About this file
This is an Acceptance and Disclosures Form for New Mexico Military Institute (NMMI) used in connection with a competitive sealed bidding process for a Dining Hall Floor Replacement project involving approximately 5,000 square feet of new flooring installation. The form establishes the foundational requirements and certifications that vendors must complete and sign to participate in the procurement process. Bidders must certify their acceptance of all general and special conditions, bid documents, specifications, scope of work, and contract terms, with bids remaining firm for up to 60 days to allow adequate evaluation time. The form requires vendors to disclose any conflicts of interest involving NMMI employees or board members, certify non-collusion under penalty of perjury, and confirm their debarment and suspension status with the Federal Government and any state or local agencies.
In addition to the standard certifications, vendors must complete a Campaign Contribution Disclosure Form as required by New Mexico Procurement Code Sections 13-1-28 et seq. and 13-1-191.1, disclosing whether the prospective contractor, family members, or representatives have made campaign contributions exceeding $250 in aggregate to applicable public officials within two years prior to proposal submission. Vendors must either itemize all qualifying contributions with dates, amounts, nature, and purpose, or certify that no contributions meeting the threshold were made. Failure to submit a fully completed disclosure statement or making campaign contributions during the procurement process pendency may result in solicitation cancellation or contract termination. All authorized representatives must sign and date the form, certifying that they have read and understand these requirements and possess the authority to bind and certify compliance for the vendor entity.
