About this file

This is a Drug-Free Workplace Certification Form required by the Hillsborough County Board of County Commissioners for the Delivery of Library Materials contract opportunity in Florida, governed under Florida Statute 287.087. The form establishes mandatory compliance requirements for vendors bidding on this four-year library materials delivery contract, valued at $985,450.00. Vendors must certify their adherence to drug-free workplace policies by implementing comprehensive programs that include publishing workplace drug prohibition statements, educating employees about substance abuse dangers and available assistance programs, distributing policy statements to all employees engaged in contract work, and requiring employee notification of any controlled substance convictions within five days of occurrence.

The form requires offerors to demonstrate implementation of sanction policies for drug violations and participation in rehabilitation programs where available, while maintaining good faith efforts to sustain a drug-free workplace environment throughout the contract period. The authorized company representative must sign and date the certification to confirm full compliance with all six specified requirements. This certification serves as a prerequisite to bid submission and reflects Florida's statutory mandate that vendors maintain drug-free workplaces as a condition of providing commodities or services under public contracts.
