PROJECT SUMMARY

The contractor shall provide all labor, materials, warranties, tools, equipment, supervision and all other items as necessary to complete the work as described. The Statement of Work (SOW) is broken down as follows: section 1, work to be completed in contract, and section 2, general requirements. The contractor is responsible for producing all measurements before ordering material for project, if necessary. The contractor shall perform to the standards in the contract. The contractor shall conduct all work in a professional manner, in accordance with traditionally recognized construction practices and techniques.

1. SCOPE OF WORK:

1.1 Remove and dispose of old carpet, install luxury vinyl tile, base cove and door transition to match new vinyl tile flooring in Bldg. 9694 second & third floor and all connecting rooms with the exceptions of Room 214 and restrooms and the third-floor secure working areas.

1.1.1 Provide and install approximately 16,252 square feet of luxury vinyl tile in Bldg.

9694 second & third floor. Contractor shall provide luxury vinyl tile, dark grey shade, with 20 mil wear thickness. Tile color and style will be approved by a 773 CES representative before installation.

1.1.2 Provide and install rubber base cove and door transitions in all areas where tile will be installed. Base cove and door transition color will be approved by a 773 CES representative before installation.

1.1.2.1 Contractor will be responsible for moving stored items, furniture, shelving, or any other item necessary to allow the removal and installation of flooring.

After floor installation contractor will be responsible for returning all moved items to their original locations. EXCEPTION: US Government representatives will be responsible for moving all electronics including computers, phones, radio equipment. These items will need to be moved prior to project start date.

1.1.2.2 A 773 CE representative will inspect completed luxury vinyl tile floor project.

1.1.2.3 Area will be cleaned according to regulations so occupants can safely utilize area after floor installation.

1.1.2.4 Building will be occupied, coordination with building occupants will be necessary to minimize impact on US Government mission.

1.1.2.5 Contractor will supply their own equipment; no government equipment or assets will be used for project.

1.1.3 Protect flooring, furniture, walls, and any other materials during project to prevent damage.

1.1.4 Contractor shall haul rubbish and debris from work area.

1.2 All dimensions listed are approximate. Those submitting quotes are required to base their estimates on their own measurements from the provided drawings.

2. GENERAL REQUIREMENTS:

2.1 The Contractor shall provide all labor, supervision, management, transportation, tools, equipment, materials, and supplies necessary to perform carpet/tile removal and installation.

2.1.1 Scheduling/Define Ordered Work: All individual work orders shall identify quantity, CLINs, and delivery date in writing at time of submittal.

2.1.2 Hours of Operation: The contractor shall perform work during regularly scheduled work hours of 0730 (7:30 AM) to 1630 (4:30 PM) Monday through Friday, excluding Federal legal holidays. Some projects may require work to be performed only after hours or on weekends. CE representative will work with building manager to determine available work schedule.

2.1.3 Denied access: In the event that the Contracting Officer or the Contracting Officers

Representative determines that the Contractor’s services are not required due to disaster, alert, and unforeseen post closures, the Contractor will be notified. When an unforeseen closure occurs, the Government shall have the following options:

2.1.3.1 To require the Contractor to perform the work on the following day

(unless the following day is a Saturday, Sunday or holiday).

2.1.3.2 To reschedule the work on any day satisfactory to both parties.

2.1.4 Any personal property found by the Contractor’s employees in performance of the contract shall be turned in to the 773 Civil Engineer Structures office.

2.1.5 The Contractor’s employees shall either wear contractor provided clothing identifying the name of the company and the name of the employee or wear a badge identifying the same.

2.1.6 The Contractor shall have in place a contingency plan for potential emergencies, accidents, and incidents.

2.1.7 A notification list of personnel and organizations to be contacted by the Contractor in the event of an incident, emergency, or contingency shall be supplied to the Contracting Officer and to the Contracting Officers Representative by the contractor.

2.1.8 The Contractor shall require a safety briefing for all employees before starting work on the project per US Occupational Safety and Health

Administration (OSHA) regulations to include: electrical hazards, fire and emergency procedures, medical services, health and safety hazards, respiratory protection devices (including mask fit tests) and work procedures.

Upon completion of the final cleaning, the Contractor and the Government appointed official, shall conduct a final visual inspection of the cleaned work area. If the Government appointed official rejects the area as not meeting final cleaning requirements, the Contractor shall re-clean as necessary and have a follow-on inspection conducted with the Government appointed official. Re-cleaning and follow-up inspections shall be at the Contractor’s expense.

2.2 Definitions:

2.2.1 Contracting Officer: The Contracting Officer is the Government official with the authority to enter, administer and/or terminate contracts, and make related determinations and findings.

2.2.2 Contracting Officers Representative: The Contracting Officers Representative is an individual designated and authorized in writing by the Contracting Officer to perform specific technical and administrative functions.

2.3 Chemicals: Chemicals, including caustics and paint strippers, used in performance of this contract, shall be properly labeled by the Contractor and stored in leak-tight containers.

2.3.1 Chemical Paint Strippers: Chemical paint strippers, used in performance of this contract, shall contain no methylene chloride and shall be formulated to prevent stain, discoloration, or raising of the substrate materials.

2.3.2 Chemical Paint Stripper Neutralizer: Neutralizers for paint strippers shall be used on exteriors only and shall be compatible with the substrate and suitable for use with the chemical stripper that has been applied to the surface.

2.3.3 Heat Blower Guns: Heat blower guns shall be the flameless, electrical, paint-softener types with controls to limit temperatures to 590 degrees C (1,100 degrees F). Heat blower shall be DI (non- grounded) 120 volts, AC and shall be equipped with cone, fan, glass protector and spoon reflector nozzles.
