About this file
This is a Drug-Free Workplace Affidavit document required for the Invitation for Bid - Demolition Services - R-11 procurement issued by Letcher County Fiscal Court in Kentucky in support of a Disaster Property Acquisition Project. The affidavit establishes mandatory workplace drug policy requirements that responding firms must implement and certify compliance with prior to contract award. Bidders must demonstrate that they have published a workplace drug prohibition statement to employees, informed staff about drug abuse dangers and available assistance programs, distributed the drug-free workplace policy to all employees engaged in the proposed services, and required employees to report any controlled substance convictions or guilty pleas within five days of occurrence.
The affidavit requires firms to impose sanctions or mandate participation in drug abuse rehabilitation programs for employees convicted of drug violations and to maintain good faith efforts in sustaining a drug-free workplace throughout contract performance. This document serves as a certification mechanism ensuring contractor compliance with federal and state drug-free workplace standards as a condition of bid submission and contract eligibility for the demolition services project.
