About this file

This document is a Cost Sheet for Salt Lake City Bid No. SLCI26017 for Surface Treatment Chip and Slurry Aggregate, targeting the procurement of aggregate materials for road surface treatments. The City of Salt Lake City is seeking to purchase approximately 2,500 tons annually of two specific materials: Type II slurry sand and Grade A chip seal, both requiring 100 percent crushed stone, slag, or high-quality particles. The materials must be clean, free from organic matter, and comply with the 2017 APWA manual of Standard Specifications.

The cost sheet requires bidders to provide per-ton pricing for the aggregate materials, with the total estimated annual cost determining the award recipient. Bidders must comply with the city's ethics clause, which prohibits offering gifts or incentives to city employees and requires transparency in the procurement process. The document emphasizes that pricing should include all associated costs such as materials, supervision, labor, insurance, transportation, delivery, and fuel surcharges. Bidders are instructed to only input pricing in yellow-shaded cells and warned that altering formulas in green or blue cells could render their bid non-responsive.
