Specific:

1. Install two TCI 613-3 Broadband Dipole antennas in Bothell WA.

a. TCI documentation will also be available for review at the Mount Weather program office.

b. The contractor will design antenna foundations based on soil reports and available engineering plans provided by the manufacturer.

c. The antennas shall be installed in the areas shown in Attachments B and C as specifically outlined by the FEMA TM.

d. FEMA will supply two (2) complete TCI 613-3 antenna assemblies as sold by the manufacturer. Parts list available in the TCI documentation.

e. The new antenna installed in location #1 as indicated in Attachment B shall be installed with an RF radiation pattern to the south at 180 degrees.

f. The new antenna installed in location #2 as indicated in Attachment C shall be installed with an RF radiation pattern to the southeast at 120 degrees.

g. The antenna shall be installed following all manufacture’s requirements to ensure proper operation to the manufacture’s specifications.

h. Concrete and rebar installation shall be poured and installed to the manufacture’s specifications.

i. The newly installed antenna assemblies shall be grounded to the manufacture’s standards throughout. In addition to the manufacture’s standards, each tower and anchor point will be grounded to a #2 AWG copper ground ring.

j. The contractor will install external ground rings around the towers. The ring will be installed at a depth of 30” below the grade. The ring conductor wire will be #2 AWG copper ground wire. Ground rods will be installed at 16’ intervals throughout the ring (See attachment I). All connections made below ground will be made using exothermic welding.

k. The Contractor will bond the ground rods for each of the three anchor points to the ground ring via #2 AWG copper ground wire using exothermic welding.

l. The Contractor will clear and level a 2’ x 2’ area surrounding each anchor point. This area will be covered with Heavy Duty Weed Barrier Landscape Fabric, 1” of all-purpose gravel and topped with (16”x 8” x 4”) solid core concrete pavestones.

m. Install two (2) 10 X 14” ANSI /OSHA compliant rust-free aluminum RF Radiation Hazard signs on each tower. Signs will be 5’ high on the tower legs facing the balun and a single sign on the center wood pole. (See Attachment K).

2. Install transmission line and conduit in Bothell WA.

a. The newly installed transmission lines will follow the yellow lines from the new antenna balun to cable vault Attachment B and C. The contractor will install new transmission lines from the balun to the bulkhead (Attachment D) in the facility radio room through the cable vault (Attachments F and G). The length of the transmission lines from the radio room bulkhead to the antenna are 650' to Fan #1 and 700' to Fan #2. The cable vault is considered a confined space.

b. Mark the transmission line location between the antenna BALUN and the cable vault. While marking each feedline path the contractor will also mark any and all other service lines, pipes, and obstructions out to eight feet from center of the proposed path for the transmission lines.

c. Open a new trench from the antenna BALUN to cable vault at least 36 inches below grade throughout.

d. Install conduit from the cable vault to the BALUN of each antenna.

· Each conduit shall be 4-inch Schedule 80 electrical PVC.

· The length of the conduits based on distances are 550' to Fan #1 and 600' to Fan #2.

· The conduit shall be suitable for underground burial and ensure water tightness.

· The conduit shall maintain water tightness at the transition point(s) to the cable vault/facility.

· The conduit at the antennas shall have the 90-degree sweep elbow and protrude 24 inches above ground level and to accommodate feedline routing for termination to the antenna BALUN.

· The conduits shall be terminated, and water sealed with a rubberized boot. The boot shall have a properly sized cushion for the installed hardline feedline.

e. Install in the bulkhead two new GFE NexTeK Surge Guard PTR7AF7AF25K protectors (See attachment J.)

f. The contractor will remove and dispose of up to 7 unused transmission lines identified and marked by the TM from the cable vault to the antenna side of the bulkhead.

g. Install Andrews AVA6-50, 1 1/4” Hardline feedline from the antenna balun through the conduit and cable vault to the NexTeK Surge Guard PTR7AF7AF25K feedline protector.

h. Properly terminate the transmission line to the antenna balun (7/8” EIA Female) and NexTeK Surge Guard PTR7AF7AF25K protector (7/16 DIN Female).

i. Install new Andrews AVA5-50, 7/8” Transmission line between the NexTeK SurgeGuard PTR7AF7AF25K Feedline Protector (7/16 DIN Female) installed on the bulkhead and the antenna matrix (1 5/8 EIA Flange Connector).

j. Install a #2 AWG copper ground wire from the antenna BALUN to a 8’x 5/8” copperweld rod 2’ from the base of the wood pole.

k. Contractor will ground the Andrews AVA6-50 transmission line to the #2 AWG copper ground wire using a grounding kit installed to the manufacturer specifications. Transmission line ground kits shall be sealed from the weather to prevent water and corrosion damage to the transmission line.

3. Antenna feedline replacement/termination to the Antenna Matrix in Bothell WA.

a. Locate and remove seven out of service feedlines from between the facility bulkhead to the antenna matrix. Cables will be marked by the FEMA Technical Monitor. Cables less than 25’ in length.

b. Install new Andrews AVA5-50, 7/8” transmission lines between the NexTeK SurgeGuard PTR7AF7AF25K Feedline Protector (7/16” DIN Female) previously installed on the bulkhead and the antenna matrix (1 5/8” EIA Flange Male Connector).

c. Restore grounding and bonding as required.

4. Install two TCI 613-3 Broadband Dipole antennas in Denton TX.

a. TCI documentation will also be available for review at the Mount Weather program office.

b. The contractor will design antenna foundations based on soil reports and available engineering plans provided by the manufacturer.

c. The antennas shall be installed in the areas shown in Attachments B and C as specifically outlined by the FEMA TM.

d. FEMA will supply two (2) complete TCI 613-3 antenna assemblies as sold by the manufacturer. Parts list available in the TCI documentation.

e. The new antenna installed in location #1 as indicated in Attachment M shall be installed with an RF radiation pattern to the northeast at 60 degrees.

f. The new antenna installed in location #2 as indicated in Attachment N shall be installed with an RF radiation pattern to the northwest at 310 degrees.

g. The antenna shall be installed following all manufacture’s requirements to ensure proper operation to the manufacture’s specifications.

h. Concrete and rebar installation shall be poured and installed to the manufacture’s specifications.

i. The newly installed antenna assemblies shall be grounded to the manufacture’s standards throughout. In addition to the manufacture’s standards, each tower and anchor point will be grounded to a #2 AWG copper ground ring.

j. The contractor will install external ground rings around the towers. The ring will be installed at a depth of 30” below the grade. The ring conductor wire will be #2 AWG copper ground wire. Ground rods will be installed at 16’ intervals throughout the ring (See attachment I). All connections made below ground will be made using exothermic welding.

k. The Contractor will bond the ground rods for each of the three anchor points to the ground ring via #2 AWG copper ground wire using exothermic welding.

l. The Contractor will clear and level a 2’ x 2’ area surrounding each anchor point. This area will be covered with Heavy Duty Weed Barrier Landscape Fabric, 1” of all-purpose gravel and topped with (16”x 8” x 4”) solid core concrete pavestones.

5. Install transmission line and conduit in Denton TX.

a. The newly installed transmission lines will follow the red lines from the new antenna balun to cable vault (Attachment P and Q). The contractor will install new transmission lines from the balun to the bulkhead (Attachment S) in the facility radio room through the cable vaults (Attachment O). The length of the transmission lines from the radio room bulkhead to the antenna are 780' to Fan #1 and 900' to Fan #2. The cable vault is considered a confined space.

b. The contractor will use directional boring to pass under the sidewalk (4’) for FAN 1 and under the roadway (30’) and Upper Parking Lot (80’). See Attachment R

c. Mark the transmission lines location between the antenna BALUN and the cable vault. While marking each feedline path the contractor will also mark any and all other service lines, pipes, and obstructions out to eight feet from center of the proposed path for the transmission lines.

d. Open a new trench from the antenna BALUN to cable vault at least 36 inches below grade throughout.

e. Install conduit from the cable vault to the BALUN of each antenna.

· Each conduit shall be 4-inch Schedule 80 electrical PVC.

· The length of the conduits based on distances are 520' to Fan #1 and 460' to Fan #2.

· The conduit shall be suitable for underground burial and ensure water tightness.

· The conduit shall maintain water tightness at the transition point(s) to the cable vault/facility.

· The conduit at the antennas shall have the 90-degree sweep elbow and protrude 24 inches above ground level and to accommodate feedline routing for termination to the antenna BALUN.

· The conduits shall be terminated, and water sealed with a rubberized boot. The boot shall have a properly sized cushion for the installed hardline feedline.

f. Install in the bulkhead two new GFE NexTeK Surge Guard PTR7AF7AF25K protectors.

g. The contractor will remove and dispose of 2 unused transmission lines identified and marked by the TM from cable vault C through cable vault A to the antenna side of the bulkhead. The lines are 265’ in length and will be cut and caped in Vault C.

h. Locate and remove two out of service feedlines from between the facility bulkhead to the antenna matrix. Cables will be marked by the FEMA Technical Monitor. Cables less than 25’ in length.

i. Install Andrews AVA6-50, 1 1/4” transmission line from the antenna balun through the conduit and cable vault to the NexTeK Surge Guard PTR7AF7AF25K feedline protector.

j. Properly terminate the transmission line to the FAN antenna balun (7/8” EIA Female) and NexTeK Surge Guard PTR7AF7AF25K protector (7/16 DIN Female).

k. Install two new Andrews AVA5-50, 7/8” transmission lines between the NexTeK SurgeGuard PTR7AF7AF25K Feedline Protector (7/16 DIN Female) installed on the bulkhead and the antenna matrix (1 5/8 EIA Flange Connector).

l. Install a #2 AWG copper ground wire from the FAN antenna BALUN to a 8’x 5/8” copperweld rod 2’ from the base of the wood pole.

m. Contractor will ground the Andrews AVA6-50 transmission lines to the #2 AWG copper ground wire at the base of the wood pole using a grounding kit installed to the manufacturer specifications. Transmission line ground kits shall be sealed from the weather to prevent water and corrosion damage to the transmission line.

6. Trenches and disturbed areas restoration in Bothell WA and Denton TX.

a. All trenches, ruts or tracks created by equipment such as trucks, lifts, backhoes or cranes shall be filled in, properly compacted and raked back to the natural ground level. Any disturbed site areas must be restored to pre-construction condition.

b. Heavily reseed the disturbed areas with a grass seed mixture suitable for the region, and cover with straw or other suitable grass seed covering for the region/climate.

7. Testing in Bothell WA and Denton TX.

a. The contractor shall preform a 2 to 30 MHz VSWR sweep along with a DTF sweep and document results on the newly installed antenna/transmission line.

b. All discrepancies in the installation of the antenna/feedline assemblies found to be outside manufactures specifications shall be resolved and the testing & documentation process repeated.

Deliverables:

1. Prepare and submit status reports in Microsoft Office Suite tools and format and participate in teleconferences as required. COR must accept all plans for installation tasks to be performed to include but not limited to schedule, drawings, as built drawings, and manufacturer information. An electronic copy must be provided to the COR for distribution and coordination. Warranties will commence to the Government upon Government receipt of the equipment.

2. Foundation design based on soil and engineering plans.

3. Four fully functional TCI 613-3 Broadband Dipole antennas installed per manufacturer’s recommended standard.

4. Transmission lines installed as directed in the SOW.

5. The contractor shall preform a 2 to 30 MHz VSWR sweep along with a DTF sweep and document results on the newly installed antenna/transmission line. All sweep documentation shall be given to the FEMA TM electronically by email in JPEG format.

6. Document antenna system installation to include location, transmission line length, location, and depth and submit to the COR electronically.

7. Ground that was disturbed by the installation brought back to original state. This will be included but not limited to grading (if necessary), spreading grass seed and straw.

Completion Criteria:

This project will be considered complete when the tasks identified in the SOW have been completed, tested and the final documentation has been delivered to and accepted by the COR.

Period of Performance (POP): All work shall be completed by the contractor within 90 calendar days from the award date of the resulting order.

General Requirements:

1. The contractor shall provide all labor, personnel, equipment, tools, materials and supplies necessary for completion of this task.

2. The contractor shall schedule a start date for the project within 5 calendar days of receipt award for this project. The contractor shall coordinate with the COR and/or the Technical Monitor (TM) in advance of commencing any work.

3. No deviation or substitution of materials specified shall be done without prior approval of the COR or Contracting Officer (CO).

4. The COR will work with the contractor to determine if any inspections and/or permits are needed.

5. Contractor will ensure that all site access requests are provided to the COR NLT 72 hours from the start of the project.

6. The Contractor shall establish that FEMA NCP is the owner of record of all materials procured for this project, such that FEMA NCP may exercise all warranty and technical support rights following final acceptance.

7. All data, software, equipment, analysis results, and reports/briefings generated are the property of the United States Government and may not be released to third parties without the express written consent of the Contracting Officer Representative (COR)/Contracting Officer (CO).

8. Inspection and maintenance may require personnel to climb. Maintenance personnel will be certified to climb towers and will adhere to all safety standards. Work to be performed under this SOW requires a minimum of two (2) tower and antenna certified people to be present at the job site. The Contractor shall provide all equipment and certified personnel associated with the entire project to operate in a safe manner and adhere to all Federal, State and local safety standards, including OSHA standards. The CO/COR or facility safety officer may require a copy of the certifications prior to the start of the project.

9. Confined Space requirements: If work to be performed under this SOW requires below grade manholes and or confined space entry the contractor shall ensure all personnel entering and working within the below grade manholes or a confined space are confined space certified as required by federal, state or local regulations. Contractor is required to have at least one other confined space certified individual when operating in confined spaces. Contractor is required to have all required confined space safety equipment setup and ready for operation when required. The CO/COR or facility safety officer may require a copy of the certifications prior to the start of the project.

10. Environmental Issues: Contractor will coordinate with the COR before any work is started and/or equipment is set up on the site. Any known environmental issues will be disclosed to the Contractor prior to the start of the project. Any environmental issues identified by the Contractor will be reported to the onsite Technical Monitor or the COR.

11. Telephone: Telephone service should not be expected to be available onsite for the Contractor’s use. Contractors may use cellular phone devices onsite. Depending on the cell phone service, reception may or may not be present.

12. Restroom Facilities: There are normally no publicly accessible restroom facilities at most sites. The Contractor must provide their own restroom facilities.

13. Utilities: If the Contractor requires a generator(s) then the Contractor must provide one.

14. Contractor employees working onsite shall wear an identification badge that at minimum displays the Contractor name, the employee’s photo, and the employee’s name. All Contractor employees shall identify themselves as Contractors when their status is not readily apparent and display all identification and visitor badges in plain view above the waist at all times.

15. The Federal Government owns the hardware and associated systems. The contractor shall use only such equipment in support of requirements outlined in this SOW.

16. In accordance with this SOW, the Government shall exercise its right to review deliverables prior to formal delivery and acceptance.

17. The Contractor shall coordinate services, deliverables, and delivery schedules with the COR or his/her designee, with the understanding that all work shall be within scope of this SOW. Contractor shall submit all deliverables to the COR for inspection and acceptance.

18. All cyber identity and cyber attribute data—including data at rest, data in transit, generated data, and metadata—will always remain the property of the United States Government. None of this data will be shared or distributed, other than within the operating parameters of the program, without prior written permission from FEMA. At the conclusion of the contract, the Contractor shall return all Government-owned data to FEMA on an appropriate storage medium and will definitively purge all Contractor-owned equipment of Government-owned data.

19. Normal business hours shall be Monday thru Friday, from 8:00 a.m. to 4:30 p.m. No onsite work shall be performed outside these hours or on holidays unless prior approval is obtained from the CO or the COR. The Contractor shall coordinate with the COR to arrange access to the work site. The Contractor will be required to report to the appropriate facility and submit to (if required) vehicle searches before access will be granted. Contractor will be prepared to provide their employees’ personal information (Full Name, social security number, date of birth, place of birth, driver’s license number, and home address) to the COR 72 hours prior to beginning work so that information can be vetted with Security. For emergency work, information will be provided as soon as it is practical, but Contractors will not be able to access sites until vetting procedures are complete. Contractors will be escorted by a properly cleared/badged Government employee. The Contractor shall make the effort to cause a minimum damage to parking lots, other paved areas, items that need to be moved during the course of the project, and other areas not included in this SOW. Damage caused by the Contractor will be repaired at the Contractor’s expense.

20. All trash & debris generated during the installation including shipping containers, packing material, and removed feedline/conduit shall be collected, contained throughout the process and removed from the facility at the end of the install.

21. Contractor Required Insurance to Work on a Government Installation: The contractor shall, at its own expense, provide and maintain during the entire performance period of this contract at least the kinds and minimum amounts of insurance required in paragraph 3). below.

1). Five days after contract award, the contractor shall certify to the CO in writing that the required insurance has been obtained. The policies evidencing required insurance shall contain an endorsement to the effect that any cancellation or any material change adversely affecting the Government's interest shall not be effective (l) for such period (cont.) as the laws of the State in which this contract is to be performed prescribe, or (2) until 30 days after the insurer or the contractor gives written notice to the CO, whichever period is longer.

2). The Contractor shall insert the substance of this clause, including this paragraph 3)., in subcontracts under this contract that require work on a Government installation and shall require subcontractors to provide and maintain a copy of all subcontractors' proofs of required insurance and shall make copies available to the Contracting Officer upon request.

3). The following coverage is the minimum insurance required for work Under this contract:

(a) Workers' compensation and employer's liability. Contractors are required to comply with applicable Federal and State workers' compensation and occupational disease statutes. If occupational disease is not compensable under those statutes, it shall be covered under the employer's liability section of the insurance policy, except when contract operations are so commingled with a contractor’s commercial operations that it would not be practical to require this coverage. Employer's liability coverage of at least $100,000 shall be required, except in States with exclusive or monopolistic funds that do not permit workers' compensation to be written by private carriers.

(b) General Liability.

The CO requires bodily injury liability insurance coverage written on the comprehensive form of the policy of at least $500,000 per occurrence.
