Contractors Bid for Public Works Form 96 Summary
This document is a standardized public works bidding form (State Form 52414, Form 96) prescribed by Indiana's State Board of Accounts and applicable to public works projects undertaken by Indiana governmental units. The form serves as the official bid submission vehicle for contractors seeking to perform public works projects for state and local agencies, including school districts such as Hamilton Southeastern Schools for projects like the varsity gym floor replacement at HSE High School. Part I of the form requires all bidders to provide essential project information including the governmental unit name, county, bidder firm details, contact information, project description, plans and specifications, and the proposed bid amount. Bidders must certify their commitment to use United States steel products as required by Indiana Code 5-16-8-2 and acknowledge non-discrimination obligations regarding employee treatment based on race, religion, color, sex, national origin, or ancestry. The form requires submission of a bond or certified check as specified in the notice of letting.
Part II of the form applies exclusively to public works projects valued at $150,000 or more and requires bidders to submit comprehensive supporting documentation under oath. This section mandates submission of an experience questionnaire detailing completed and ongoing public works projects, references, equipment availability, subcontractor information, and material sourcing documentation. A certified financial statement is mandatory and must be sufficiently detailed to allow the governmental unit to assess the bidder's financial capability to complete the project. Additionally, bidders must submit a non-collusion affidavit confirming the bid was prepared independently without coordination with competitors, and execute an oath affirming the accuracy of all bid information before a notary public. The form requires identification of all intended subcontractors, their equipment, bonding requirements, and an explanation of the contractor's performance plan including timeline, workforce allocation, and project completion strategy.
