About this file
This document is a Vendor Disqualification Checklist that outlines the required submission components for bids and quotes related to a state or local contract opportunity. Prospective vendors must include the DQ number and title in all quotes and submissions, which must be delivered through bidnetdirect.com. The checklist specifies four required exhibits that accompany bid submissions: Exhibit B (Required Vendor Information), which must be signed in ink or electronically; Exhibit C (W-9 Request for Taxpayer ID Number), which must be completed and returned; Exhibit D (Price Proposal); and Exhibit E (Vendor Disclosure Statement). Additionally, vendors are required to review all remaining exhibits and attachments prior to submission.
This checklist serves as a preliminary qualification document designed to ensure vendor compliance with submission requirements and administrative standards. Failure to include any of the specified exhibits or documents may result in bid disqualification. The requirement for electronic signature capability on Exhibit B indicates the procurement process accommodates modern submission methods through the bidnetdirect.com platform.
