About this file
This is an Invitation for Bids (IFB) issued by the Utah Department of Health and Human Services for optometry services at Utah State Hospital under solicitation number DHHS26051. The procurement seeks a single five-year contract with the lowest responsive and responsible bidder to provide optometry services to hospital patients. Bids must be submitted electronically through Utah Public Procurement (U3P) by the published closing date and time. Bidders are required to submit responses during the Question and Answer period, which closes on a date specified in U3P, and may propose exceptions or additions to terms during this period. The solicitation does not specify a formal bidders' conference or mandatory site visit, though bidders are encouraged to monitor U3P for any addenda that may modify the scope of work, schedule, qualification requirements, or other terms.
Bidders must meet four mandatory minimum requirements: possession of a current and valid Utah optometry license, a doctorate of optometry degree, a minimum of five years of experience as an optometrist, and at least one year of experience working directly with the mentally ill. Bidders must submit a completed cost form, and the lowest responsive bid will be selected for award. A bidder must guarantee its pricing for the entire five-year contract term, with price adjustments permitted only with 30 days' notice and supporting market analysis documentation, subject to approval by the Director of DHHS's Office of Procurement and Contract Management. Contracted support coordinators are ineligible to bid unless their existing contracts are terminated. All terms and conditions are non-negotiable after the solicitation closes, and any bid containing exceptions or additions to the standard terms will be rejected. Work may not commence until after award and contract execution.
