About this file

This is an Identification of Subcontractor/Supplier/Subconsultant Form (ISF) issued by the Cook County Office of the Chief Procurement Officer for use in conjunction with the County-Wide Bottled Drinking Water contract opportunity. The form requires contractors bidding on Cook County contracts to fully complete and submit an ISF identifying each subcontractor, supplier, or subconsultant that will be utilized on the contract. Contractors must provide detailed information for each proposed subcontractor, including authorized contacts, company addresses, telephone and fax numbers, email addresses, and estimated start and completion dates. The form requires a description of services or supplies and the total price of each subcontract. Any changes to subcontractor utilization during contract performance require submission of an updated ISF form.

The form includes explicit requirements that contractors remain fully responsible for the organization, performance, and quality of work performed by subcontractors, and that subcontractors may not use contract obligations as a reason to neglect progress on other contracts. The form also stipulates that any changes to the contract's approved Minority Business Enterprise (MBE) and Women Business Enterprise (WBE) utilization plan must be submitted separately to the Office of Contract Compliance for approval. Upon request, contractors must provide copies of all written subcontractor agreements to the Cook County Office of the Chief Procurement Officer. The form includes sections for OCPO review and verification of completeness and contractor disqualification status.
