About this file

This is a sworn statement form required by Kentucky Revised Statute 45A.395 for bidders and offerors on state and local contract opportunities in Kentucky. The statement serves as a prerequisite for determining bidder responsibility and requires the authorized representative of the bidding entity to certify under penalty of perjury that neither the signatory nor anyone acting on behalf of the bidder has knowingly violated Kentucky's campaign finance laws, and that contract award will not result in such violations. The form includes blank fields for the company name, authorized signatory, title, office or title of the signatory, date of execution, and the specific bidder or offeror identification. The statement must be completed and submitted with bid responses to demonstrate compliance with Kentucky campaign finance requirements.

The document does not contain pricing terms, set-aside information, incumbent details, funding sources, or other financial specifications. Rather, it is a mandatory compliance and attestation document applicable to all bidders pursuing state and local contracts in Kentucky. The form emphasizes that "knowingly" is defined as awareness or should-be awareness that conduct violates campaign finance provisions. This statement is a procedural requirement independent of the specific goods, services, or construction being procured and must be executed by an authorized representative with knowledge of the bidder's campaign finance compliance status.
