air Force physical security regulations and procedures, per this contract and as briefed at the pre-construction conference.
6.1.10 The Contractor and Government escort/inspector on-site will be responsible for on-site security. The Contractor shall be responsible for Contractor personnel, equipment, and the accuracy and completeness of EAL data.
6.1.11 During all on-site work it is Contractor personnel responsibility to remain in visible contact with the escort/inspector. Failure to remain within the area being monitored will be a serious breach of security and safety regulations. Take any and all actions necessary to ensure compliance. The Government Inspector reserves the right to deny entry to any Contractor personnel who refuse to conform to security and/or safety regulations. Delays resulting from such entry denial shall not result in any cost to the Government.
6.1.12 Work Area Restrictions During Air Force Inspections, Exercises, And Investigations:
Contractor personnel may be required to leave the area(s) or stop working and relocate within the area during Air Force inspections and exercises. For certain Air Force actions, contractor personnel will be prohibited from entering specified areas. If personnel are already in these area(s), they may be allowed to relocate or may be required to exit the area(s) entirely. These actions can be expected to occur two (2) times for periods up to 1 hour per occurrence during the contract period. During certain actual exercises and investigations, contractor and sub-contractor personnel will be under increased surveillance although they are not working near the area affected. If the contractor personnel are not involved in the investigation, these personnel may continue with their work.
6.2 Missile Field Entry Authority List (EAL)
6.2.1 The Contractor will be issued a request for a Contractor Entry Authority List (CEAL) by the Contract Administrator. Upon receipt, compile the information required for the CEAL and submit a completed CEAL to the Contract Administrator. Until this occurs, no employees will be permitted to enter Malmstrom AFB, missile field restricted areas, MAF, LF, or free zones. Required CEAL information consists of the following: full name, date of birth, driver’s license number, social security number, height, weight, eye color, hair color, home residence and company. All contractors listed on the CEAL will be put into the Criminal Justice Information Network (CJIN) database for a background check in accordance with United States Air Force directives. Reasons for unfavorable results from the background check will be marked UNF and lined through and result in individuals being denied access to Malmstrom AFB, MAFs, LFs, and free zones. Unfavorable results are listed in AFI 31-113 and include, but are not limited to, individuals that are determined to fall into one or more of the following categories: those having outstanding wants and warrants, sex offenders, violent offenders, drug possession with intent to sell, drug distribution, those who are on probation, and those who are in a pre-release program. The definition of sex offender and violent offender can be found at Montana Code Annotated § 46-23-502. Security Forces (SF) is allotted five days to complete these checks. If an employee on a CEAL terminates his or her employment or is involved in an incident resulting in an unfavorable result, including, but not limited to those identified above, the Contractor shall immediately notify the Contracting Officer in writing.
6.2.2 Identify which personnel on the CEAL need to enter restricted areas. List all contractor personnel, to include sub-contractor personnel on the CEAL for entry to restricted areas. The CEAL will be used to accomplish the restricted area entry authority list (EAL) and will be used for entry into free zones. Keep the number of personnel to a reasonable number in order to accomplish the required project. The Contracting Officer may deny any CEAL requests with excessive numbers of workers. The contractor must submit a current State Driver’s License and requested EAL information to the Contracting Officer at least thirty (30) days prior to work/requiring access. All changes, additions, and/or deletions must be sent to the Contracting Officer in writing.
6.2.3 All vehicles to be brought onto a MAF or LF must be listed on the EAL.
Submit vehicle Make, Model and VIN to be put on the EAL. Commercial or company vehicles will be allowed access to the site provided the vehicle has a company logo, is licensed to the company (not to an individual) and listed on the EAL. Rental equipment will be allowed access to the site provided the rental companies emblems are permanently attached to the sides of the equipment. The Government inspector/escort will specify the parking location of the vehicles and equipment. Vehicles used exclusively for personnel transportation will not be permitted on site but may be parked off the fenced site.
6.3 Site Entry/Exit
6.3.1 Missile Launch Facility (LF)
6.6.1.1 Site Entry:
6.6.1.1.1 Upon arrival at the LF site, park on the access road at least fifty feet (50') from the site security gate, remain in the vehicle and wait arrival of the escort.
6.6.1.1.2 Upon arrival, the escort will identify all Contractor personnel. Present to the escort the government/State issued ID card that matches the EAL for all personnel entering the site.
6.6.1.1.3 Processing onto the LF will require approximately thirty (30) minutes and will not be considered delay time (see paragraph 1.9 GOVERNMENT
DELAYS).
6.6.1.1.4 Personnel are not authorized to bring personal cell phones on an LF.
This will be briefed by the escort official prior to entry onto the site.
6.6.1.2 Site Exit:
6.6.1.2.1 Notify the escort official prior to departing the site. At this time, personnel and vehicles may be searched.
6.6.1.2.2 Processing off site will take up to thirty (30) minutes and shall not be considered delay time.
6.6.1.2.3 All contractors are to be off-site by 5:00 pm each workday. The project superintendent shall plan accordingly to have the project at an acceptable stopping point and clean-up performed prior to processing off-site by 5:00 pm each workday.
6.6.2 Missile Alert Facility (MAF)
6.6.2.1 Site Entry:
6.6.2.1.1 Government personnel will brief control methods and actions taken to enforce control in and around secure areas. Contract personnel may be removed from secure areas if classified material or information is present. Follow up briefings will be given to all personnel throughout the contract period.
6.6.2.1.2 Upon arrival at the MAF site, park on the access road at least fifty feet (50') from the site security gate, remain in the vehicle and wait arrival of the escort official.
6.6.2.1.3 Processing onto the MAF will require approximately thirty (30) minutes and will not be considered delay time.
6.6.2.1.4 Upon authorization to enter, the escort official will escort all personnel to the Security Control Center for further identification. All personnel must remain with the authorized escort official at all times.
6.6.2.1.5 Ensure all vehicles and equipment are parked in a manner to allow response vehicles a clear route for exit and entry to/from the MAF.
6.6.2.2 Site Exit:
6.6.2.2.1 Notify the escort official prior to departing the site. At this time, personnel and vehicles may be searched.
6.6.2.2.2 Processing off site will take up to thirty (30) minutes and shall not be considered delay time.
6.6.2.2.3 All contractors are to be off-site by 5:00 pm each workday. The project superintendent shall plan accordingly to have the project at an acceptable stopping point and clean-up performed prior to processing off-site by 5:00 pm each workday.
6.9 Government Delays
6.9.1 If the Contracting Officer determines that the contractor incurred a work stoppage or security delay and the stoppage or delay was caused by the government, the contractor will be paid for lost progress at the rate in the proposal schedule. The escort/inspector official and contractor will determine government delays at the time of the government delay.
6.9.2 Thirty (30) minute time allowance occurs within the work schedule times designated in 01 00 00 3.7 WORK TIMES (i.e., 7:00 to 7:30 or 4:30 to 5:00). All delay times will be paid as a Crew-hour (Cr-Hr) regardless of the number of Contractor's employees on-site.
6.9.3 Should work at a site be completed and the Contractor mobilizes to another site during working hours, the time it takes the escort to process off the completed site (thirty (30) minutes), drive to the next site following the Air Force designated primary route at the prescribed driving speeds (25MPH on gravel and as posted all others), and process onto the next site (thirty (30) minutes), shall not be considered delay time. All delay times will be paid as a Crew-hour (Cr-Hr) regardless of the number of Contractor's employees on-site. All entry and exit times will be submitted weekly to the Contracting Officer. These entry and exit times will be used to calculate the weekly security delays to the nearest one-quarter hour.
