About this file

This document is a Drug-Free Workplace Affidavit that outlines specific requirements for businesses bidding on contractual services. The affidavit mandates that responding firms take comprehensive steps to establish and maintain a drug-free workplace, including publishing statements to employees about controlled substance prohibitions, informing workers about drug abuse dangers, and providing drug counseling and rehabilitation resources.

The affidavit requires firms to distribute a drug-free workplace statement to all employees involved in the proposed contract, notify employees of workplace drug law consequences, and commit to imposing sanctions or requiring rehabilitation participation for employees convicted of drug-related offenses. Firms must also demonstrate a good faith effort to continuously maintain a drug-free work environment, with specific obligations such as notifying employers within five days of any drug law convictions and establishing clear workplace policies against controlled substance manufacturing, distribution, dispensing, possession, or use.
